Occurrence and clinical course of immune checkpoint inhibitor-associated inflammatory arthritis and polymyalgia rheumatica-like syndromes: a systematic review and meta-analysis.

BACKGROUND: Immune checkpoint inhibitor-associated inflammatory arthritis (ICI-IA) and polymyalgia rheumatica (PMR)-like syndromes are clinically important rheumatic immune-related adverse events, but definitions vary across studies. We estimated their occurrence in real-world cohorts and summarized clinical course and management. METHODS: PubMed, Ovid MEDLINE, Embase, Web of Science Core Collection, and Cochrane CENTRAL were searched from inception through July 25, 2026. Two reviewers independently screened studies, extracted data, and assessed risk of bias, with disagreements resolved by a

The 2026 British Society for Rheumatology guideline for the treatment of polymyalgia rheumatica.

Polymyalgia rheumatica (PMR) is a common inflammatory rheumatic condition in older adults. Glucocorticoids (GCs) remain the cornerstone of treatment, but many people with PMR relapse during tapering and are exposed to prolonged GC therapy with associated toxicity. There is therefore a need for evidence-based strategies that optimize disease control while minimizing GC-related harm. This guideline was developed in line with the British Society for Rheumatology's Creating Clinical Guidelines Protocol (v6.0), using GRADE and AGREE II frameworks, and supersedes previously published 2009 BSR/BHPR P

A systematic literature review to inform the 2025 EULAR recommendations for the management of polymyalgia rheumatica and primary large vessel vasculitis: management of disease including relapse and complications.

OBJECTIVES: EULAR recommendations for polymyalgia rheumatica (PMR) and large vessel vasculitis (LVV) were published in 2015 and 2020, respectively. New evidence necessitated an update. We conducted a systematic literature review (SLR) to inform the 2025 EULAR recommendations for PMR and LVV. METHODS: An SLR was conducted on MEDLINE, EMBASE, and CENTRAL on adults with PMR, giant cell arteritis (GCA), or Takayasu arteritis (TAK), using Medical Subject Heading terms related to management strategies on 31 December 2024 (PROSPERO: CRD42024560403). Eligible studies were randomised controlled trials

The epidemiology of inflammatory rheumatic diseases in Germany in a European context: a systematic review to support workforce planning.

OBJECTIVES: The number of adults living with inflammatory rheumatic diseases (iRMDs) in a country is key for calculating the demand for rheumatologists. Our aim was to estimate the incidence and prevalence of iRMD in Germany by compiling current epidemiologic German and other European data. METHODS: A systematic review included original studies on prevalence and incidence of iRMDs in Germany and other European countries, covering study data from 2013 to the present. Considering heterogeneous methodology, we qualitatively estimated the prevalence and incidence for 11 iRMDs and extrapolated the

Inflammatory rheumatic diseases under mogamulizumab: a multicenter series of 11 cases.

BACKGROUND: Mogamulizumab (MOGA), an anti-CCR4 monoclonal antibody for mycosis fungoides (MF) and Sézary syndrome (SS), can trigger immune-mediated complications, such as rashes or endocrine disorders. Rheumatological manifestations are rare. OBJECTIVES: To describe cases of inflammatory rheumatism (IR) induced or worsened by MOGA in MF/SS patients. METHODS: A retrospective study collected cases of IR (polymyalgia rheumatica [PMR], rheumatoid arthritis [RA], spondylarthritis, or unclassified IR) in MF/SS patients treated with MOGA across centers of the French Cutaneous Lymphomas Group. Elderly-onset rheumatoid arthritis versus polymyalgia rheumatica: A systematic review and meta-analysis on pathophysiology, diagnosis and clinical progression.

Elderly-onset rheumatoid arthritis (EORA) and polymyalgia rheumatica (PMR) are among the most frequent inflammatory rheumatic diseases in patients over 50, yet their clinical differentiation, particularly when EORA presents with polymyalgic onset, poses a common diagnostic challenge. This systematic review aimed to identify the pathophysiological, clinical, imaging and treatment-response features that distinguish EORA from PMR. MEDLINE, CENTRAL and Scopus (inception to January 2026), with supplementary searches of LILACS, Google Scholar and trial registries (June 2026) were searched, including

Real-World Use of Avacopan in Antineutrophil Cytoplasmic Antibody-Associated Vasculitis: Early and Long-Term Clinical, Renal, and Patient-Reported Outcomes in an Italian Rheumatology-Nephrology Network.

OBJECTIVE: Complement activation, particularly through the C5a-C5a receptor axis, plays a pivotal role in the pathogenesis of antineutrophil cytoplasmic antibody-associated vasculitis (AAV) and may represent an important therapeutic target. Avacopan, an oral C5a receptor antagonist, was the first therapy targeting this pathway to enter in clinical practice. Prospective real-world data on its long-term effectiveness, safety, and impact on quality of life (QoL) remain limited. In the context of an evolving therapeutic and regulatory landscape, such data provide valuable evidence on the clinical

Reduced Lumbar Lordosis in Fibromyalgia: Radiographic Evidence of Lumbar Straightening by L1-L5 Cobb Angle.

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs. OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain. DESIGN: Retrospective, cross-sectional observational study. SETTING: Rheumatology office practice. PARTICIPANTS: A consecutive series of 245 patients

Plant-based diet quality and risk of giant cell arteritis and polymyalgia rheumatica: findings from the E3N prospective cohort study.

OBJECTIVES: Environmental determinants of giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) remain poorly understood. We investigated whether adherence to plant-based dietary patterns (overall (PDI), healthful (hPDI), and unhealthful (uPDI)) is associated with incident GCA and PMR in a large prospective cohort. METHODS: The E3N study follows 98,995 French women since 1990. Dietary intake was assessed in 1993 using a validated food frequency questionnaire. PDI, hPDI, and uPDI were constructed following the approach of Satija et al. Incident GCA and PMR cases were identified and valid

Therapeutic divergence between polymyalgia rheumatica and giant cell arteritis: lessons from IL-17A inhibition.

Graft-versus-host disease (GvHD) is a life-threatening complication of hematopoietic stem cell transplantation (HSCT), which is caused by allogeneic T cells recognizing molecules of the recipient as foreign. Endogenous glucocorticoids (GC) released from the adrenal gland are crucial in regulating such inflammatory diseases. Here we demonstrate that genetically engineered mice, that are largely unresponsive to GC, suffer from aggravated clinical symptoms and increased mortality after HSCT, effects that could be tempered by neutralization of IL-6. Interestingly, selective ablation of the GC rece

Whole-Body MRI of Inflammatory and Infectious Musculoskeletal Diseases: Emphasis on Chronic Nonbacterial Osteomyelitis and Systemic Inflammatory Disorders.

Whole-body magnetic resonance imaging (WBMRI) has emerged as the primary imaging tool for multifocal inflammatory and infectious musculoskeletal diseases. By extending coverage from the vertex to the toes in a single, radiation-free examination, WBMRI enables the detection of clinically occult lesions, disease-specific pattern recognition across bone marrow, joints, entheses, and muscles, and objective monitoring of disease activity and treatment response. This review describes the WBMRI findings-with detailed imaging features and distribution patterns-in chronic nonbacterial osteitis, SAPHO s

Metastatic melanoma presenting with paraneoplastic dermatomyositis mimicking polymyalgia rheumatica.

A man in his late 70s presented with 6-week bilateral shoulder and hip girdle pain, morning stiffness and significant weight loss. Although polymyalgia rheumatica was initially suspected, red flag weight loss prompted referral through the 2 week-wait cancer pathway with parallel laboratory and imaging assessment.Markedly elevated creatine kinase (>5000 U/L), proximal weakness and subtle cutaneous features raised suspicion of dermatomyositis. A CT scan then demonstrated extensive intra-abdominal and right inguinal lymphadenopathy, initially suggesting lymphoma and prompting haematology referral
