Real-World Use of Avacopan in Antineutrophil Cytoplasmic Antibody-Associated Vasculitis: Early and Long-Term Clinical, Renal, and Patient-Reported Outcomes in an Italian Rheumatology-Nephrology Network.

OBJECTIVE: Complement activation, particularly through the C5a-C5a receptor axis, plays a pivotal role in the pathogenesis of antineutrophil cytoplasmic antibody-associated vasculitis (AAV) and may represent an important therapeutic target. Avacopan, an oral C5a receptor antagonist, was the first therapy targeting this pathway to enter in clinical practice. Prospective real-world data on its long-term effectiveness, safety, and impact on quality of life (QoL) remain limited. In the context of an evolving therapeutic and regulatory landscape, such data provide valuable evidence on the clinical

Peripheral ossifying fibroma mimicking oral Kaposi's sarcoma in an immunosuppressed patient with Behçet's disease: a case report.

INTRODUCTION AND IMPORTANCE: Kaposi's sarcoma (KS) is an HHV-8-associated angioproliferative neoplasm occurring preferentially in immunosuppressed individuals, with oral involvement documented in up to 65% of HIV/AIDS patients. Several benign reactive gingival lesions, including peripheral ossifying fibroma (POF), may clinically mimic oral KS, creating a significant diagnostic challenge. In patients with established systemic KS, newly detected oral lesions carry a high risk of diagnostic anchoring bias, making HHV-8 (LANA-1) immunohistochemistry indispensable for definitive diagnosis. PRESENT

Echocardiographic characteristics and clinical features of cardiac involvement in Behçet's disease: a retrospective single-center case series.

BACKGROUND: Cardiac involvement in Behçet's disease (BD) is uncommon but potentially life-threatening and may mimic other cardiovascular disorders, complicating early diagnosis. This study aimed to characterize the clinical and echocardiographic features, treatment, pathological findings, and clinical outcomes of patients with cardiac BD, with particular emphasis on the utility of echocardiography in raising diagnostic suspicion. METHODS: We retrospectively screened 186 patients with diagnosed or suspected BD at The Second Affiliated Hospital of Jiangxi Medical College, Nanchang University be

Manifestations and predictors of vascular involvement in Behçet's disease.

BACKGROUND: Behçet´s disease (BD) is a variable vessel vasculitis in which vascular involvement (VBD) occurs in up to 40% of patients and represents a poor prognosis marker. Predictors of VBD have been poorly defined. AIMS: We aimed to characterise VBD patients and to identify predictors of this clinical subtype. METHODS: We performed a single-centre, retrospective study including patients fulfilling the 2013 International Criteria for BD, registered in the Rheumatic Diseases Portuguese Registry (Reuma.pt) between October 2014 and May 2023. VBD was defined as arterial or venous manifestation

Reduced Lumbar Lordosis in Fibromyalgia: Radiographic Evidence of Lumbar Straightening by L1-L5 Cobb Angle.

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs. OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain. DESIGN: Retrospective, cross-sectional observational study. SETTING: Rheumatology office practice. PARTICIPANTS: A consecutive series of 245 patients

Primary anetoderma as a possible preclinical cutaneous clue of Behçet's disease: evidence suggesting early microvascular-driven dermal elastolysis.

Anetoderma is a rare elastolytic dermatosis characterized by focal loss of dermal elastic fibers leading to localized flaccid, atrophic, or herniated skin lesions. Although it may occur as a primary disorder or secondary to inflammatory dermatoses, infections, or trauma, immune-mediated vascular injury and dysregulation of extracellular matrix remodeling are increasingly recognized as key pathogenetic mechanisms. Behçet's disease (BD) is a multisystem inflammatory vasculitis driven by neutrophil hyperactivation and endothelial dysfunction, but elastolytic dermatoses have not been included amon

Uveitis as a Correlate of Systemic Manifestations in Patients Diagnosed with Behçet's Disease.

PURPOSE: Uveitis is common in Behçet's disease (BD), but risk stratification of potentially severe systemic disease by ocular involvement is still impossible. Here, we aimed to determine whether the risk for systemic manifestations of BD in Patients Diagnosed with Behçet's Disease (PDBD) depended on the presence of intra-ocular inflammation in one of the largest global health research networks. METHODS: Screening > 150 million Electronic Medical Records (EMR), we compared subjects whose ICD-10 codes selected them as PDBD with or without a BD uveitis-reconcilable coding pattern. Outcomes inclu

Plant-based diet quality and risk of giant cell arteritis and polymyalgia rheumatica: findings from the E3N prospective cohort study.

OBJECTIVES: Environmental determinants of giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) remain poorly understood. We investigated whether adherence to plant-based dietary patterns (overall (PDI), healthful (hPDI), and unhealthful (uPDI)) is associated with incident GCA and PMR in a large prospective cohort. METHODS: The E3N study follows 98,995 French women since 1990. Dietary intake was assessed in 1993 using a validated food frequency questionnaire. PDI, hPDI, and uPDI were constructed following the approach of Satija et al. Incident GCA and PMR cases were identified and valid

Factors Associated with Adalimumab Initiation in Non-Infectious Uveitis: A Propensity Score Matched Analysis.

PURPOSE: To identify factors associated with adalimumab initiation in non-infectious uveitis. METHODS: This was a retrospective case-control study using multicentric data of 3204 patients. The primary outcome was whether adalimumab was initiated, and the secondary outcome was time to initiation. To investigate factors influencing the primary outcome, propensity score matching (1:3) of adalimumab recipients (n = 53, cases) to non-recipients (n = 159, controls) was done to reduce confounding by age, sex, race, anatomical category of disease, and follow-up time. Subsequently, subgroup analysis w

Therapeutic divergence between polymyalgia rheumatica and giant cell arteritis: lessons from IL-17A inhibition.

Graft-versus-host disease (GvHD) is a life-threatening complication of hematopoietic stem cell transplantation (HSCT), which is caused by allogeneic T cells recognizing molecules of the recipient as foreign. Endogenous glucocorticoids (GC) released from the adrenal gland are crucial in regulating such inflammatory diseases. Here we demonstrate that genetically engineered mice, that are largely unresponsive to GC, suffer from aggravated clinical symptoms and increased mortality after HSCT, effects that could be tempered by neutralization of IL-6. Interestingly, selective ablation of the GC rece

Glymphatic dysfunction as a potential mechanistic link to cognitive ımpairment in Behçet's disease: a hypothesis.

Behçet's disease (BD) is a chronic multisystem inflammatory disease in which cognitive impairment is increasingly recognized but remains incompletely understood. Although cognitive dysfunction is more common in patients with Neuro-Behçet's disease (NBD), it has also been reported in patients without clinically apparent neurological involvement, suggesting that mechanisms beyond focal inflammatory lesions may contribute to cognitive impairment. The glymphatic system is a brain-wide perivascular clearance pathway that maintains cerebral homeostasis by facilitating the exchange of cerebrospinal a

Whole-Body MRI of Inflammatory and Infectious Musculoskeletal Diseases: Emphasis on Chronic Nonbacterial Osteomyelitis and Systemic Inflammatory Disorders.

Whole-body magnetic resonance imaging (WBMRI) has emerged as the primary imaging tool for multifocal inflammatory and infectious musculoskeletal diseases. By extending coverage from the vertex to the toes in a single, radiation-free examination, WBMRI enables the detection of clinically occult lesions, disease-specific pattern recognition across bone marrow, joints, entheses, and muscles, and objective monitoring of disease activity and treatment response. This review describes the WBMRI findings-with detailed imaging features and distribution patterns-in chronic nonbacterial osteitis, SAPHO s
