Unexplained Giant Esophageal Ulceration in a Pregnant Woman With Severe Immunosuppression.

Esophageal ulceration is a well-recognized cause of morbidity in patients with advanced human immunodeficiency virus (HIV) infection, most commonly from opportunistic infections such as cytomegalovirus, herpes simplex virus, or Candida species. Idiopathic esophageal ulceration (IEU) is a diagnosis of exclusion associated with profound immunosuppression, typically with CD4 counts <50 cells/mm³. Its occurrence during pregnancy, particularly in congenital HIV infection, is exceedingly rare and presents unique diagnostic and therapeutic challenges. We report a 32-year-old pregnant woman at 21 week

Mycobacterium avium complex (MAC) elimination slopes from sputum are potential treatment response biomarkers in patients with HIV and pulmonary MAC.

BACKGROUND: Lack of quantitative and reproducible treatment response biomarkers is slowing the development of better therapy regimens for pulmonary Mycobacterium avium complex (P-MAC) disease. We hypothesised that steeper bacterial elimination rate slopes (BERSs) would be associated with faster mycobacterial cure and more favourable outcomes. METHODS: We retrospectively compared serial sputum culture (SSC) trajectories and outcomes of 29 P-MAC patients, of whom 26/29 (90%) had concurrent HIV infection, treated with guideline-based therapy (GBT) versus non-GBT regimens at a tertiary hospital i

High two-month mortality in Gabonese adults with and without tuberculosis: A prospective cohort study of patients with presumed pulmonary tuberculosis.

OBJECTIVES: Pulmonary tuberculosis (PTB) remains a leading cause of mortality in sub-Saharan Africa but often remains unconfirmed. Patient outcomes in unconfirmed TB or non-TB cases are rarely studied. We assessed 2-month outcomes and associated factors in presumed TB cases independent of TB status. METHODS: Hospitalized adults with presumed PTB in Lambaréné, Gabon, were prospectively enrolled (September 2024-January 2025). Demographic data, including a socioeconomic status (SES) score, were collected. Laboratory analyses included HIV-test, GeneXpert and mycobacterial culture. Chest X-rays (C

Association of Human Cytomegalovirus exposure with tuberculosis disease in South African adults with presumptive tuberculosis.

Recent studies suggested that human cytomegalovirus (HCMV) exposure may increase tuberculosis (TB) disease risk. We assessed the association between active HCMV infection and recent HCMV exposure with tuberculosis (TB) disease among TB-presumptive South African adults. This was a cross-sectional case-control study that utilized stored plasma and serum samples collected from adults (≥18 years) with presumptive TB self-presenting to primary care clinics in in the Kraaifontein District in Cape Town, South Africa. This study analyzed TB Cases (n = 98) who were mycobacterial culture and or GeneXper

Predictors of mortality in hospitalized patients with disseminated histoplasmosis.

INTRODUCTION: Histoplasmosis is an important opportunistic infection among People Living with HIV (PLHIV) and still presents significant mortality. Reliable prognostic markers are essential to guide early management and appropriate clinical decision-making. This study aimed to evaluate predictors of death at admission among patients with disseminated histoplasmosis. METHODS: This was a single-center retrospective cohort study including adults with HIV and disseminated histoplasmosis admitted to a Brazilian tertiary hospital between 2009 and 2023. The primary outcome was in-hospital mortality.

Renal Trajectories Across Histopathological Phenotypes in Treated HIV: A Multicenter Biopsy-Based Observational Study.

BACKGROUND: Despite widespread antiretroviral therapy (ART), kidney disease in people living with HIV remains clinically and biologically heterogeneous. Persistent immune imbalance-reflected by CD4/CD8 inversion-often coexists with virologic suppression, yet its relevance within biopsy-defined renal phenotypes is incompletely understood. We aimed to characterize clinicopathological phenotypes and longitudinal renal trajectories in treated people living with HIV using the KDIGO histopathological framework. METHODS: We conducted a retrospective multicenter cohort study of PLWH undergoing native

Effects of the COVID-19 Pandemic on Tuberculosis Outcomes in the United States: A Bayesian Analysis.

BACKGROUND: Tuberculosis (TB) notifications and deaths in the United States fluctuated substantially during the coronavirus disease 2019 (COVID-19) pandemic. We analyzed multiple data sources to understand the factors contributing to these changes and estimated future TB trends. METHODS: We identified 4 mechanisms potentially contributing to observed TB trends during 2020-2023: immigration, respiratory contact rates, rates of accurate diagnosis and treatment initiation, and mortality rates for persons experiencing TB disease. We employed a Bayesian approach to synthesize evidence on how these

Long-term Mortality Trends Among Individuals With Tuberculosis: A Retrospective Cohort Study of Individuals Diagnosed With Tuberculosis in Brazil.

BACKGROUND: Even after successful treatment, individuals surviving tuberculosis (TB) disease experience elevated mortality rates. However, there is limited evidence on how these risks vary over time and by individual characteristics. METHODS: We conducted a retrospective cohort study of individuals diagnosed with TB in Brazil, using national TB notifications and linked mortality records for 2007-2016. We estimated mortality rate ratios (MRRs) and cumulative mortality by year since TB diagnosis, compared to general population mortality matched on age, sex, year, and state. We identified clinic

Epidemiologic and Bacterial Factors Facilitating Long-term Transmission of Multidrug-Resistant Tuberculosis in Shanghai, China.

BACKGROUND: Long-term transmission of multidrug-resistant tuberculosis (MDR-TB) challenges TB control by generating new cases and enabling the emergence of extensively resistant strains. We investigated its epidemiologic and bacterial drivers in Shanghai, China. METHODS: We conducted a retrospective study of Mycobacterium tuberculosis isolates and associated epidemiological data from individuals diagnosed with rifampicin-resistant TB in Shanghai over 14 years (2004-2018). Using whole-genome sequencing, Bayesian reconstruction of transmission trees, and multivariable regression analysis to ide

Transmission of bedaquiline-resistant Mycobacterium tuberculosis in KwaZulu-Natal, South Africa.

RATIONALE: Bedaquiline is a critical component of new drug-resistant tuberculosis (TB) treatment regimens. Emergence of bedaquiline-resistant Mycobacterium tuberculosis (Mtb) threatens to undermine already limited treatment options. Understanding the evolution and transmission of bedaquiline-resistant strains is a high priority for TB control. OBJECTIVES: We evaluated the prevalence, strain genetic diversity, and transmission of bedaquiline resistance among people with drug-resistant TB. METHODS: We conducted a prospective study of all patients diagnosed with TB resistant to a fluoroquinolon

NAT2 Polymorphisms and Antituberculosis-Induced Hepatotoxicity in Thai People Living With HIV: Insights From a Pharmacogenetic-Pharmacokinetic Cohort Study.

BACKGROUND: The N-Acetyltransferase (NAT2) slow acetylator phenotype has been associated with a higher risk of isoniazid (INH)-induced hepatotoxicity. We investigated the association between NAT2 genotypes with inferred acetylator status, hepatotoxicity, and INH pharmacokinetics in Thai people living with HIV (PLWH) receiving INH-based Tuberculosis (TB) preventive or treatment regimens. METHODS: In this prospective cohort study of Thai PLWH initiating INH-based regimens; NAT2 genotyping classified participants as slow (SA), intermediate (IA), or rapid acetylators (RA). Hepatotoxicity was defi

Persistent Increased Plasmacytoid Dendritic Cells and Inflammation in People With HIV Years After Tuberculosis.

BACKGROUND: People living with human immunodeficiency virus (HIV-1) who have a history of cured tuberculosis have worse outcomes, including increased all-cause mortality rate and risk of recurrent tuberculosis. We hypothesized that persistent and global immune deficits could contribute to these outcomes in people with a history of tuberculosis. METHODS: We completed FLEX Cellular Indexing of Transcriptomes and Epitopes by Sequencing (FLEX-CITE-Seq) of peripheral blood mononuclear cells of people living with HIV with (n = 6) or without (n = 3) a history of tuberculosis at GHESKIO Centers in Ha

Tuberculosis Disease Prevalence Among People Who Smoke Illicit Drugs: A Respondent- Driven Sampling Study in the Western Cape, South Africa.

BACKGROUND: Tuberculosis (TB) transmission is heterogenous, yet high-risk populations remain poorly defined. We aimed to assess whether people who smoke drugs (PWSD) have elevated TB disease rates in a high-burden setting. METHODS: We recruited PWSD from a rural community in the Western Cape, South Africa, using respondent-driven sampling (RDS). Participants were ≥15 years old, tested positive for methamphetamine and/or methaqualone, and completed TB and HIV testing and biobehavioral surveys. We defined TB disease as culturable TB, Xpert MTB/RIF Ultra (Ultra) MTB detected with no history of T

Asymptomatic Tuberculosis in Children With Household Exposure to Mycobacterium tuberculosis.

BACKGROUND: Asymptomatic tuberculosis (TB) is common in adult community prevalence surveys. Evidence to inform our understanding of asymptomatic TB in children is lacking, because diagnostic algorithms typically include only symptomatic children. METHODS: Children aged 2-60 months, without human immunodeficiency virus (HIV), with household exposure to an adult TB patient in the last year, were enrolled in South Africa. Symptom screening, chest radiography (CXR) and induced sputum were performed. Confirmed TB (requiring positive Xpert Ultra and/or MGIT culture) and unconfirmed TB (requiring 2

Impact of the COVID-19 pandemic on tuberculosis epidemiology in California, Florida, New York, and Texas.

RATIONALE: The United States experienced a considerable decline in tuberculosis (TB) incidence in 2020 following the COVID-19 pandemic. OBJECTIVES: While TB rates have since returned to near prepandemic levels, analyzing the pandemic's impact offers insight into TB epidemiology in the United States. METHODS: Focusing on California, Florida, New York, and Texas-the 4 states with the highest TB incidence-we explored 3 potential mechanisms of pandemic-related disruption on TB epidemiology: (1) reduced immigration, (2) reduced Mtb transmission (through social distancing and other behavior change
