Fbrsl1 is required for cranial neural crest development and reflects a conserved function of the human disease-associated protein.

We recently identified a rare complex syndrome with craniofacial malformations caused by truncating variants in fibrosin-like 1 (FBRSL1). To investigate the function of Fbrsl1 in craniofacial development, we used the Xenopus laevis model to study the cranial neural crest (NC). While Fbrsl1 was largely dispensable for NC induction and early migration, its loss of function impaired NC differentiation and cartilage formation. This was accompanied by increased expression of p53 and cleaved caspase-3, as well as by exon skipping in the mdm2 gene, a negative regulator of p53. Fbrsl1 may directly aff

Loss of SPECC1L in cranial neural crest cells results in increased hedgehog signaling and frontonasal dysplasia.

SPECC1L encodes a cytoskeletal scaffolding protein that interacts with filamentous actin, microtubules, and cell junctional components. In humans, autosomal dominant mutations in SPECC1L cause a syndrome characterized by craniofrontonasal anomalies including broad nasal bridge, ocular hypertelorism, prominent forehead, and cleft lip/palate. Complete loss of SPECC1L in mice on a homogenous genetic background results in perinatal lethality, accompanied by subtle cranial differences and incompletely penetrant cleft palate. This lethality limits postnatal analysis of craniofacial development. Beca

Evaluating the Genetic Overlap Between Congenital Heart Disease and Neuroblastoma Risk.

Children with congenital heart disease (CHD) have elevated neuroblastoma (NB) risk, potentially due to shared neural crest origins. We analyzed rare exonic de novo single-nucleotide variants in 702 CHD and 454 NB trios from the Neuroblastoma Epidemiology in North America Study, Gabriella Miller Kids First Program, and a published cohort. Seven genes, including CHD risk genes POGZ and LZTR1 (linked to Noonan syndrome and Schwannomatosis), showed nominal (p < 0.05) enrichment in both cohorts. CIC, IREB2, POGZ, and PCDHG cluster are associated with neurodevelopmental disorders, supporting shared

Why and How Are Infants with Hutchinson-Gilford Progeria Syndrome Born Without Severe Manifestations?

Children with Hutchinson-Gilford progeria syndrome (HGPS) are born without height and weight abnormalities, and postnatal development is delayed from two months of age. The pathophysiological manifestations of HGPS can be categorized into the three tissue systems that are primarily affected: bone and cartilage, the smooth muscular layer of the vasculature, and the dermis layer. To understand the biology of the syndrome's complications resulting from the inherited dominant mutation of the LMNA gene, HGPS has to be considered in embryogenesis. Since the development of the primarily affected HGPS

Aspergillus Endocarditis in Native Valves in Non-Traditional Hosts: A Systematic Review of a Case in a Patient with CREST Syndrome and Advanced Liver Cirrhosis.

Background: Aspergillus endocarditis is a rare but life-threatening form of infective endocarditis that typically occurs in patients with a history of cardiac surgery, prosthetic valve implantation, or profound immunosuppression. Native valve involvement in non-traditional hosts remains exceptionally rare and is diagnostically challenging. Case presentation: We describe a 56-year-old woman with CREST syndrome and advanced liver disease awaiting transplantation who developed native aortic valve endocarditis. Blood cultures and serum biomarkers (galactomannan and β-d-glucan) were also negative.

Embryological Divergence and Molecular Mechanisms in Thoracic and Abdominal Aortic Aneurysms: Bridging Developmental Biology and Clinical Insights.

Aortic aneurysms are complex, predominantly asymptomatic vascular diseases with distinct incidence patterns depending on anatomical localisation. The incidence of thoracic aortic aneurysms (TAAs) has moderately increased, whereas that of abdominal aortic aneurysms has declined, primarily due to public health measures. Undiagnosed or poorly managed aneurysms are at significant risk of progression to acute aortic syndrome, with high associated mortality. The embryological origins of the aorta may have a substantial impact on its structural, cellular, and functional heterogeneity. Specifically, s

Case Report: SPECT-CT-guided minimally invasive transverse process resection for Bertolotti syndrome.

Lumbosacral transitional vertebrae (LSTV) are a common congenital anomaly that often manifests as chronic low back or radicular pain, a condition clinically referred to as Bertolotti syndrome. One specific cause of Bertolotti syndrome is pseudoarticulation of the L5 transverse process with the sacrum or ilium due to LSTV. Although conventional magnetic resonance imaging (MRI) and computed tomography can identify structural changes, they provide limited functional information regarding sites of active arthropathy. Single-photon emission computed tomography (SPECT-CT) enables the localization of

SMARCB1-related schwannomatosis and other SMARCB1-associated phenotypes: clinical spectrum and molecular pathogenesis.

SMARCB1 is a core unit of the BAF chromatin remodelling complex and its functional impairment interferes with the self-renewal and pluripotency of stem cells, lineage commitment, cellular identity and differentiation. SMARCB1 is also an important tumour suppressor gene and somatic SMARCB1 pathogenic variants (PVs) have been detected in ~ 5% of all human cancers. Additionally, germline SMARCB1 PVs have been identified in patients with conditions as clinically diverse as Rhabdoid Tumour Predisposition Syndrome type 1 (RTPS1), schwannomatosis and neurodevelopmental disorders such as Coffin-Siris

Analysis of the clinical features of neurocristopathy-related hearing loss and how these relate to outcomes after cochlear implantation.

The clinical features of neurocristopathy-related hearing loss, and the correlation between these features and patient improvements after cochlear implantation (CI) are unknown. This study enrolled 16 children with sensorineural hearing loss due to four types of neurocristopathies, Waardenburg syndrome (WS), Noonan syndrome (NS), Kabuki syndrome (KS), and CHARGE syndrome (CS), who underwent CI. Neurodevelopmental assessment was conducted using the Gesell Developmental Schedules, ear development was evaluated using temporal bone computed tomography, and the post-CI auditory nerve response was a

NBS1 facilitates preribosomal RNA biogenesis.

Mutations in the NBS1 gene result in Nijmegen breakage syndrome (NBS), and the gene encodes NBS1 that forms a complex with MRE11 and RAD50 and participates in DNA damage repair. However, the molecular mechanism by which NBS1 mutations cause clinical phenotypes of NBS, such as craniofacial dysmorphism, is still unclear. Here, we show that NBS1 localizes at the ribosomal DNA (rDNA) loci in nucleoli and interacts with ribosomal RNA (rRNA) transcription machinery including RNA polymerase I (Pol I) and TCOF1. Loss of NBS1 impairs Pol I-dependent transcription of pre-rRNA and induces nucleolar stres

Surgical excision of Genitourinary neurofibromatosis complicated by enlarged scrotum and penis: A case report.

Neurofibromatosis, the most common neurocutaneous syndrome, is characterized by numerous neural crest tumors and abnormal skin pigmentation. Genitourinary neurofibromatosis in children is extremely rare and bladder involvement is relatively common. Few cases of neurofibromatosis reported so far involve external genitalia. Here we present a 17-year-old boy with genitourinary neurofibromatosis who has a significantly enlarged penis and scrotum. Although the patient had no obvious symptoms, a diagnosis and surgical evaluation were necessary due to the underlying malignancy and the patient's disco

Nkx2.7 is a conserved regulator of craniofacial development.

Craniofacial malformations arise from developmental defects in the head, face, and neck with phenotypes such as 22q11.2 deletion syndrome illustrating a developmental link between cardiovascular and craniofacial morphogenesis. NKX2-5 is a key cardiac transcription factor associated with congenital heart disease and mouse models of Nkx2-5 deficiency highlight roles in cardiac development. In zebrafish, nkx2.5 and nkx2.7 are paralogues in the NK4 family expressed in cardiomyocytes and pharyngeal arches. Despite shared cellular origins of cardiac and craniofacial tissues, the function of NK4 fact

Spectrum of sensitive skin in India: a collaborative expert position statement.

INTRODUCTION: Sensitive skin poses a significant dermatological concern in India, influenced by diverse climatic conditions, cultural practices, and pollution levels. These factors contribute to the prevalence of sensitive skin, which is characterized by symptoms such as redness, itching, and dryness. Sensitive skin is better understood as a distinct syndrome rather than a linear progression from dry skin to conditions like acne, atopic dermatitis, and rosacea. The condition impacts the overall quality of life of affected individuals, making it crucial to understand its spectrum and management

Review of the Pathophysiology and Clinical Manifestations of 22q11.2 Deletion and Duplication Syndromes.

22q11.2 deletion and duplication syndromes are complex genetic syndromes composed of a wide spectrum of clinical manifestations, mostly affecting cardiovascular, endocrine, neurodevelopmental, and immune functioning. 22q11.2 deletion syndrome (22q11.2 DS) is more common and widely recognized compared to the duplication counterpart. Most of the literature focuses on delineating the genetic, molecular, and clinical impact of 22q11.2 DS, and less information focuses on the 22q11.2 duplication syndrome (22q11.2 DupS). We will cover both variants in this review and shed light on the less reported a

Locked Knee and Inferior Patellar Dislocation in an Elderly Patient: A Case Report and Review of the Literature.

INTRODUCTION: Inferior patellar dislocations in elderly patients are rare and typically occur in the presence of significant patellofemoral arthritis. These dislocations often present as a locked knee, which can be mistaken for other knee pathologies. While treatment has generally involved closed reduction, there is limited literature on recurrence and the role of surgical intervention. CASE REPORT: An 85-year-old female presented with a locked right knee following a fall, unable to bear weight or extend the knee. Radiographs and MRI revealed an inferior patellar dislocation due to a superior

miR-199a functions downstream of MeCP2 in neurons of MECP2 duplication syndrome models.

Duplication of the methyl-CpG-binding protein 2 (MECP2) gene causes MECP2 duplication syndrome (MDS), a severe neurodevelopmental disorder with an unclear pathology. We previously showed that MeCP2 promotes the processing of specific microRNAs (miRNAs), including miR-199a, to regulate neuronal functions. Here, we demonstrate that neurons derived from MDS model mice and patient-induced pluripotent stem cells (iPSCs) exhibit morphological abnormalities, such as abnormal dendrite outgrowth, enlarged soma size, increased glutamatergic synapse density, and hyperactivation of the mechanistic target

Functional hemispherectomy for seizure control in encephalocraniocutaneous lipomatosis: illustrative case.

BACKGROUND: Encephalocraniocutaneous lipomatosis (ECCL) is a rare neurocutaneous syndrome composed of a spectrum of congenital cutaneous, ocular, and brain anomalies. Principal anomalies include nevus psiloliparus lesions of the scalp, ocular choristomas consisting of dermolipomas, and intracranial lipomas and arachnoid cysts. The hypothesized pathogenic basis in ECCL is a developmental mesenchymal defect involving neural crest cell derivatives. Management is typically focused on treating symptoms, directed at the organ system and supportive neurorehabilitation. Reports to date have only focus

Genetics of Skin, Hair, and Eye Color in Human Pigmentation Disorders.

Skin, hair, and eye (oculocutaneous) color is due to melanin, a pigment produced by melanocytes. This review considers processes required for pigmentation and the complex genetic network that regulates them. The first requisite is migration of neural crest-derived melanoblasts, which populate various embryonic sites, then differentiate into melanocytes or seed stem cell niches. Differentiation is marked by expression of genes essential for melanogenesis, which takes place in melanosomes and involves conversion of tyrosine into melanin. Melanosome biogenesis requires premelanosome maturation th

Gastrointestinal Hemorrhage in CREST Syndrome: A Case Report.

Systemic scleroderma is an autoimmune disorder caused by microvascular dysfunction, excessive collagen deposition, and progressive fibrosis affecting the skin and other organs. CREST syndrome, also known as limited scleroderma, is an acronym for calcinosis, Raynaud's phenomenon, esophageal dysfunction, sclerodactyly, and telangiectasias. Among patients with systemic sclerosis (SSc), anticentromere antibodies are typically associated with the CREST syndrome variant. These antibodies are considered disease-specific and are known to correlate with the extent of disease progression. The pathogenes
