INTRODUCTION: Lesotho has among the world's highest TB prevalences and the national program finding only 50% of incident cases. Lesotho underwent its first TB prevalence study in 2019 in reviewing this study, the authors recognized that the standard measures for screening and diagnosis TB often failed to detect the presence of culture positive pulmonary TB. This review seeks to determine if the systematic under detection of culture positive TB by standard methods is true among prevalence surveys in Africa.
METHODS: We conducted a review to identify TB prevalence surveys conducted in Africa fr

BACKGROUND: Tuberculosis (TB) remains the leading infectious-disease cause of death globally, with an estimated 10.7 million incident cases and 1.23 million deaths in 2024. Its persistence is driven not by microbial factors alone, but by a complex interplay of biological, psychological, and social determinants influencing exposure, vulnerability, and access to care. Despite growing recognition of these biopsychosocial dimensions, the available evidence remains fragmented across disciplines, and no previous review has comprehensively mapped their scope while simultaneously characterizing the st

RATIONALE: Near point-of-care nucleic acid amplification tests (NPOC-NAATs) are a new class of diagnostics that can be used for tuberculosis diagnosis. NPOC-NAATs are suitable for use in peripheral levels of healthcare systems as they are battery-operated instrument-based tests without special infrastructure requirements and can be performed by health care workers with basic technical skills. We evaluated the accuracy of NPOC-NAATs in diagnosing pulmonary tuberculosis using respiratory samples.
OBJECTIVES: To assess the diagnostic accuracy of NPOC-NAATs for detecting pulmonary tuberculosis us

BACKGROUND: Tuberculosis (TB) preventive treatment (TPT) is a core component of TB control. Levofloxacin was recently recommended by the World Health Organization as TPT in children exposed to multidrug-resistant tuberculosis (MDR-TB). However, the affordability and feasibility of TPT provision for governments, health services, individual households and global donors is uncertain.
METHODS: This costing analysis used empirical data collected alongside the TB-CHAMP clinical trial which investigated levofloxacin TPT in children exposed to MDR-TB in South Africa. We modelled the societal (health

BACKGROUND: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasingly recognised, yet initiating antimycobacterial therapy remains challenging. Following a three-year period of national educational activity in Italy, we re-assessed which factors Italian clinicians perceive as relevant to the decision to start treatment.
METHODS: We conducted a repeated cross-sectional web-based survey. The second wave was administered between November 2023 and February 2024 to members of the Italian SITA and IRENE networks, using the instrument administered in 2020. Participants rated the relev

BACKGROUND: Medication adherence is a persistent challenge in the treatment of multidrug-resistant tuberculosis (MDR-TB) and HIV. Routine counseling notes are an underutilized source of information on patient adherence context.
METHODS: We applied natural language processing (NLP) to routine counseling notes from a randomized controlled trial of MDR-TB/HIV treatment in South Africa. Topics were derived using topic modeling; notes were semantically matched to topics and clustered using K-means. We fitted linear mixed-effects (LME) regression models with participant-level random intercepts and

BACKGROUND: China has made substantial progress in tuberculosis (TB) control over the past decades. However, continued surveillance of drug-resistant and HIV-associated TB remains essential for optimizing future TB control strategies. This study aimed to comprehensively assess the long-term burden and future trends of TB and its major subtypes in China, with particular emphasis on extensively drug-resistant TB (XDR-TB) and HIV-associated TB.
METHODS: Data on incidence, prevalence, mortality, and disability-adjusted life years (DALYs) of TB in China from 1990 to 2023 were extracted from the Gl

BACKGROUND: Extrapulmonary tuberculosis (EPTB) accounts for a substantial proportion of the global tuberculosis (TB) burden and continues to pose major diagnostic and therapeutic challenges, particularly in low-incidence settings. Comprehensive descriptions of the clinical spectrum, diagnostic yield of available laboratory modalities, and treatment-related toxicity of EPTB in the Arabian Gulf are limited. We aimed to describe the clinical characteristics, diagnostic performance of microbiological and histopathological tests, and safety profile of first-line anti-TB therapy in adult patients wi

BACKGROUND: Although children are at a higher risk of progressing to tuberculosis (TB) disease compared with adults, research and publications addressing TB management have mostly emphasised adult disease.
METHODS: MEDLINE, Embase, CENTRAL and grey literature were searched from inception to 3 May 2021, and updated on 12 December 2024 for the systematic review and network meta-analysis (NMA). Randomised controlled trials of children and/or adolescents under 18 years with TB infection, contacts of drug-susceptible TB, without HIV, evaluating the effectiveness and safety of available regimens fo

BACKGROUND: Young children with exposure to or infection with Mycobacterium tuberculosis are at high risk of developing tuberculosis disease, and postexposure tuberculosis preventive treatment decreases this risk. Evidence is limited regarding the dosing and safety of the 12-dose once-weekly rifapentine and isoniazid regimen (3HP) for tuberculosis preventive treatment in children younger than 2 years and in children living with HIV.
OBJECTIVE: This protocol describes Tuberculosis Trials Consortium (TBTC) Study 35, which aims to establish practical age- and weight-banded rifapentine doses for

OBJECTIVE: In response to growing numbers of people with drug-resistant tuberculosis (DR-TB), South Africa was, in 2011, among the first countries to launch a national policy on the decentralisation of DR-TB management, preceding normative guidance from the World Health Organization. The objective was to understand the process of formulation of the South African policy on DR-TB service decentralisation.
DESIGN: Qualitative policy analysis. We reviewed policy-related documents to create a timeline, then conducted in-depth interviews with key individuals who had influence over or insights into

BACKGROUND: Tuberculosis (TB) remains a major global public health challenge, with Indonesia accounting for a substantial proportion of global incident disease. Although previous Indonesian studies have demonstrated the prevalence and consequences of TB-related stigma, less is known about how people receiving TB treatment experience the interconnected processes of internalised, family and public/social stigma, and how they understand the factors driving stigmatization in everyday life.
AIM: This study explored stigma and stigmatization experienced by people with TB receiving treatment in two

BACKGROUND: The safety of a 3-month weekly rifapentine-isoniazid (3HP) for tuberculosis prevention in HIV-positive pregnant women on antiretroviral treatment (ART) is unknown. As part of the WHIP3TB trial, 3HP administered once or annually for two years was compared to six months of isoniazid (6H). We report outcomes among women who became pregnant, with drug exposure in early pregnancy.
METHODS: Women were classified as exposed if conception occurred while taking study drug, and unexposed if conception occurred after the last dose. Outcomes included live birth, stillbirth or abortion. A comp

BACKGROUND: Tuberculosis (TB) remains a major global health burden, particularly in low- and middle-income countries. Persistent TB-related inflammation has been implicated in significant endocrine and metabolic dysregulation. While previous studies have documented alterations in sex hormones or lipid profiles independently, the potential interplay between these biological systems has not been systematically investigated in patients with active TB. Moreover, comprehensive evidence from sub-Saharan Africa, including Ethiopia, where the burden of TB is substantial, remains limited. Elucidating t

BACKGROUND: TB meningitis (TBM) has up to 50% mortality in people living with HIV. We investigated differences in cerebrospinal fluid (CSF) host immune responses associated with short-term mortality.
METHODS: We enrolled a prospective cohort of adults with definite, probable and possible HIV-related TBM in Kampala, Uganda. Metagenomic next-generation sequencing (mNGS) of bulk CSF RNA was used to detect co-infecting or alternate CNS pathogens and refine cohort diagnosis. Host transcriptomic profiles from the refined cohort were then compared between 14-day survivors and non-survivors.

BACKGROUND: Anemia represents a significant comorbidity in tuberculosis (TB) patients, affecting treatment outcomes and mortality. Despite Southeast Asia accounting for 46% of global TB cases, no comprehensive regional synthesis of anemia burden exists, limiting evidence-based policy development.
OBJECTIVES: This systematic review and meta-analysis aimed to estimate the pooled prevalence of anemia among tuberculosis patients across Southeast Asia and to examine variations in prevalence by geographic, demographic, and clinical characteristics.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) undermines global TB control, particularly in high-burden African settings, with high HIV co-infection rates. While Tanzania is among the 30 high-burden TB countries, data on resistance patterns at tertiary referral hospitals remain limited. We aimed to determine the prevalence and factors associated with drug-resistant TB among patients at Muhimbili National Hospital (MNH) in Tanzania.
METHODS: We conducted a retrospective cross-sectional study of 170 TB patients managed at MNH between July 2024 and July 2025.Clinical and demographic data
