Comparing Novel and Legacy Health Assessment Questionnaire Scoring Methods: A Scleroderma Patient-centered Intervention Network Cohort Study.

OBJECTIVE: To compare 5 Health Assessment Questionnaire (HAQ) scoring methods to measure functional disability among people with systemic sclerosis (SSc). METHODS: Scleroderma Patient-centered Intervention Network (SPIN) Cohort participants completed the HAQ (20 items, 8 domains) at enrollment. We calculated (1) HAQ-Disability Index (HAQ-DI) scores, which sum the highest item score for each domain and account for the use of aids, devices, or assistance; (2) HAQ-Alternative Disability Index (HAQ-ADI) scores, which are calculated similarly but do not account for aids, devices, or assistance; an

Improving quality of care in systemic sclerosis.

Systemic sclerosis is a complex multisystem autoimmune disease associated with early mortality, disability, and impaired quality of life. This chapter reviews the quality of care delivered to people with systemic sclerosis and addresses three questions: how current practice aligns with guideline-based care, where the major gaps occur across the disease course, and which quality improvement strategies show promise. Studies from cohorts, registries, and patient surveys identify incomplete cardiopulmonary screening, underuse of rehabilitation and preventive care, and fragmented multispecialty man

Exploratory evaluation of early 68Ga-FAPI-46 PET/CT volume-based parameters in systemic sclerosis-associated interstitial lung disease.

PURPOSE: Systemic sclerosis-associated interstitial lung disease (SSc-ILD) is a severe complication of SSc, characterised by progressive fibrosis and inflammation. Current diagnostic tools do not accurately differentiate progressors from non-progressors, limiting timely intervention. 68Ga-FAPI-46 is a positron emission tomography (PET) tracer targeting fibroblast activation protein alpha (FAP), a marker of activated fibroblasts, with high affinity and retention. Optimal imaging timepoints and PET parameters for 68Ga-FAPI-46 PET/CT imaging in fibroinflammatory disorders remain undefined. This e

Spectroscopic and machine learning approaches for clinical subtyping in systemic sclerosis.

Systemic sclerosis (SSc) is a heterogeneous autoimmune disease characterized by fibrosis, vascular damage, and immune dysregulation. In this study, we evaluated the potential of Fourier-transform infrared (FTIR) spectroscopy of whole blood samples combined with multivariate and machine learning approaches to differentiate between disease subtypes and the presence of interstitial lung disease (ILD). Subtle but consistent spectral differences were observed in the amide I/II and lipid-associated regions (~ 1500-1700 cm-1 and ~ 2900 cm-1). Principal Component Analysis (PCA) revealed clear clusteri

TPI1 and TPM4 are strong candidate RNA biomarkers for systemic sclerosis.

BACKGROUND: Systemic Sclerosis (SSc) is an autoimmune rheumatic disease (ARD) with unclear aetiopathogenesis. Disease prognosis, diagnosis, and treatment are challenging, thus mandating the discovery of reliable biomarkers to improve patient care. Candidate biomarkers have been proposed in the literature, and this study aimed to validate some of them in an easily accessible tissue. METHODS: We collected peripheral blood samples from patients with SSc and other rheumatic diseases, and extracted total RNA. We assessed the expression levels of selected molecules with real-time PCR and performed s

Juvenile systemic sclerosis.

Juvenile systemic sclerosis (jSSc) is a rare multisystem autoimmune disease, representing 4-10 % of systemic sclerosis (SSc) cases, and causes significant morbidity during growth and development. While sharing features with adult-onset disease, jSSc differs in phenotype, with more frequent diffuse cutaneous involvement, overlap syndromes, distinct autoantibody profiles and different outcomes. Evidence to guide care remains limited because of disease rarity and the lack of paediatric trials. This review summarises epidemiology, classification, clinical features and outcomes of jSSc, highlightin

Atypical Presentation of Morphoea in an Elderly Male: Diagnostic Challenges in the Absence of Autoantibodies and Malignancy.

Morphoea, or localized scleroderma, is a rare autoimmune connective tissue disorder distinguished by progressive dermal fibrosis and abnormal collagen deposition. While the condition most commonly affects children and middle-aged women, it can occasionally present in elderly individuals with atypical clinical features. Characteristic presentation includes well-defined plaques of thickened, indurated skin, often accompanied by skin discolouration, dryness, or loss of hair and sweat glands. This case describes the unusual presentation of morphoea in an 83-year-old male, whose systemic and cutane

Delayed gastric emptying identifies a high-risk clinical subgroup in patients with systemic sclerosis.

OBJECTIVES: To determine the prevalence of objectively defined gastric dysmotility in SSc and to evaluate its associations with SSc clinical and immunological features and adverse outcomes (SSc-related death and/or lung transplantation). METHODS: We conducted a retrospective cohort study. All participants underwent a standardized 4-h solid gastric emptying scintigraphy. Gastric dysmotility was defined as ≥20% gastric retention at 4-h. We examined associations between gastric dysmotility and demographic, clinical and immunologic characteristics of SSc, as well as relevant clinical outcomes. R

Reactive oxygen species driven immune inflammation in systemic sclerosis: mechanisms and emerging therapeutics.

Systemic sclerosis (SSc) is an autoimmune disease characterized by vascular abnormalities, immune dysregulation, and progressive fibrosis of the skin and internal organs. Oxidative stress is thought to be a potential initiating factor in SSc vasculopathy, with sustained reactive oxygen species (ROS) production implicated in disease progression and tissue fibrosis. Immunosuppressants have been used to treat SSc, and there are still unmet clinical needs. This review highlights newly validated and investigational therapies targeting ROS-driven immune and cytokine pathways, which have key roles in

Analysis of the Dynamics of Clinical Parameters and Serum Angiogenic Factors in Systemic Sclerosis Patients Undergoing Tocilizumab Treatment.

Systemic sclerosis (SSc) is a systemic autoimmune disease characterised by vasculopathy and fibrosis of the skin and internal organs. In SSc, normal angiogenic processes are impaired due to abnormalities in vascular endothelial cells and pericytes. Nailfold videocapillaroscopy (NVC) is useful for evaluating microvascular injury in SSc. Tocilizumab (TCZ), an anti-IL-6 receptor antibody, has demonstrated efficacy in SSc-associated interstitial lung disease (SSc-ILD); however, its effects on vascular abnormalities in SSc remain poorly understood. We evaluated longitudinal changes in NVC findings

Evaluation of serial cardiovascular magnetic resonance monitoring and immunosuppressive therapy in predicting outcomes in systemic sclerosis.

OBJECTIVE: Cardiac involvement is a major complication of systemic sclerosis (SSc), with myocardial fibrosis as a hallmark feature. Cardiovascular magnetic resonance (CMR) is used to detect cardiac involvement. However, the prognostic value of serial CMR monitoring and the effects of immunosuppressive therapy remain unclear. This study aimed to assess the prognostic significance of CMR parameter evolution for long-term cardiovascular outcomes and to evaluate the impact of immunosuppressive treatments on myocardial fibrosis. METHODS: Patients from a single centre who underwent two CMR scans us

Engaging the PD-1 pathway in systemic sclerosis attenuates inflammation-driven fibrosis.

OBJECTIVES: The study elucidates the impact of the programmed cell death protein 1 (PD-1) pathway on immune activation and fibrosis in diffuse cutaneous systemic sclerosis (dcSSc). METHODS: We obtained blood and skin biopsies from patients with dcSSc and healthy controls (HCs). Soluble PD-1 was measured in serum. Human recombinant PD-1 protein (PD-1:Fc) and anti-PD-1 antibodies were used to stimulate peripheral blood mononuclear cells (PBMCs), HC, and systemic sclerosis (SSc) dermal fibroblast cultures, and cocultures. PD-1-positive (PD-1pos) and PD-1-negative (PD-1neg) populations of dcSSc P

Probabilistic Clustering Using Multivariate Growth Mixture Model in Clinical Settings-A Scleroderma Example.

BACKGROUND: Scleroderma (systemic sclerosis; SSc) is a chronic autoimmune disease known for wide heterogeneity in patients' disease progression in multiple organ systems. Our goal is to guide clinical care by real-time classification of patients into clinically interpretable subpopulations based on their baseline characteristics and the temporal patterns of their disease progression. METHODS: A Bayesian multivariate growth mixture model was fit to identify subgroups of patients from the Johns Hopkins Scleroderma Center Research Registry who share similar lung function trajectories. We jointly

Exploring the optimal follow-up for systemic sclerosis patients: study protocol for a Dutch multicenter randomized controlled trial.

BACKGROUND: Currently, evidence-based guidelines for the frequency and intensity of follow-up of systemic sclerosis (SSc) patients are not available. Based on expert opinion, an annual extensive evaluation is recommended. A multidisciplinary Care Pathway that integrates this annual extensive evaluation at the Leiden University Medical Center has shown increased patient satisfaction, decreased healthcare utilization, and improved outcomes in SSc patients (1, 2). However, for a subgroup of SSc patients with relatively mild disease, this annual extensive evaluation might be redundant. Therefore,

Advancing drug development for systemic sclerosis by prioritizing findings from human genetic association studies.

OBJECTIVES: SSc is a rare rheumatological disease associated with significant morbidity and mortality. Despite significant recent international clinical trial activity, the yield of approved compounds has been disappointingly low. Our aim was to identify and prioritize potential 'druggable' targets with insights from human genetics, by integrating the available evidence with publicly available bioinformatics sources relevant for SSc drug development. METHODS: Genetic variants for SSc were identified through a search of the GWAS Catalog, and the associated-mapped genes were cross-referenced wi

Assessing digital vasculopathy in systemic sclerosis.

Digital vasculopathy (a spectrum of Raynaud's phenomenon, digital ulceration and critical ischaemia) is one of the most characteristic manifestations of systemic sclerosis (SSc). It is an area of unmet need with a major impact on quality of life: current treatments are only poorly effective. SSc-related digital vasculopathy is a result of structural as well as functional change at the level of both the microcirculation and the digital artery, explaining its severity. This review begins with a brief description of digital vasculopathy, followed by its assessment in the clinical setting, relevan
