Minimally Invasive Tissue Sampling for Postmortem Diagnosis of Tuberculous Meningitis in Zambian Adults.

BACKGROUND: The true burden of tuberculous meningitis (TBM), particularly in low and middle-income countries, is unknown due to imperfect diagnostics. Complete diagnostic autopsy (CDA) to confirm TBM in deceased meningitis patients is uncommon. Minimally invasive tissue sampling (MITS) has been shown to be a viable alternative to CDA to establish infectious etiologies of meningitis. We used MITS to determine the burden of TBM among deceased adult Zambians who presented with meningitis. METHODS: We performed a prospective cohort study of Zambian adults who presented with symptoms suggestive of

Determinants of favorable treatment outcomes in multidrug-resistant tuberculosis in Zambia: A retrospective cross sectional study, 2018-2022.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) is a persistent public health challenge in Zambia, with favorable treatment outcomes remaining below the World Health Organization target of 90%. However, limited national evidence on treatment outcomes and their determinants hinders the development of effective programmatic responses. This study aimed to estimate the proportion of MDR-TB patients achieving favorable treatment outcomes and to identify factors independently associated with these outcomes in Zambia from 2018 to 2022. METHODS: A retrospective cross-sectional study was conduct

Artificial Intelligence and Machine Learning-based prediction of tuberculosis treatment failure: A systematic review and meta-analysis.

BACKGROUND: Tuberculosis (TB) remains a leading cause of infectious disease mortality worldwide, and treatment failure contributes to ongoing transmission, drug resistance, and poor clinical outcomes. Artificial intelligence (AI) and machine learning (ML) approaches have attracted growing interest in predicting TB treatment outcomes, but the literature is heterogeneous and lacks a comprehensive synthesis. METHODS: We systematically searched PubMed/MEDLINE and Embase (January 2000-October 2025) for studies developing or validating AI/ML models to predict TB treatment failure. Two reviewers ind

Mixed-strain infection and heteroresistance in drug-resistant tuberculosis: insights from a whole-genome sequencing pilot.

BACKGROUND: Mixed-strain Mycobacterium tuberculosis (M. tuberculosis; Mtb) infections and heteroresistance are increasingly recognized as under-detected contributors to drug resistance dynamics. In high-burden rural settings such as the Eastern Cape, these genomic features may reflect both within-host diversity and ongoing transmission. However, their epidemiological and clinical implications remain incompletely understood, particularly in contexts with high HIV co-infection. METHODS: We conducted an exploratory whole-genome sequencing (WGS) analysis of 28 drug-resistant M. tuberculosis isola

Daily HIV pre-exposure prophylaxis enhances monocyte activation and reprogrammes immune cell metabolism.

INTRODUCTION: Pre-exposure prophylaxis (PrEP) with tenofovir/emtricitabine (TDF/FTC) is highly effective for HIV prevention. While antiretroviral therapy (ART) is linked to chronic inflammation in people living with HIV, its direct effects on immune phenotype, function, and metabolism in HIV-negative individuals remain unclear. This study aimed to investigate how daily TDF/FTC pre-exposure prophylaxis modulates immune activation, functional responses, and metabolic programming in innate and adaptive immune cells in HIV-negative individuals. METHODS: Gay, bisexual, and other men who have sex w
