Efficacy and tolerability of linezolid as an adjunctive treatment for nontuberculous mycobacterial infections in patients with adult-onset immunodeficiency syndrome: a prospective cohort study.

BACKGROUND: Patients with nontuberculous mycobacterial (NTM) diseases associated with anti-interferon gamma autoantibodies (anti-IFN-γ auto-Abs) often experience recurrences, despite prolonged treatment, which requires intravenous anti-mycobacterial therapy. Linezolid (LZD) shows a potential as an adjunct treatment, but there is limited data on its long-term efficacy and safety. METHODS: We conducted a prospective study from December 2017 to April 2022, to evaluate the efficacy of a nine-month oral LZD regimen in reducing NTM treatment failure, defined as relapse and the need for intravenous

Helicobacter pylori infection carrying the CagA gene may be associated with the diagnosis and disease progression of tuberculosis.

BACKGROUND AND OBJECTIVE: Helicobacter pylori (H. pylori) infection is associated with infectious diseases caused by pathogens such as Orientia tsutsugamushi (the causative agent of scrub typhus) and Brucella. However, the impact of H. pylori infection on tuberculosis (TB) remains unclear, and in particular, the clinical significance of infection with CagA-positive(CagA+) strains is worthy of exploration. METHODS: A total of 120 TB patients clinically diagnosed at the Affiliated Hospital of Youjiang Medical University for Nationalities and the Fourth People's Hospital of Nanning from June 202

Multimorbidity and the risk of post-tuberculosis lung disease: a systematic review and meta-analysis.

INTRODUCTION: Post-tuberculosis lung disease (PTLD), a chronic respiratory syndrome affecting approximately half of tuberculosis (TB) survivors, is an emerging public health threat in low- and middle-income countries (LMICs). Multimorbidity, defined as two or more coexisting chronic health conditions, is also rising in LMICs. This systematic review and meta-analysis examines associations between multimorbidity or individual comorbidities and PTLD. METHODS: Medline, EMBASE, Web of Science, Global Health and Scopus databases up to 7 March 2024 were searched and studies in LMICs assessing PTLD a

Antigen-based skin tests in community-wide active case finding for TB.

<title>BACKGROUND</title>In 2022, Mycobacterium tuberculosis antigen-based skin tests (TBSTs) were added to global recommendations for diagnosing TB infection (TBI). TBSTs have higher specificity than tuberculin skin tests (TSTs), particularly in Bacillus Calmette-Guérin (BCG)-vaccinated populations. We report results from community-wide TB active case finding (ACF) using TBSTs in remote, high-incidence communities.
<title>METHODS</title>From May to June 2024, TBSTs were implemented in community-wide ACF for TB in the Mortlock and Northwest Islands of the Federated States of Mic

NETosis associates with human TB lung tissue destruction and disease pathogenesis.

Understanding drivers of tuberculosis (TB) associated lung pathological damage is vital in identifying targets for host directed therapies (HDT). NETosis is a neutrophil specific cell death characterized by release of neutrophil extracellular traps (NETs). The role of NETosis in TB-associated lung damage and disease pathogenesis is still poorly understood. We analysed human lung TB granuloma samples using a proteomics approach, which revealed enrichment of neutrophil-associated proteins in necrotic regions of caseous and cavitary granulomas. Using immunohistochemistry (IHC), we validated the a

Triple infection with pulmonary tuberculosis, chronic hepatitis C and systemic brucellosis in an HIV/AIDS patient: a case report from northwestern China.

BACKGROUND AND OBJECTIVES: Profound immune suppression in people living with HIV/AIDS predisposes to multiple opportunistic and endemic infections. While HIV/tuberculosis (TB) and HIV/hepatitis C virus (HCV) coinfections are well recognized, simultaneous infection with Mycobacterium tuberculosis, HCV, and Brucella spp. is exceptionally rare. Such triple infection poses substantial diagnostic and therapeutic challenges, especially in endemic settings where overlapping febrile syndromes may obscure multiple concurrent etiologies. We describe a complex case of HIV/TB/HCV/brucellosis coinfection f

Tuberculosis in households with infectious cases in Kampala city: Harnessing health data science for new insights on an ancient disease with persistent, unresolved problems.

Tuberculosis (TB) is prevalent in Uganda and overlaps with a high rate of HIV/TB coinfection. While nearly all hospital-based TB cases in Kampala, the capital of Uganda, show clear TB symptoms, 30% or more of undiagnosed TB cases found through active screening are asymptomatic. Additionally, the host risk factors for TB in Kampala cannot be distinguished from environmental risk factors. These TB-specific challenges are just part of the complexity, especially in areas with high HIV/AIDS burden. Data science techniques, especially Artificial Intelligence (AI) and Machine Learning (ML) algorithms

Comparative impacts and cost-effectiveness of tuberculosis systematic screening strategies in prisons in Brazil, Colombia, and Peru: A mathematical modeling study.

BACKGROUND: Incarceration is a leading driver of tuberculosis in Latin America. Systematic screening in prisons may reduce tuberculosis burden, but optimal strategies and cost-effectiveness remain uncertain. We examined the population-wide health impacts and cost-effectiveness of systematic screening in prisons in Brazil, Colombia, and Peru, comparing different timepoints, frequencies, and screening algorithms. METHODS AND FINDINGS: Using dynamic transmission models calibrated to Brazil, Colombia, and Peru, we simulated annual or biannual (twice-yearly) prison-wide screening, alone or combine

Survival and predictors of mortality among TB-HIV co-infected children attending ART clinics in public hospitals in Addis Ababa, Ethiopia: a retrospective cohort study.

BACKGROUND: The double burden of tuberculosis (TB) and human immunodeficiency virus (HIV) remains a major global health challenge in the twenty-first century. Children (ages 0-15 years) are particularly vulnerable to TB-HIV co-infection, exhibiting higher morbidity and mortality rates than other age groups, with the burden especially pronounced in developing countries. Approximately 75% of global TB-HIV co-infections occur in Sub-Saharan Africa. This study aimed to assess the five-year survival status and identify predictors of mortality among TB-HIV co-infected children attending antiretrovir

An update on recent developments in the drug treatment of tuberculous meningitis.

INTRODUCTION: Tuberculous Meningitis (TBM) is an often lethal form of tuberculosis with damage to the brain resulting from a combination of direct effects from the causative pathogen M. tuberculosis and from the host inflammatory response. Cerebral infarction is frequent. Those who do survive often suffer severe neurological disability. The treatment regimen for TBM comprises 4 antitubercular drugs and antiinflammatory glucocorticoids. The regimen has not changed for over 40 years. AREAS COVERED: In this review we summarize recent clinical trials, supported by human pharmacokinetic studies an

QIAreach® QuantiFERON-TB diagnostic performance to detect tuberculosis infection among people living with HIV in Brazil: A prospective study.

BACKGROUND: Identifying individuals with tuberculosis infection (TBI) at increased risk of progressing to active TB remains challenging. The World Health Organization recommends both the tuberculin skin test (TST) and the interferon-gamma release assay (IGRA), including QuantiFERON-TB Gold Plus (QFT®-Plus), for the detection of TBI. QIAreach®, a novel IGRA assay designed for resource-limited settings, has shown strong concordance with QFT®-Plus. However, data on immunosuppressed individuals are limited. To address this gap, we assessed the diagnostic performance of QIAreach® compared with QFT®

Effect of prior TB preventive therapy on all-cause mortality during TB treatment among people with HIV/TB in rural eastern Uganda: an observational causal analysis.

BACKGROUND: Tuberculosis preventive therapy (TPT) is the cornerstone for preventing TB disease. However, it is uncertain whether prior TPT completion improves survival once TB disease develops in people with human immunodeficiency virus (PWH) while on anti-retroviral therapy (ART). We evaluated the effect of prior TPT completion on all-cause mortality during TB treatment among PWH who developed TB disease while on ART in rural eastern Uganda. Prior TPT completion served as a marker of sustained engagement in HIV care. METHODS: We applied inverse probability of treatment weighting using propen

Cost-Effectiveness Analysis of Xpert MTB/RIF Ultra for the Diagnosis of HIV-Associated Tuberculous Meningitis.

BACKGROUND: Human immunodeficiency virus (HIV-1)-associated tuberculous meningitis (TBM) is the most lethal form of extrapulmonary tuberculosis, with a high mortality rate in sub-Saharan Africa. Due to the limited diagnostic performance of current testing, empiric treatment is frequently required as untreated TBM is almost universally fatal. Xpert MTB/RIF Ultra (Xpert Ultra) is the World Health Organization recommended first-line diagnostic test for TBM but is costly relative to the standard of care (SOC). We performed a cost-effectiveness analysis of Xpert Ultra for HIV-associated TBM in Zamb

Youth-friendly health services for tuberculosis care: an integrative review and implementation science synthesis.

BACKGROUND: Adolescents and young adults (AYAs) face disproportionate risk of tuberculosis (TB) due to biological vulnerability, mobility, distinct risks for HIV infection and treatment failure, structural barriers, and limited access to adolescent-responsive care. Although the World Health Organization recommends youth-friendly health services (YFHS), implementation in TB care has received little attention. This review synthesizes current evidence on the application of YFHS principles across the TB care continuum to inform models responsive to the needs of AYAs. METHODS: An integrative revie
