Autoimmune hepatitis and primary biliary cholangitis are among a subset of autoimmune diseases defined by profound co-prevalence and genetic risk association with the IL12 pathway.

BACKGROUND AND AIMS: Distinct autoimmune diseases can share symptoms and autoantibodies, but the molecular pathways underlying this shared etiology remain poorly understood. We assessed genetic support for IL12 signaling across 27 diseases. APPROACH AND RESULTS: We identified a rare variant upstream of IL12A with large risk-increasing effects on autoimmune hepatitis [AIH; per-allele odds ratio (OR)=2.98, p=3×10-9], primary biliary cholangitis (PBC; OR=2.80, p=2×10-8), systemic sclerosis (SSc; OR=2.64, p=2×10-11), Sjogren's disease (SjD; OR=1.94, p=10-11), systemic lupus erythematosus (SLE; OR

GVHD-Associated Angiomatosis: An Overlooked Vascular Sequela of Chronic Graft-Versus-Host Disease.

Graft-versus-host disease-associated angiomatosis (GVHD-AA) is a rare clinicopathologic variant of chronic GVHD characterized by benign vascular proliferations arising within areas of sclerodermatous skin change. The entity was formally named in 2014 and presents clinically as red-to-violaceous papules or nodules, confined to fibrotic GVHD skin. We present a case of GVHD-AA in a 29-year-old man with Philadelphia chromosome-positive acute lymphoblastic leukemia, diagnosed in 2017 and treated with matched unrelated donor allogeneic peripheral blood stem cell transplantation. His posttransplant c

OBJECTIVE: Nailfold videocapillaroscopy (NVC) is a key imaging technique for assessing microcirculation in patients with Raynaud's phenomenon and is essential for the diagnosis of scleroderma-related diseases and monitoring of systemic sclerosis. Few studies have evaluated structured NVC training programs, and none have investigated their impact on medical residents. This study aimed to assess skill acquisition during the First Residential NVC Course promoted by the Italian Society of Rheumatology for first-year rheumatology trainees, held in Rome on May 29-30, 2025. METHODS: Trainees from 14

Pulmonary arterial hypertension susceptibility and mortality among patients with systemic sclerosis with the DNASE1L3 rs35677470 (Arg206Cys) variant.

OBJECTIVES: A loss-of-function variant in deoxyribonuclease 1-like 3 (DNASE1L3), encoding Arg to Cys substitution at amino acid 206 (R206C), was previously identified as a heritable susceptibility factor for systemic sclerosis (SSc). We analysed mortality and pulmonary outcomes in patients with SSc with or without the DNASE1L3 R206C variant. METHODS: Mortality, pulmonary hypertension (PH), and interstitial lung disease (ILD) were ascertained in the UTHealth Houston, Royal Free Hospital, and Johns Hopkins University cohorts. Mortality risk and precapillary PH time-to-event from SSc onset were

Reactive oxygen species-induced lipid peroxidation mediates Wnt-dependent lipodystrophy and associated skin fibrosis.

Skin fibrosis is marked by extracellular matrix accumulation and dermal white adipose tissue (DWAT), resulting in tissue stiffening. We show that sustained Wnt activation in mature dermal adipocytes induces lipodystrophy and promotes fibrotic remodeling via oxidative stress. Meta-analysis of early systemic sclerosis skin transcriptomes reveals increased reactive oxygen species (ROS) production and lipid peroxidation. In cultured mouse dermal adipocytes, Wnt activation triggers a robust increase in ROS and lipid peroxidation, which is suppressed by scavengers. In vivo, Wnt activation elevates o

Hyper-aging in systemic sclerosis: linking genome instability, mitochondrial dysfunction, and inflammaging to fibrosis.

PURPOSE OF REVIEW: Systemic sclerosis (SSc) is a progressive autoimmune disease characterized by fibrosis, vasculopathy, and immune dysregulation. Emerging evidence indicates that SSc shares multiple hallmarks of accelerated biological aging, including genomic instability, telomere attrition, mitochondrial dysfunction, cellular senescence, and chronic innate immune activation. This review summarizes recent advances published during the last 18 months supporting the concept of SSc as a disorder of maladaptive or 'hyper-aging'. RECENT FINDINGS: Recent epigenetic clock studies, including Dunedin

Artificial intelligence for lung disease quantification in systemic sclerosis-associated interstitial lung disease and other connective tissue disease-associated interstitial lung disease.

PURPOSE OF REVIEW: Systemic sclerosis-associated interstitial lung disease (SSc-ILD) is the leading cause of disease-related mortality in systemic sclerosis and the connective tissue disease-associated ILD (CTD-ILD) in which artificial intelligence imaging has advanced most rapidly. Visual high-resolution CT (HRCT) scoring is reader-dependent and limits clinical decision-making. This review summarizes clinically relevant artificial intelligence and radiomics publications from approximately the last 18 months, focusing on quantification, diagnosis and classification, response assessment, and pr

Gastrointestinal ultrasound in systemic sclerosis: A scoping review of current evidence.

Systemic sclerosis (SSc) is a rare systemic autoimmune rheumatic disease frequently associated with gastrointestinal (GI) involvement. GI manifestations are heterogeneous, may occur throughout the entire tract from the very early stages of the disease and are often associated with a significant burden of symptoms and complications, leading to high morbidity and mortality. Diagnosis often remains challenging due to the lack of sensitive instruments for early detection and the reliance on invasive or expensive diagnostic tools. In recent years, ultrasound (US) has become an increasingly importan

Scleroderma Renal Crisis-Associated Thrombotic Microangiopathy Preceding Skin Sclerosis in Diffuse Cutaneous Systemic Sclerosis: A Case Report.

A prompt and accurate diagnosis is essential because the management of thrombotic microangiopathy (TMA) depends on its underlying etiology. We report a case of diffuse cutaneous systemic sclerosis (dcSSc) in which scleroderma renal crisis-associated TMA preceded the development of overt skin sclerosis. A 53-year-old woman presented with TMA and severe renal dysfunction. She tested positive for anti-RNA polymerase III antibodies and subsequently developed skin sclerosis, leading to a diagnosis of dcSSc. Angiotensin-converting enzyme inhibitor therapy resulted in a clinical improvement in the pa

Beyond the Glands: Extraglandular Manifestations Associated With Sicca Symptoms in Systemic Sclerosis.

BACKGROUND/OBJECTIVE: Sicca symptoms, including ocular and oral dryness, are frequently reported among patients with systemic sclerosis (SSc). Yet most studies have focused on patients with associated Sjögren disease (SjD) and data on SSc patients with isolated sicca symptoms remain limited. Therefore, we aimed to (i) investigate the characteristics of SSc patients with and without sicca symptoms, after excluding individuals with associated SjD or other potential causes of dryness, and (ii) identify factors independently associated with the presence of sicca manifestations. Exploratory Machine Learning Prioritizes Shared Blood Transcriptional Candidate Genes and Immune Correlates Across Antiphospholipid Syndrome and Systemic Sclerosis.

Antiphospholipid syndrome (APS) and systemic sclerosis (SSc) are distinct autoimmune diseases, but both are characterized by immune dysregulation, persistent inflammation, and vascular injury. To explore whether they also share blood transcriptional features, we analyzed peripheral blood datasets from GEO (GSE102215, APS: nine patients/nine controls; GSE231691, SSc: 49 patients/18 controls). Differential expression analysis performed separately in each cohort using edgeR with cohort-specific fold-change thresholds and nominal p < 0.05 identified 281 genes that were altered in the same directio

Gastrointestinal Manifestations of Systemic Sclerosis: Screening and Management to Improve Outcomes.

Gastrointestinal (GI) involvement is among the most common and clinically important manifestations of systemic sclerosis (SSc), affecting up to 90% of patients and substantially contributing to morbidity and reduced quality of life. SSc-related GI disease results from microvascular injury, enteric nervous system dysfunction, and progressive smooth muscle atrophy with fibrosis, producing widespread dysmotility throughout the gastrointestinal tract. Clinical involvement can extend from the oral cavity to the anorectum, with esophageal dysfunction, gastroesophageal reflux disease (GERD), gastropa
