Points to consider for reporting digital ulcers in systemic sclerosis interventional studies: An initiative from the world Scleroderma Foundation digital ulcer ad hoc committee.

BACKGROUND: Digital ulcers (DUs) are among the most painful and functionally disabling complications of systemic sclerosis (SSc), affecting up to 50% of patients. Despite their clinical relevance, interventional studies on DUs are limited and vary widely in design, definitions, and outcome reporting, hindering comparability and the development of standardized treatment approaches. OBJECTIVES: This initiative, led by an international expert group under the World Scleroderma Foundation (WSF), aims to establish points to consider for the standardized reporting of DUs in interventional studies, i

Computed tomography of interstitial lung disease in systemic sclerosis: dataset and deep learning model for pulmonary lesion segmentation.

OBJECTIVE: Systemic sclerosis-associated interstitial lung disease (SSc-ILD) is a major cause of morbidity and mortality in systemic sclerosis patients. High-resolution computed tomography (HRCT) plays a crucial role in SSc-ILD diagnosis and management. In this study, we aimed to develop and evaluate a deep learning-based automated segmentation model for quantifying SSc-ILD lesions in HRCT images and assess its clinical relevance. METHODS: We developed a convolutional neural network model to segment normal lung, established fibrosis (EF), and ground-glass opacity (GGO) in HRCT scans from 40 S

Nintedanib in systemic sclerosis-associated interstitial lung disease: real-world cohort study on tolerability and discontinuation.

OBJECTIVES: Nintedanib slows progression of systemic sclerosis-associated interstitial lung disease (SSc-ILD), but its tolerability in this patient group is a potential concern. METHODS: This multicentre study evaluated tolerability and treatment-related interruptions in SSc-ILD patients receiving nintedanib. Factors associated with interruption were analysed by logistic regression, with multivariable adjustment for potential confounders. Fisher's exact test was used when complete separation precluded logistic regression. Cox regression assessed time to first interruption. Weight changes befo

Faecal incontinence in systemic sclerosis: A narrative review.

Although associated with impaired health-related quality of life, faecal incontinence (FI) is underscreened in cases of systemic sclerosis (SSc). Its pathophysiology is complex and multifactorial, encompassing vasculopathy, fibrosis and probably (autoimmune) neuropathy. FI prevalence in SSc seems to be 5-6-fold higher than in the age- and sex-matched general population and is severe in nearly 15% of cases. FI is associated with diarrhoea, small intestinal bacterial overgrowth, constipation, urinary incontinence, anticentromere positivity and some features of vasculopathy. FI should be regularl

Raynaud's phenomenon and digital ulceration in systemic sclerosis.

Systemic sclerosis (SSc) is a vascular disease. Immune-mediated endothelial injury leading to vasculopathy characterised by a progressive obliterative microangiopathy is a key aetiopathogenic driver of SSc. This typically manifests clinically as Raynaud's phenomenon (RP), which occurs in virtually all individuals with SSc and digital ulcers (DU), which develop in around half of patients at some stage in the disease course. The term RP encompasses a constellation of clinical features associated with digital vasospasm, typically in response to cold exposure. People with SSc-DU typically report m

Reciprocal regulation of fibroblast-macrophage equilibrium governs skin integrity.

Fibroblast-macrophage crosstalk is well-established in vitro, and fibroblast-derived colony-stimulating factor 1 (CSF1) supports macrophages in select tissues. However, whether macrophages regulate fibroblasts in vivo remains unknown. Leveraging genetic mouse models, single-cell multi-omics, flow cytometry and imaging, we show that fibroblast depletion or loss of fibroblast-derived growth factors impacts skin macrophage populations in the dermis and hypodermis. Conditional deletion of Csf1 in Dpt+ fibroblasts progressively decreases CD64+ and CD11c+ macrophages, impairing skin wound healing. R

Evaluation of non-invasive techniques for the diagnosis of pulmonary complications in systemic sclerosis and scleroderma-like overlap syndromes: role of lung ultrasound.

BACKGROUND AND AIM: Pulmonary arterial hypertension (PAH) and interstitial lung disease (ILD) are the leading cause of death in Systemic Sclerosis (SSc) and severely affect patients' quality of life. Early, accurate, and non-invasive diagnostic tools are essential for timely treatment. This study investigates the role of lung ultrasonography (LUS) as a screening method for detecting pulmonary involvement in SSc. METHODS: A monocentric cross-sectional study was conducted on 72 patients diagnosed with SSc. Lung ultrasound was performed using a 1-5 MHz convex probe across 14 lung fields. B-lines

The multifaceted role of platelets in systemic sclerosis: beyond haemostasis!

PURPOSE OF REVIEW: Systemic sclerosis (SSc) is characterized by early and persistent vascular injury, immune dysregulation, and fibrosis, with a growing recognition of an excess thrombotic burden that cannot be fully explained by traditional cardiovascular risk factors. Increasing experimental and clinical evidence positions platelets as mediators at the interface of vasculopathy, inflammation, and coagulation in systemic sclerosis. This review addresses emerging platelet-driven mechanisms that extend platelet function beyond haemostasis and highlight their role as intravascular messengers cap

Recent advances in the heart and soul of primary cardiac involvement and cardiovascular disease in systemic sclerosis.

Systemic sclerosis (SSc) has direct effects on the heart. Primary SSc-associated heart involvement (pSHI) is a spectrum of disease, with myriad presentations that range from acute heart failure and myocarditis, breathlessness due to diastolic dysfunction to asymptomatic ventricular ectopy. There have been recent significant advances towards the standardized identification of pSHI with the development of both a consensus definition and classification criteria for pSHI. Such instruments are required to improve the quality of research in this area by enabling the recruitment of homogenous patient

Complement protein C1q induces endothelial apoptosis through C1qBP regulation of mitochondrial function in lung endothelial cells in systemic sclerosis.

BACKGROUND: Systemic sclerosis (SSc) is an autoimmune disease characterized by vasculopathy and fibrosis, with pulmonary complications as the leading cause of mortality. Although abnormal activation of the complement system is implicated in SSc pathogenesis, the role of complement protein C1q in pulmonary endothelial injury remains unclear. METHODS: In this study, we employed a bleomycin (BLM)-induced SSc mouse model and cultured lung microvascular endothelial cells to investigate the effect of C1q on mitochondrial function and endothelial apoptosis. We further investigated the involvement of

Identification of key macrophage-related genes in systemic sclerosis-associated interstitial lung disease based on single-cell and bulk transcriptomic data.

BACKGROUND: Systemic sclerosis-associated interstitial lung disease (SSc-ILD) is a major clinical challenge with no effective treatments. It is also the leading cause of death in patients with systemic sclerosis. Thus, understanding its underlying molecular mechanisms, particularly those related to macrophage-related gene functions, is critical to address this urgent medical need. METHODS: In this study, single-cell and transcriptomic data retrieved from a public database were analyzed to investigate the underlying molecular mechanisms of SSc-ILD. A series of comprehensive analyses was conduc

Longitudinal Assessment of the Juvenile Systemic Sclerosis Severity Score: Application in the National Registry for Childhood Onset Scleroderma.

OBJECTIVE: Juvenile-onset systemic sclerosis (jSSc) is a rare, heterogeneous pediatric autoimmune disease. Existing severity tools are often adapted from adult systemic sclerosis and lack pediatric-specific validation. The Juvenile Systemic Sclerosis Severity Score (J4S) was developed to address this gap by capturing multiorgan disease burden. This study evaluated the clinical use of the J4S using data from the National Registry for Childhood Onset Scleroderma (NRCOS). METHODS: We applied the J4S to 58 patients in the NRCOS cohort, analyzing scores at baseline and at one-, two-, and three-yea
