CCL19+ fibroblast-CCR7+ T-cell crosstalk coordinates immunofibrotic signalling networks in systemic sclerosis.

BACKGROUND: Understanding the roles of diverse fibroblast subsets is of great importance in elucidating the pathogenesis of systemic sclerosis (SSc). How the immunoregulatory function of fibroblasts contributes to the pathology of SSc remains poorly understood. OBJECTIVES: To explore how fibroblasts promote fibrosis through immunoregulatory signalling and crosstalk with immune cells in SSc. METHODS: Skin and blood samples from seven patients with SSc and four healthy control participants were analysed with single-cell RNA sequencing (scRNAseq). Key findings were confirmed by multiplex immuno

[Systemic sclerosis : A disease with multiple faces].

Systemic sclerosis (SSc) is a rare autoimmune connective tissue disease within the spectrum of rheumatologic disorders. It is characterized by a combination of vasculopathy and inflammatory fibrosis affecting the skin and various internal organs. The disease presents with heterogeneous manifestations, which can complicate diagnosis and must be carefully considered during both monitoring and treatment. In recent decades, advances in research have expanded the range of therapeutic options available for SSc. Treatment strategies need to be tailored individually, depending on the specific clinical

Gut microbiome-metabolome dysregulation in systemic sclerosis: a multi-omics study.

OBJECTIVES: The interplay between the gut microbiome (GM), plasma metabolites and systemic sclerosis (SSc) has not been systematically studied. We hypothesized that disruption at the GM-metabolome interface contributes to the pathogenesis of SSc. This study aims to investigate the faecal microbiome composition and plasma metabolite profiles in SSc patients. METHODS: To evaluate the interactions, deep shotgun metagenomic sequencing was conducted on faecal samples from 15 SSc patients and 33 healthy controls. Simultaneously, untargeted liquid chromatography-tandem mass spectrometry metabolomic

A Cooperative Release of Mitochondrial DNA From Platelets and Neutrophils Drives an Interferon Signature in Systemic Sclerosis.

OBJECTIVE: Mitochondria are organelles with a hypomethylated circular genome. Mitochondrial DNA (mtDNA) in the systemic circulation has been implicated in inflammation. This study investigates the role of circulating DNA in systemic sclerosis (SSc) and the cellular mechanisms governing its release. METHODS: Total DNA was isolated from the plasma of healthy controls (HCs) and patients with SSc. Copy numbers were analyzed for mtDNA (ATP-6) and GAPDH abundance by quantitative real-time polymerase chain reaction. mtDNA was isolated from HCs and patients with SSc. Neutrophils and platelets were in

Long-Term Outcomes of Reduced-Intensity Conditioning Hematopoietic Stem Cell Transplantation for Patients With Systemic Sclerosis With Impaired Cardiac Function.

OBJECTIVE: High-intensity conditioning autologous hematopoietic stem cell transplantation (AHSCT) is standard of care for patients with advanced systemic sclerosis (SSc). The role of reduced-intensity conditioning (RIC) before AHSCT in this population remains unclear. We conducted this study to determine the long-term outcomes of RIC AHSCT in patients with SSc and cardiac involvement. METHODS: We evaluated 42 participants who sequentially underwent RIC AHSCT 5 years after the last participant was treated. Progression was defined as reinitiation of disease-modifying antirheumatic drugs or wors

Targeting the cGAS-STING pathway for therapeutic gain in autoimmune inflammatory cutaneous diseases with a focus on systemic lupus erythematosus and systemic sclerosis.

The cGAS-STING [cyclic GMP-AMP (cGAMP) synthase (cGAS)-stimulator of interferon genes (STING)] pathway is an evolutionarily conserved pathway for sensing and responding to cytosolic double-stranded (ds)DNA, whether pathogen-associated or endogenous. Detection and binding of dsDNA by cGAS enzymatically generates the second messenger cGAMP, which in turn binds the endoplasmic reticulum-resident signalling protein STING, activating IRF3 (interferon regulatory factor 3), leading to expression of type I interferons. This pathway is necessary for both viral infection and antitumoral responses. Vario

Deficiency of NRF2 aggravates BLM-induced systemic sclerosis-associated fibrosis and inflammation in mice.

BACKGROUND: Systemic sclerosis (SSc), also called scleroderma, is a chronic autoimmune connective tissue disorder characterized by fibrosis of the skin and internal organs and vasculopathy. Its pathogenesis involves abnormal activation of fibroblasts, inflammatory response and oxidative stress. Studies have shown that nuclear factor E2-related factor 2 (NRF2), as a key transcription factor regulating oxidative stress, not only affects the process of organ fibrosis, but also dynamically modulates the inflammatory response. Data from clinical studies showed that the remarkably abnormal expressio

Soluble CD13 in systemic sclerosis: clinical observations and transcriptomic insights from peripheral blood.

BACKGROUND: Soluble CD13 (sCD13), generated through cleavage of membrane-bound CD13 by matrix metalloproteinase-14 (MMP14), exhibits proinflammatory, angiogenic, and arthritogenic properties. Its known receptors include bradykinin receptor B1 (B1R) and protease-activated receptor 4 (PAR4). Building on our previous findings that the sCD13-B1R axis contributes to fibrosis in systemic sclerosis (SSc), we investigated whether plasma sCD13 levels and gene expression by peripheral blood cells are associated with clinical features of SSc. METHODS: Plasma sCD13 levels were quantified by ELISA in SSc p

Towards effective targeted therapies in morphea.

Morphea is a rare, clinically diverse inflammatory fibrosing skin condition presenting in children and adults. More severe subtypes of morphea can result in significant cosmetic and functional morbidity and warrant early systemic therapeutic intervention to minimize the risk of permanent tissue damage. Significant challenges exist in consensus classification and the development of robust validated measures of disease activity and damage. As a consequence, there are presently no approved therapies, and widely utilized consensus treatment guidelines are also lacking. At the core of the treatment
