Intestinal Behçet's Disease in the Era of Biologics: Current Evidence and Future Perspectives.

Intestinal Behçet's disease (BD) is a rare but potentially life-threatening manifestation of Behçet's syndrome characterized by recurrent gastrointestinal symptoms and typical intestinal ulceration on endoscopic and radiological examinations. Despite advances in understanding intestinal BD, its diagnosis and management remain challenging because of heterogeneous clinical presentations, the absence of validated diagnostic biomarkers, and limited high-quality evidence to guide the optimal use and sequencing of biologic therapies. This review summarizes recent advances in the pathogenesis, diagno

Otorhinolaryngological manifestations of systemic autoimmune and autoinflammatory diseases: a comprehensive review.

In many autoimmune and autoinflammatory diseases, the head and neck are the first sites of symptom manifestation. Recognizing these early signs is crucial to prevent diagnostic delays and fatal consequences. This review addresses: (1) specific otorhinolaryngological (ENT) manifestations indicating systemic diseases (2), evidence for differentiating these from isolated ENT disorders, and (3) the multidisciplinary role in patient management. A comprehensive narrative literature review was conducted utilizing established scientific databases, including PubMed/Medline, Scopus, Web of Science, and

Acute Psychosis as the First Manifestation of Neuro-Behçet's Disease.

Behçet's disease is a chronic, recurring systemic inflammatory vasculitis of unknown etiology. If the central nervous system is involved, the condition is referred to as neuro-Behçet's disease. In addition to classical neurological involvement, patients with Behçet's disease may present with complex neurobehavioural and mental disorders, which are termed neuro-psycho-Behçet's disease. We describe a rare case of a 28-year-old man presenting with acute psychosis as the initial presentation of neuro-Behçet's disease.

Exploring gut microbiome dysbiosis in vasculitis in adults: a systematic review.

BACKGROUND AND AIM: Although studies have confirmed that disruptions in the gut microbiota (dysbiosis) contribute to the development of autoimmune disorders, the precise pathways through which the gut microbiota influences the onset and progression of vasculitis remain largely unknown. In recent years, research has increasingly concentrated on assessing the contribution of the gut microbiota to vasculitis. This systematic study sought to assess the dominant gut microbiota profiles in vasculitis patients and compare them to those of healthy individuals. METHODS: The Cochrane methodology was us

Serum zonulin levels and intestinal permeability in Behçet's syndrome: a cross-sectional study.

OBJECTIVES: Increased intestinal permeability has been implicated in the pathogenesis of autoimmune and inflammatory conditions, including Behçet syndrome (BS). Zonulin, an endogenous regulator of intestinal tight junctions, serves as a biomarker for assessing intestinal barrier integrity. We aimed to evaluate intestinal permeability in patients with BS using serum zonulin levels, a novel and validated biomarker for intestinal barrier dysfunction. METHODS: A total of 173 participants were enrolled: 87 patients with BS, 20 patients with Crohn's disease (CD), and 66 healthy controls. Patients w

The 2026 British Society for Rheumatology guideline for the treatment of polymyalgia rheumatica.

Polymyalgia rheumatica (PMR) is a common inflammatory rheumatic condition in older adults. Glucocorticoids (GCs) remain the cornerstone of treatment, but many people with PMR relapse during tapering and are exposed to prolonged GC therapy with associated toxicity. There is therefore a need for evidence-based strategies that optimize disease control while minimizing GC-related harm. This guideline was developed in line with the British Society for Rheumatology's Creating Clinical Guidelines Protocol (v6.0), using GRADE and AGREE II frameworks, and supersedes previously published 2009 BSR/BHPR P

Isolated granulomatous pulmonary artery vasculitis associated with Hughes-Stovin syndrome: a case report.

BACKGROUND: Hughes-Stovin syndrome (HSS) is an exceptionally rare vasculitis, characterized by pulmonary artery aneurysms with in situ thrombosis and is increasingly regarded as part of the vascular Behcet spectrum. Diagnosis remains challenging due to its rarity, lack of validated diagnostic criteria, and overlap with other vasculitis. CASE SUMMARY: We describe a 71-year-old male who presented with progressive dyspnoea and chest pain. Serial imaging demonstrated rapid enlargement of a main pulmonary artery aneurysm from 5 mm to 68 × 55 mm over 18 months, prompting surgical repair. Histopatho

Elderly-onset rheumatoid arthritis versus polymyalgia rheumatica: A systematic review and meta-analysis on pathophysiology, diagnosis and clinical progression.

Elderly-onset rheumatoid arthritis (EORA) and polymyalgia rheumatica (PMR) are among the most frequent inflammatory rheumatic diseases in patients over 50, yet their clinical differentiation, particularly when EORA presents with polymyalgic onset, poses a common diagnostic challenge. This systematic review aimed to identify the pathophysiological, clinical, imaging and treatment-response features that distinguish EORA from PMR. MEDLINE, CENTRAL and Scopus (inception to January 2026), with supplementary searches of LILACS, Google Scholar and trial registries (June 2026) were searched, including
