Beyond infection: the longitudinal impact of genitourinary tuberculosis on urologic and reproductive health.

PURPOSE: To address the limited understanding of the long-term morbidity of genitourinary tuberculosis (GUTB), by characterizing the clinical course and risk of urologic events (UE) both preceding and following GUTB diagnosis. METHODS: This retrospective age-, sex- and comorbidity-matched cohort study evaluated all patients with GUTB in Merative Marketscan (2011-2021). Patients were followed throughout enrollment to capture comorbid diseases (pulmonary tuberculosis and HIV) and urologic events (diagnoses and procedures). Events- and diagnoses-of-interest that occurred both prior and after GUT

Tuberculosis Among Artisanal Miners in the Mulongo Health Zone, Democratic Republic of Congo: Unmatched Case-Control Study.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with 10.6 million new cases and 1.3 million deaths reported in 2022. Artisanal miners in sub‑Saharan Africa are particularly vulnerable due to poverty, overcrowded living conditions, and high HIV prevalence. In the Democratic Republic of Congo (DRC), TB prevalence among miners is high, yet determinants remain insufficiently studied. OBJECTIVE: This study aimed to identify sociodemographic, behavioral, and clinical factors associated with TB among artisanal miners in the Mulongo health zone, Haut‑Lomami Province, DRC. METH

Self-reported prevalence of tuberculosis and selected associated vulnerability factors among rural populations in six districts of India: A community-based cross-sectional study.

BACKGROUND: Tuberculosis is a major public health challenge in India, contributing to one-fourth of global burden of TB disease. Comorbidities in uninfected individuals make them vulnerable to TB disease. This study estimates the self-reported prevalence of tuberculosis and selected vulnerability factors (HIV, current smoking, silicosis, occupational silica-dust exposure, diabetes mellitus and persistent cough >2 weeks) among the rural demography of six districts in India, with the aim of informing targeted active case finding under the National Tuberculosis Elimination Programme (NTEP). Comparative serum cytokine and chemokine profiles across tuberculosis, latent TB, HIV, and COVID-19 infections in Namibian adults.

BACKGROUND: The devastating pandemic of coronavirus disease 2019 (COVID-19) highlighted the importance of early detection and global collaboration in combating infectious diseases. Namibia is one of the countries that bears a high burden of tuberculosis (TB) and human immunodeficiency virus (HIV) infection. The overlap of these diseases underscores the need to better understand host immune responses, which play a critical role in disease pathogenesis and may differ across infections. This study aimed to profile selected cytokines and chemokines among Namibian individuals with TB, latent TB inf

Predictors of tuberculosis preventive therapy initiation among people living with HIV in the Volta region, Ghana.

BACKGROUND: Tuberculosis (TB) remains a leading cause of morbidity and mortality among people living with HIV (PLHIV) despite the availability of effective preventive interventions. Tuberculosis Preventive Therapy (TPT), strongly recommended by the World Health Organization, substantially reduces the risk of active TB among PLHIV, however, uptake of TPT remains suboptimal in many settings. This study, therefore, assessed the predictors of TPT initiation, beginning the course of anti-TB medications among PLHIV. METHODS: This was a health facility-based analytical cross-sectional study among 80

Neurological Manifestations in People Living with Human Immunodeficiency Virus (PLHIV) in India: A Scoping Review and the Need for a National HIV-Neurology Special Interest Group.

BACKGROUND AND OBJECTIVES: Despite a low national prevalence of 0.20%, India's absolute burden of 2.544 million people living with human immunodeficiency virus (HIV) presents clinicians with varied neurological complications. While combination antiretroviral therapy (ART) has extended survival, the neuro-HIV profile in India, characterized by the predominance of HIV-1 clade C, remains distinct from that reported in Western cohorts. This scoping review aimed to identify patterns of neurological manifestations in India across the pre-ART, ART, and "treat-all" eras and to support the establishmen

Inflammasome-associated immune dysregulation in TB-HIV coinfection.

BACKGROUND: Tuberculosis (TB) and HIV co-infection are characterized by dysregulated inflammation and increased risk of immune reconstitution inflammatory syndrome (IRIS) following antiretroviral therapy (ART) initiation. Inflammasomes are key regulators of innate immune responses, but their role in TB-HIV co-infection and IRIS remains incompletely understood. We investigated inflammasome-related gene expression and cytokine profiles in TB, HIV-1, and TB-HIV individuals, and their association with IRIS development. METHODS: Fifty-nine participants were stratified into four groups: TB-HIV (n =

Analysis of tuberculosis prevalence surveys in Africa: evidence to change the status quo in case finding.

INTRODUCTION: Lesotho has among the world's highest TB prevalences and the national program finding only 50% of incident cases. Lesotho underwent its first TB prevalence study in 2019 in reviewing this study, the authors recognized that the standard measures for screening and diagnosis TB often failed to detect the presence of culture positive pulmonary TB. This review seeks to determine if the systematic under detection of culture positive TB by standard methods is true among prevalence surveys in Africa. METHODS: We conducted a review to identify TB prevalence surveys conducted in Africa fr

Biopsychosocial determinants of tuberculosis care and treatment outcomes: a scoping review and bibliometric analysis.

BACKGROUND: Tuberculosis (TB) remains the leading infectious-disease cause of death globally, with an estimated 10.7 million incident cases and 1.23 million deaths in 2024. Its persistence is driven not by microbial factors alone, but by a complex interplay of biological, psychological, and social determinants influencing exposure, vulnerability, and access to care. Despite growing recognition of these biopsychosocial dimensions, the available evidence remains fragmented across disciplines, and no previous review has comprehensively mapped their scope while simultaneously characterizing the st

Near point-of-care nucleic acid amplification tests (NPOC-NAATs) using respiratory specimens for detection of pulmonary tuberculosis.

RATIONALE: Near point-of-care nucleic acid amplification tests (NPOC-NAATs) are a new class of diagnostics that can be used for tuberculosis diagnosis. NPOC-NAATs are suitable for use in peripheral levels of healthcare systems as they are battery-operated instrument-based tests without special infrastructure requirements and can be performed by health care workers with basic technical skills. We evaluated the accuracy of NPOC-NAATs in diagnosing pulmonary tuberculosis using respiratory samples. OBJECTIVES: To assess the diagnostic accuracy of NPOC-NAATs for detecting pulmonary tuberculosis us

Health system and household costs of preventive treatment delivery for children exposed to multidrug-resistant tuberculosis in South Africa.

BACKGROUND: Tuberculosis (TB) preventive treatment (TPT) is a core component of TB control. Levofloxacin was recently recommended by the World Health Organization as TPT in children exposed to multidrug-resistant tuberculosis (MDR-TB). However, the affordability and feasibility of TPT provision for governments, health services, individual households and global donors is uncertain. METHODS: This costing analysis used empirical data collected alongside the TB-CHAMP clinical trial which investigated levofloxacin TPT in children exposed to MDR-TB in South Africa. We modelled the societal (health

Factors perceived as relevant to starting antimicrobial treatment in patients with non-tuberculous mycobacterial lung disease in the Italian scenario: a repeated cross-sectional survey after three years of educational activities.

BACKGROUND: Non-tuberculous mycobacterial pulmonary disease (NTM-PD) is increasingly recognised, yet initiating antimycobacterial therapy remains challenging. Following a three-year period of national educational activity in Italy, we re-assessed which factors Italian clinicians perceive as relevant to the decision to start treatment. METHODS: We conducted a repeated cross-sectional web-based survey. The second wave was administered between November 2023 and February 2024 to members of the Italian SITA and IRENE networks, using the instrument administered in 2020. Participants rated the relev

Temporal proximity to counseling is associated with longitudinal bedaquiline and antiretroviral therapy adherence in multidrug-resistant tuberculosis and HIV treatment.

BACKGROUND: Medication adherence is a persistent challenge in the treatment of multidrug-resistant tuberculosis (MDR-TB) and HIV. Routine counseling notes are an underutilized source of information on patient adherence context. METHODS: We applied natural language processing (NLP) to routine counseling notes from a randomized controlled trial of MDR-TB/HIV treatment in South Africa. Topics were derived using topic modeling; notes were semantically matched to topics and clustered using K-means. We fitted linear mixed-effects (LME) regression models with participant-level random intercepts and

Clinical Characteristics, Diagnostic Performance, and Treatment Outcomes of Extrapulmonary Tuberculosis at a Tertiary Care Center in Oman: A 10-year Retrospective Cohort.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) accounts for a substantial proportion of the global tuberculosis (TB) burden and continues to pose major diagnostic and therapeutic challenges, particularly in low-incidence settings. Comprehensive descriptions of the clinical spectrum, diagnostic yield of available laboratory modalities, and treatment-related toxicity of EPTB in the Arabian Gulf are limited. We aimed to describe the clinical characteristics, diagnostic performance of microbiological and histopathological tests, and safety profile of first-line anti-TB therapy in adult patients wi
