Maternal, Pregnancy, and Infant Outcomes in Women Treated for Multidrug-Resistant/Rifampicin-Resistant Tuberculosis With Novel and Repurposed Drugs in KwaZulu-Natal, South Africa.

BACKGROUND: There is limited information on the safety and efficacy of new and repurposed second-line anti-tuberculosis (TB) drugs in pregnant women. METHODS: Pregnant women initiating treatment for multidrug-resistant/rifampicin-resistant (MDR/RR) TB from 1 January 2018 to 31 March 2024 were included in this prospective, observational study. Maternal TB treatment and pregnancy outcomes were documented through ongoing record reviews and clinical assessments conducted to describe infant outcomes. Using Poisson regression with robust standard errors, we evaluated factors associated with favorab

Tuberculosis Infection Screening Recommendations for Targeted Immunotherapies: Comparison of US Prescribing Information, Clinical Resources, and Quality Measures.

BACKGROUND: Targeted immunotherapies have transformed the treatment of many diseases. However, some increase the risk of tuberculosis (TB) disease. We sought to develop a comprehensive list of targeted immunotherapies with TB infection screening recommendations in US Food and Drug Administration (FDA)-approved prescribing information and compare these recommendations to clinical resources and quality measures. METHODS: Through a gray literature review, we identified TB clinical resources and US quality measures. We created a list of targeted immunotherapies and TB infection screening recommen

Adverse Drug Reactions Related to Isoniazid Preventive Therapy Among Persons Living With HIV on Dolutegravir-Based Regimens in Uganda Between July 2019 and March 2020.

BACKGROUND: Isoniazid preventive therapy (IPT) is recommended for the treatment of latent tuberculosis (TB) infection and is beneficial for persons living with HIV (PLHIV) even after initiation of antiretroviral therapy (ART). After the roll out of dolutegravir (DTG)-based ART in 2018, the Uganda Ministry of Health rapidly expanded IPT coverage among PLHIV. We assessed if concomitant IPT and DTG administration (C-IPT) increased the risk of IPT-related adverse drug reactions (ADRs) compared with delayed IPT administration (D-IPT). METHODS: We abstracted data from medical records of PLHIV aged

The Effect of Rifampicin on the Plasma and Intracellular Pharmacokinetics of Tenofovir Alafenamide Fumarate in HIV-associated Tuberculosis.

BACKGROUND: Rifampicin reduces plasma tenofovir and intracellular tenofovir diphosphate (TFV-DP) concentrations when coadministered with tenofovir alafenamide fumarate (TAF). Standard-dose TAF with rifampicin still provided higher TFV-DP concentrations than tenofovir disoproxil fumarate (TDF) in a healthy volunteer study, but this interaction has not been assessed in people with HIV-associated tuberculosis. SETTING: Open-label, three-period pharmacokinetic study in participants with HIV-1 on tenofovir-based ART (plus emtricitabine and efavirenz) in the maintenance phase of weight-based tuberc

HIV testing during systematic screening for tuberculosis among household contacts in high-tuberculosis burden settings: a systematic review and meta-analysis.

BACKGROUND: Tuberculosis household contacts are at elevated risk of HIV, and systematic screening for tuberculosis is an opportunity for people to know their status. We aimed to assess the coverage and positivity of HIV testing during household systematic screening for tuberculosis. METHODS: For this systematic review and meta-analysis (PROSPERO: CRD42024471979), we searched MEDLINE, Embase, Global Health, and Africa Wide databases from Jan 1, 2000, to June 24, 2025. The primary analysis population was household contacts of people with tuberculosis without known HIV. Studies were included if

Pharmacokinetics and tolerability of one-month daily rifapentine for latent tuberculosis treatment in haemodialysis patients.

BACKGROUND: A one-month regimen of daily isoniazid plus rifapentine (1HP) is endorsed by WHO for latent tuberculosis infection (LTBI), showing efficacy and safety in people with human immunodeficiency virus (HIV). However, the pharmacokinetics (PK) of rifapentine in end-stage renal disease (ESRD) patients on dialysis remain unknown. METHODS: We conducted a 24-hour multiple-dose PK study of daily rifapentine in 11 dialysis-dependent patients with LTBI. Participants received rifapentine 450-600 mg plus isoniazid 300 mg daily for 4 weeks. Plasma samples were collected on day 14 at eight time poi

Pharmacokinetics of Dexamethasone in Tuberculous Meningitis.

BACKGROUND: Dexamethasone is recommended as adjunctive therapy for tuberculous meningitis (TBM). Co-administration with rifampicin is expected to reduce dexamethasone exposure in TBM, an effect that may be more pronounced with the higher rifampicin doses currently being evaluated in clinical trials. METHODS: This pharmacokinetic study was nested in a randomized controlled trial comparing the safety of high-dose rifampicin (oral, 35 mg/kg; intravenous, 20 mg/kg) plus linezolid, with or without aspirin, versus standard-dose rifampicin (10 mg/kg) for adults with HIV-associated TBM. Hidden in Success: Gendered Patterns of Suboptimal Care Engagement Among Tuberculosis Patients Who "Successfully" Completed Treatment in South Africa.

BACKGROUND: Timeously completing tuberculosis (TB) treatment is a key to optimal health outcomes. Suboptimal treatment refill patterns, even among those categorized as "treatment success," may portend unfavorable outcomes. METHODS: Using patient-level medication refill data, latent-class mixed modeling was used to identify longitudinal trajectories of care engagement among TB patients achieving programmatic treatment success. Logistic regression was conducted to investigate participant-level characteristics associated with trajectory class membership. RESULTS: Among 548 participants, we iden

Pre-extensively drug-resistant tuberculosis of the spine in a previously healthy young adult: case report.

Spinal tuberculosis (TB), previously known as Pott’s disease, is the most common bone infection caused by Mycobacterium tuberculosis. The diagnosis of spinal TB is based on clinical, radiographic, microbiological, and histological findings. Its indolent nature, subacute progression, paucibacillary state, and difficulties in obtaining adequate samples often result in delayed diagnosis. The emergence of multidrug-resistant (MDR) and extensively drug-resistant (XDR) TB has posed significant challenges in the management of TB infections. Here, we present a case of pre-XDR tuberculosis of the spine

Baseline determinants of adherence for drug-sensitive TB treatment in a South African prospective cohort: a focus on HIV infection and anti-retroviral therapy, clinical care access, and TB stigma.

BACKGROUND: Inadequate adherence to tuberculosis (TB) treatment increases the risk of treatment failure and recurrence. Identifying factors contributing to poor adherence could refine targeting strategies and optimize resource distribution. We examined specific individual-level factors for TB treatment adherence among adults with drug-sensitive TB, including HIV status, antiretroviral therapy, time to access clinical care, and perceived stigma. METHODS: Data are from the “TB Mate” cluster-randomized trial, which evaluated a TB treatment adherence intervention across 18 public health clinics in

Dolutegravir concentrations in children receiving 3-month weekly isoniazid and rifapentine (3HP) for tuberculosis prevention.

In Tuberculosis Trials Consortium Study 35, which investigated the optimal dosing and safety of once-weekly isoniazid and rifapentine (3HP) in 69 children requiring tuberculosis preventive therapy, 7 children with HIV received 50 mg of once-daily dolutegravir. Plasma dolutegravir concentrations were consistent with those predicted in children on dolutegravir not receiving 3HP or potential metabolic inducers. All participants maintained viral suppression. Despite the expected induction effect of 3HP, results suggest that dolutegravir dose adjustments are not required.IMPORTANCEThis work describ

Advancements in nanotrap technology for the prevention, diagnosis and treatment of infectious diseases.

Nanotraps are particles designed to capture and concentrate target molecules and have numerous applications in infectious diseases. This review outlines how nanotrap technologies may improve the detection and treatment of bacterial and viral pathogens, including Mycobacterium tuberculosis, Borrelia burgdorferi, Yersinia pestis, HIV, SARS-CoV-2, and others. Nanotraps can enhance the sensitivity of diagnostic tools and support treatment by neutralizing bacterial toxins, capturing inflammatory mediators, and preserving viral proteins for detection. Nanotraps have also been investigated for vaccin

Cerebrospinal Fluid Hypo-inflammation Drives Mortality in HIV-Associated Tuberculous Meningitis.

BACKGROUND: Outcomes in tuberculous meningitis (TBM) are closely linked to host inflammation. Anti-inflammatory corticosteroid therapy improves survival in HIV-negative TBM, but not in people with HIV, among whom TBM is fatal in 40-50% of cases. Therefore, we investigated how mortality is associated with the local immune response in people with HIV-associated TBM. METHODS: We measured baseline concentrations of immune signaling mediators in cerebrospinal fluid (CSF) of 149 adults with HIV in Uganda, who presented with definite or probable TBM. Participants received both antimycobacterial and

Success rates of DR-TB patients and associated factors in Tanzania: a six-year cohort study.

BACKGROUND: The treatment of drug-resistant tuberculosis (DR-TB) is complex, longer, and it is associated with adverse drug reactions, contributing to higher morbidity and mortality rates. DR-TB presents a serious public health challenge, particularly in low- and middle-income countries where the burden is highest. We assessed DR-TB success rates and associated factors in Tanzania. METHODS: We collected data from DR-TB national data base and analyzed final treatment outcomes of 1,998 DR-TB patients registered for the six years (2017–2022) contributing 69% of the total DR-TB patients started on

Unmasking Mycobacterium avium: two case reports of cutaneous lesions in HIV patients after initiation of an integrase inhibitor-based regimen.

BACKGROUND: Mycobacterium avium complex (MAC) is the most common non-tuberculous mycobacteria (NTM) causing disease in immunosuppressed patients. People living with human immunodeficiency virus (PLHIV) typically present with disseminated infections characterized by non-specific constitutional symptoms such as fever, night sweats, and weight loss. Cutaneous lesions in disseminated MAC are rare and include panniculitis, granulomas, pustules, papules, nodules, abscesses and ulcerations, and ulcers. Here, we present two HIV patients who developed a disseminated MAC infection with skin lesions. CAS

Mycobacterium tuberculosis-Specific CFP-10/ESAT-6 CD4 and CD8 T-Cell Non-IFN-γ+ Responses Are Common in Young Kenyan Children Despite Low Reported Tuberculosis Exposure.

BACKGROUND: Reduced early-life interferon-γ (IFN-γ) production capacity may limit sensitivity of IFN-γ release assays to detect Mycobacterium tuberculosis (Mtb)-specific responses in young children. Measuring non-IFN-γ cytokine responses may improve detection. METHODS: Mononuclear cells isolated from peripheral blood from children exposed to HIV but uninfected and children unexposed to HIV in Western Kenya were collected at 6 to 10 weeks, and 12 and 24 months of age. Cells were incubated overnight with Mtb-specific CFP-10/ESAT-6 peptides and Staphylococcus enterotoxin B (positive control). CD
