A Case of Pulmonary Asbestosis Presenting with Polymyalgia Rheumatica: A Case Report.

An 83-year-old male patient with pulmonary asbestosis was admitted to our hospital with a subacute onset of arthromyalgia affecting the shoulder girdles. He had a history of moderate asbestos exposure in the earlier part of his occupation and was diagnosed with pulmonary asbestosis at 65 years of age. The patient exhibited recent-onset stiffness and myalgia of the shoulder and finger joints. Joint ultrasound findings showed bilateral long head biceps, resulting in a diagnosis of polymyalgia rheumatica (PMR) according to the European League Against Rheumatism/American College of Rheumatology (E

Treatment of giant cell arteritis: focus on upadacitinib.

INTRODUCTION: Giant cell arteritis (GCA) is the most common primary vasculitis and is associated with substantial morbidity. Glucocorticoids play a central role in GCA, resulting in cumulative glucocorticoid exposures that far exceed those used in other rheumatic conditions. AREAS COVERED: This review summarizes current treatment strategies for GCA, with a particular focus on the Janus kinase (JAK) inhibitor upadacitinib. Evidence from randomized controlled trials, observational studies, and recent conference data is discussed, including efficacy, safety, and comparative considerations with e

PET/CT in inflammatory joint disorders: update and future trends.

Inflammatory joint disorders are highly prevalent in most countries and represent a huge physical burden to the patients as well as major costs to health systems. Clinical evaluation is the basis for a correct diagnosis and appropriate treatment but in some indications, additional imaging may be necessary for a more precise assessment before and following therapy. Functional imaging using positron emission tomography combined with computerized tomography (PET/CT) offers high sensitivity for this purpose even in clinically silent disease. [18F]-FDG PET/CT is widely available and currently the m

Rare Gain-of-Function Variants in IDH1 and IDH2 Confer Increased Risk of Immune-Mediated Rheumatic Diseases.

OBJECTIVE: Gain-of-function variants in IDH1 and IDH2 are enriched among individuals with coexisting myeloid neoplasms and autoimmune diseases. However, the association between IDH1/IDH2 variants and rheumatic diseases independent of hematologic neoplasms remains unclear. We investigated the association between rare gain-of-function and loss-of-function genetic variants in IDH1 and IDH2 and immune-mediated rheumatic diseases. METHODS: Individuals with gain-of-function and loss-of-function variants in IDH1/IDH2 were identified by whole genome sequencing in the UK Biobank and the All of Us Rese

Cough as a clinical manifestation of giant cell arteritis.

OBJECTIVES: Cough is an underrecognized and atypical manifestation of giant cell arteritis (GCA), which may lead to diagnostic delay. Literature on cough in GCA is limited and lacks comprehensive data on clinical relevance. The aim of this study was to assess the frequency of cough at diagnosis in GCA patients and to compare clinical characteristics between those with and without cough. METHOD: Consecutive patients diagnosed with GCA between 2000 and 2020 and followed for ≥ 12 months at the University Hospitals Leuven (Belgium) were included retrospectively. RESULTS: We included 398 GCA pati

18F-fluorodeoxyglucose PET/CT uptake characteristics in immune checkpoint inhibitor-induced polymyalgia rheumatica compared to idiopathic polymyalgia rheumatica.

OBJECTIVES: Polymyalgia rheumatica (PMR)-like manifestations are among the most common rheumatological immune checkpoint inhibitor (ICI)-related adverse events. 18F-fluorodeoxyglucose (FDG) PET/CT is useful for the follow-up of various solid organ cancers and for PMR assessment. The aim of this study was to describe PET/CT aspects of ICI-related PMR (ICI-PMR) in comparison with idiopathic PMR. METHODS: This was an observational, comparative, retrospective, single-centre study. Inclusion criteria were a diagnosis of ICI-PMR or idiopathic PMR and availability of an 18F-FDG PET/CT within 3 month

Periprosthetic Joint Infection With Salmonella Species 2 Years After Unicompartmental Knee Arthroplasty Treated With Debridement, Antibiotics, and Implant Retention (DAIR): A Case Report and Review of the Literature.

BACKGROUND: Unicompartmental knee arthroplasty (UKA) is a minimally invasive surgical procedure aimed at treating selected patients with osteoarthritis confined to a single compartment of the knee. The concept is to minimize soft-tissue damage retaining the natural ligaments and thus providing kinematics close to normal knee. Periprosthetic joint infection (PJI) is a potentially devastating complication following UKA. Although not a common occurrence, it can lead to prolonged hospitalization, need for repeated surgical intervention, and joint failure. Although Staphylococcus spp. consisting th

Patient-reported outcome measures in incident Polymyalgia Rheumatica: Changes after treatment initiation, short-term glucocorticoid discontinuation, and at one-year follow-up.

OBJECTIVES: The role of patient-reported outcomes measures (PROMs) in polymyalgia rheumatica (PMR) remains scarcely studied. This study evaluated PROMs at baseline, after glucocorticoid (GC) initiation, following short-term GC discontinuation, and one year after diagnosis in patients with PMR. METHODS: A prospective cohort of patients suspected of PMR was enrolled from three Danish rheumatology departments. Patients with PMR initiated GC treatment. Clinical assessment, including physicians' global assessment of disease activity (PhGA), laboratory tests, and PROMs, were performed at baseline,

Steroid-sparing strategies in polymyalgia rheumatica: a systematic review and meta-analysis of tocilizumab with practical guidance for tapering.

Polymyalgia rheumatica (PMR) is an inflammatory rheumatic condition primarily managed with glucocorticoids (GCs), but prolonged GC use carries significant adverse effects. Tocilizumab, an interleukin−6 receptor inhibitor, has emerged as a promising steroid-sparing therapy for patients with relapsing or GC-dependent PMR. We conducted a systematic review and meta-analysis according to PRISMA guidelines to assess the efficacy and safety of tocilizumab in PMR. Four randomized controlled trials (RCTs) involving 317 patients were included in the meta-analysis, and one multicentre observational cohor

Targeted small molecules in polymyalgia rheumatica: the emerging role of JAK inhibitors beyond glucocorticoids and DMARDs.

INTRODUCTION: Glucocorticoids remain the cornerstone of therapy for polymyalgia rheumatica (PMR), but their long-term use is associated with substantial toxicity. Glucocorticoid-sparing strategies, particularly anti-IL-6 therapies, are now well established. Targeted small molecules such as Janus kinase (JAK) inhibitors have emerged as potential alternatives to reduce glucocorticoid exposure. AREAS COVERED: This narrative review briefly summarizes conventional and biologic therapies in PMR and focuses on JAK inhibitors, particularly baricitinib and tofacitinib. These agents target the JAK/STAT

Superior Cluneal Nerve Entrapment: An Overlooked Cause of Low Back Pain and the Role of Nerve Blocks.

BACKGROUND: Low back pain is a leading cause of disability; superior cluneal nerve entrapment (SCN-E) is an underdiagnosed etiology. SCN-E mimics common pain generators such as lumbar radiculopathy, facet joint dysfunction, and sacroiliac joint pain, thus leading to diagnostic challenges and delayed treatment. CASE REPORTS: We describe 2 cases of SCN-E with distinct predisposing factors, including spinal scoliosis. A 74-year-old woman with a history of polymyalgia rheumatica presented with persistent low back pain and buttock pain despite standard treatments. A 77-year-old man with prior lumb

Eosinophilic Granulomatosis With Polyangiitis (EGPA) Manifesting as Giant Cell Arteritis After Initiating Treatment With Dupilumab: An Unusual Clinical Presentation.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated multisystemic small vessel inflammatory disease. Its diverse clinical presentation overlaps both within its own phases and with other disorders, complicating diagnosis. We present a case of a 60-year-old female who developed symptoms suggestive of giant cell arteritis (GCA) with polymyalgia rheumatica (PMR) shortly after starting treatment with dupilumab for severe rhinosinusitis. On further evaluation, the temporal artery biopsy was negative for arteritis, and the skin biopsy c
