Optic neuropathy in Behçet's disease is rare, despite wide ocular and neurological involvement. Progressive atrophy of the optic disc and severe visual loss is not uncommon in Behçet's disease; however, visual loss due to acute optic neuropathy is less well known. We report three cases of optic neuropathy in Behçet's disease. The clinical picture was variable in our patients, presenting either as acute retrobulbar optic neuritis or anterior optic neuropathy. It is interesting to note that although the neurological picture resembles multiple sclerosis, there seems to be less predilection to opt

To know the prognosis of Behçet's disease with HLA-Bw 51 antigen, comparison studies of 24 cases of Behçet's disease were investigated. The sex distribution was 14 males (58%) and 10 females (42%). The patients were grouped into three clinical types: the complete type with 11 cases (45%), the incomplete type with eight cases (33%) and the suspect type with five cases (22%), according to the criteria established by the Behçet's Disease Research Committee in Japan (1982). They were also divided into three ocular types according to the location of the inflammation: the anterior segment type, the

This paper describes the ophthalmological features of 150 patients with idiopathic retinal vasculitis, 67 of whom had isolated retinal vasculitis (RV) and 83 had RV associated with systemic inflammatory disease (RV + SID). The diagnosis of retinal vasculitis was made by ophthalmoscopy and fluorescein angiography, and patients with any identifiable cause (infection, ischaemia, or malignancy) were excluded from the study. Patients with isolated RV tended to have peripheral vascular sheathing, macular oedema, and diffuse capillary leakage. Those with RV accompanying Behçet's disease often had bra

A case of Behçet's disease with peripheral nervous system involvement is described. A 58-year-old female with a 16-year history of Behçet's disease was admitted to our hospital because of numbness in multiple areas in both the upper and lower extremities. A biopsy of the sural nerve revealed degeneration of the axons and a capillary lesion. This finding was compatible with mononeuritis multiplex. This case of Behçet's disease with biopsy-proved mononeuritis multiplex is a very rare one.

A 32-yr-old man was admitted to our clinic complaining chiefly of paroxysmal attacks of headaches and palpitations. He had had intermittent episodes of joint pains, erythema nodosum-like eruptions of the lower limbs, oral aphtha, and a fever lasting for three years. Ultrasonography, computerized tomography and magnetic resonance imaging scanning showed a mass in the left adrenal gland. A diagnosis of pheochromocytoma was made from the elevated plasma and urinary catecholamines and abnormal uptake of 131I-metaiodobenzylguanidine. The tumor was successfully removed, and the patient's plasma and

Behçet's disease is a chronic systemic disorder characterized by recurrent uveitis, aphthous lesions of the mouth, and ulcerations of the genitalia. Vision loss from ocular involvement is frequently the presenting symptom and optometrists should be aware of the various ocular signs of Behçet's disease. Management of the Behçet's patient involves systemic therapy and therefore requires comanagement with an internist and/or ophthalmologist. A case report of this infrequent type of uveitis is described along with a discussion of the care and visual prognosis of the Behçet's patient.

Serum IgA, IgC, IgM and secretory IgA levels in patients with Behçet disease were examined. Among them, serum IgA and secretory IgA levels, which increase in mucosal inflammation, were significantly higher than controls (p less than 0.001, p greater than 0.01). These results indicate that mucosal inflammation, oral and gastrointestinal inflammation, may lead to gram-negative bacterial infection and endotoxin-induced uveitis in patients with Behçet disease.

Adamantiades-Behçet disease is a well known clinical entity in the Mediterranean area. Signs and complications in these patients, arising from the macular area, are often overlooked for a prolonged period of time, because they are masked by the inflammatory signs of the anterior segment and/or the vitreous. Although blindness, in most cases, is the result of the optic disc atrophy, the macular alterations--as a sequelae of inflammation--are responsible for the low visual acuity. The authors in a retrospective clinical study of the last 100 consecutive cases of A-B disease, studied these macula

Diffuse pulmonary hemorrhages may occur in pulmonary vasculites, including Wegener disease, systemic lupus erythematosus, Behçet disease, and other systemic vasculites. The pulmonary bleeding is often the first manifestation of the disease and frequently denotes a severe form. The etiologic diagnosis can be established only by performing a surgical lung biopsy, which is often a difficult decision. Leukoclastic capillaritis of the alveolar septa is the common features shared by all the vasculites responsible for pulmonary hemorrhage.

This is a report of a long term prospective study of 13 seriously ill patients with Budd-Chiari syndrome as a result of occlusion of the hepatic veins who were treated by side to side portacaval shunt from four to 78 weeks after the onset of symptoms and who were under observation for three to 16 years. The patient population was young, ranging in age from 19 to 45 years; seven were men and six were women. The presumed cause was the use of oral contraceptives in three, polycythemia rubra vera in two, Behcet disease in one patient and unknown in seven patients. All of the 13 patients had abdomi

Four patients with palatal myoclonus (PM) were studied with magnetic resonance imaging (MRI). Increased signal intensity and bilateral enlargement of the inferior olives were recognized in two patients with bilateral PM, pontine haemorrhage and neuro-Behçet disease, and a similar olivary change on the contralateral side was noted in a case of pontine infarction with unilateral PM. These findings were consistent with the pathology. The changes were more obvious in proton density-weighted images than in T2-weighted images, which thus differ from the changes in common gliosis. The other patient w

A 32-year-old man with Behçet syndrome presented with hemoptysis and abnormal chest radiography. A 3 cm opacity was present in the apical posterior segment of the left upper lobe and the hila were enlarged with lobulated contours. Computed tomography revealed bilateral pulmonary artery aneurysms, a complication of Behçet syndrome. Computed tomography is recommended for the evaluation of patients with Behçet syndrome who have abnormal chest radiography.

A 31-year-old man, suffering from Behçet disease, was admitted to our hospital because of a palpable mass in the right hypochondrium. On testing, laboratory data showed an abnormal liver function, positive HBs antigens, and a high AFP level. An abdominal CT and a SCAG revealed large tumors in the liver. The patient died of hepatic failure 6 months after the first admission and an autopsy was performed. At autopsy, hepatocellular carcinomas were found to occupy most of the entire liver. Histologically, the residual liver showed no remarkable changes, such as liver cirrhosis or chronic hepatitis

During a period of more than 30 years, 34 patients suffering from the Adamantiades-Behçet disease have been examined and followed between at least one year till 16 years. Different treatments have been administered and particularly the immunodepressors with good results, but about all patients became drug-dependents. Long-term observations concerning the ocular and general evolution of the disease are presented and discussed. Some patients have been operated for a cataract without any inconvenience.

Behcet disease is rare in children. There are only two reports of Behcet disease in childhood, describing seven patients. Three pediatric patients are described, in whom the age of onset ranged from 6 to 11 years. Aphthous stomatitis and arthritis were present in all of the patients; genital ulcers, iridocylitis, erythema nodosum, and CNS involvement were present in two patients. Other manifestations included Stevens-Johnson-like eruption, fever of unknown origin, and testicular involvement. All of the patients responded to glucocorticoids; two were also treated with colchicine and one was tre

A woman with neuro-Behçet disease characterized by recurrent attacks of meningoencephalitis is reported. During an attack of brain stem dysfunction characterized by dysarthria, diplopia, and ataxia, the postcontrast CT showed an enhancing lesion of the upper brain stem. During a subsequent attack characterized by subcortical dementia and amnesia, magnetic resonance (MR) showed abnormal signal intensity in the hypothalamus and upper brain stem. Upon remission of her symptoms, follow-up contrast enhanced CT and MR were entirely normal. Magnetic resonance is especially useful in localizing acute

Twenty-one intraocular lesions associated with various systemic diseases in 15 patients were studied by MR imaging. The disorders included diabetes mellitus, cardiovascular disease, Behçet disease, sarcoidosis, and ankylosing spondylitis. MR was performed on a 0.5-T system using a surface-coil technique. Ophthalmoscopic visualization of the fundus was precluded by the presence of opaque media in all cases. MR was found to be effective in demonstrating intraocular bleeding, vitreous opacity, detached lesions of the posterior pole, and eyeball deformity. Surface-coil MR is a useful adjunct in th

We have observed a significant increase of zinc concentration in plasma and erythrocytes in a group of Italian Behcet patients, evidencing a rare hyperzinchemic condition associated with the disease. The increased zinc level observed does not show any relation to the presence of therapy or with the impairment of the blastogenic response to HSV1, a specific cellular defect in Behcet disease, confirmed in the present study. The possible biological meaning of these observations is briefly discussed.
