From barriers to solutions: a qualitative study of access to HIV and TB care for forced migrants from Ukraine.

BACKGROUND: This study investigates the barriers faced by individuals who fled Ukraine following the Russian invasion in accessing HIV, tuberculosis (TB), and opioid agonist treatment (OAT). It aims to understand how forced displacement intersects with healthcare access in the context of war, drawing on the frameworks of social determinants of health and transnationalism. METHODS: The research is based on in-depth interviews conducted between 2023 and 2025 as part of an international community-based project spanning six countries. Empirical data were analyzed to identify systemic, regulatory,

Ending Tuberculosis through prevention: Zimbabwe's PEPFAR Tuberculosis preventive treatment scale-up journey (2016-2024).

BackgroundTuberculosis (TB) is the leading cause of death among people living with HIV (PLHIV). Globally in 2023, an estimated 161,000 PLHIV died of TB. TB preventive treatment (TPT) can reduce TB mortality yet scale up remains a challenge. We documented Zimbabwe's experience in scaling TPT among PLHIV.MethodologyWe analyzed routine aggregate data from the President Emergency Plan for AIDS Relief (PEPEFAR) database, Data for Accountability Transparency and Impact Monitoring (DATIM). We conducted a desk review of national guidelines, reports, training manuals, policy and strategic plans. Progra

The implementation of tuberculosis preventive therapy in HIV care clinics in Africa, Asia and Latin America: a multiregional site survey.

INTRODUCTION: Towards the 'End TB Strategy' targets, the WHO recommends the provision of tuberculosis (TB) preventive therapy (TPT) for high-risk groups including people living with HIV (PLWH). 3 years after the release of the updated 2020 WHO guidelines, we investigated the implementation of TPT services at HIV clinics in low-income and middle-income countries (LMICs), focusing on TB screening, populations eligible for TPT and available TPT regimens. METHODS: In 2023, we surveyed HIV care clinics in the International Epidemiology Databases to Evaluate AIDS consortium in Africa, the Asia-Paci

Hierarchical analysis of unsuccessful tuberculosis treatment among people living with HIV in Brazil using nationwide surveillance data.

INTRODUCTION: People with tuberculosis-HIV coinfection face multiple barriers to effective treatment, including social vulnerability, stigma and limited access to healthcare. This study examined factors associated with loss to follow-up and death among individuals with tuberculosis-HIV in Brazil. METHODS: We conducted a longitudinal study using a nationally linked database from surveillance systems. Poisson regression models with robust variance were applied to identify factors associated with unfavourable outcomes, guided by a theoretical-conceptual hierarchical framework. RESULTS: We analy

HIV impairs and exploits pulmonary Th17 and Th22 cell-mediated immune responses to Mycobacterium tuberculosis.

Tuberculosis (TB) kills an estimated 1.25 million people annually and is the leading cause of death in people with HIV (PWH) (1). The CD4+ T helper (Th) populations play significant roles in protective immunity to Mycobacterium tuberculosis (Mtb) and are essential hosts for HIV pathogenesis. Emerging evidence in blood and gastrointestinal mucosa of PWH suggests that, among Th cells, Th17 and Th22 may be preferentially depleted during HIV infection. Targeting of Th17 and Th22 cells by HIV could pose important and poorly understood risks for Mtb containment in those with co-infection. Mtb-driven

Characterizing the Dual Burden of Stigma in People With Multidrug-Resistant Tuberculosis and HIV co-infection in South Africa: A mixed-methods, Prospective Cohort Study.

BACKGROUND: Addressing stigma is a key priority of the WHO End TB Strategy. However, gaps persist in our understanding of stigma associated with multidrug/rifampin-resistant tuberculosis (MDR/RR-TB), particularly in people living with HIV. METHODS: Adults with MDR/RR-TB and HIV on antiretroviral therapy and starting bedaquiline-containing regimens in KwaZulu-Natal, South Africa, were prospectively enrolled. TB- and HIV-related stigma were evaluated using adapted questionnaires, with exploratory factor analysis (EFA) to develop stigma scales. In-depth interviews were recorded in isiZulu, trans

Implementing a Program of Comprehensive Tuberculosis Preventive Treatment: Safety, Effectiveness, and Acceptability of Moxifloxacin or Bedaquiline Use for Contacts Exposed to Drug-Resistant Strains.

BACKGROUND: Rates of drug-resistant tuberculosis (DR-TB) are increasing worldwide. Tuberculosis preventive treatment (TPT) for contacts of people with active TB is essential to halt infection progression and transmission. While newer TPT regimens for exposure to drug-susceptible and rifampin (R)-resistant strains (MDR-TB/RR-TB) are expanding, optimal treatment for contacts exposed to fluoroquinolone-resistant strains of TB (pre-XDR- or XDR-TB) remains unclear. In 2019-2020, Vladimir City, Russia, introduced moxifloxacin (Mfx)- and bedaquiline (Bdq)-based TPT regimens to prevent disease develop

Schistosoma mansoni co-infection in tuberculosis patients with or without human immunodeficiency virus and factors associated with treatment outcomes: a prospective cohort study.

BACKGROUND: Tuberculosis (TB) has resurfaced as the leading cause of death by an infectious agent after the COVID-19 pandemic. The fight against TB needs to consider factors associated with mycobacterial reactivation and treatment of TB disease. Recent findings showed that Schistosoma mansoni co-infection leads to a Th2/Th1 profile that results in an immune modulation that favors the escape of the Mycobacteria. Schistosoma mansoni may contribute to tuberculosis (TB) incidence in endemic regions. We aimed to determine the co-infection rate and treatment outcomes. METHODS: A prospective cohort s

QuantiFERON-TB gold plus in people living with HIV: independence from CD4+ T-cell counts in a low TB-burden setting.

BACKGROUND: Tuberculosis screening is recommended for people living with HIV. The QuantiFERON-TB test measures the cell-mediated response against M. Tuberculosis. The Gold-In-Tube measures the response of CD4+ T-cells, often leading to indeterminate results. The new Gold-Plus (QFT-GP) also measures CD8+ T-cells response, thus reducing uncertainties. However, studies on people living with HIV, that would benefit from a test independent from CD4+ T-cells, are scarce. OBJECTIVE: This study addresses this gap by evaluating the performance of QFT-GP specifically in a large cohort of people living

Optimized Mass Spectrometry to Uncover M. tuberculosis Biomarkers in Extracellular Vesicles From Asymptomatic Tuberculosis Patients.

BACKGROUND: Tuberculosis (TB), caused by Mycobacterium tuberculosis (Mtb), remains the leading cause of death from a single infectious agent worldwide. Exposure to Mtb results in diverse outcomes: bacterial clearance, latent infection, asymptomatic, or symptomatic TB. Current diagnostic tools cannot reliably distinguish these outcomes. Our previous studies identified Mtb proteins in extracellular vesicles (EVs) from TB patients' serum, suggesting their potential as biomarkers. Here, we aimed to discover Mtb proteins and peptides in serum EVs across TB stages, focusing on asymptomatic individua
