Associated characteristics of multidrug-resistant tuberculosis (MDR-TB) cases in Sub-Saharan Africa: a systematic review and meta-analysis.

BACKGROUND: The persistent spread of tuberculosis (TB), driven by multidrug-resistant TB (MDR-TB), poses a major public health threat in Sub-Saharan Africa (SSA), worsening the region's already high burden of infectious disease mortality. Despite fragmented evidence, a comprehensive quantitative assessment of MDR-TB patient characteristics that fuel resistance across the continent has been limited. This study aimed to synthesize and quantify the demographic, clinical, socio-economic, and behavioral patient profiles associated with MDR-TB in SSA to inform targeted control strategies. METHODS:

Performance of a next-generation rapid lipoarabinomannan urine assay for tuberculosis in outpatients with and without HIV in South Africa and Uganda: a two-centre diagnostic accuracy study.

BACKGROUND: Non-sputum-based tuberculosis tests are a global priority because sputum-dependent diagnostics systematically miss populations at highest risk of disease, including people living with HIV. We evaluated the diagnostic accuracy of a next-generation urine lipoarabinomannan assay. METHODS: In this two-centre diagnostic accuracy study, we enrolled adults aged 18 years or older, regardless of their HIV status, with presumptive pulmonary tuberculosis, attending outpatient clinics in South Africa and Uganda. Participants who had received tuberculosis treatment within the previous 2 months

Disease burden and attributable risk factors of Tuberculosis among adults aged 60 years and older in China from 1990 to 2021 and its prediction to 2041.

BACKGROUND: TB continues to pose considerable threats to elderly populations worldwide. Drawing on the Global Burden of Disease 2021 dataset spanning 3 decades, this investigation explores how TB burden has shifted among Chinese seniors aged 60 and above. METHODS: Standardized epidemiological metrics for incidence, prevalence, mortality, disability-adjusted life years, age-standardized rate and average annual percent change were calculated. Joinpoint regression analysis was applied to identify periods manifesting notable changes and an age-period-cohort framework was employed to estimate the

An Fc receptor and IgA functional signature identifies TB disease in children living with HIV.

BACKGROUND: Tuberculosis (TB) is a leading cause of morbidity and mortality among children living with HIV (CLHIV). TB disease is difficult to detect with current clinical microbiological, immunological, and radiographic tools. Serological assays that determine levels of Mycobacterium tuberculosis reactive antibodies inconsistently identify TB but antibody Fc receptor engagement and effector functions are promising biomarkers of TB disease. The objectives of this study are to identify antigen specific antibody signatures in CLHIV with TB disease that both 1) respond to treatment and 2) disting

Targeted tuberculosis vaccination strategies in the United States: A modeling study.

BACKGROUND: Tuberculosis (TB) incidence in the United States has remained elevated above pre-pandemic levels since 2021, with over 85% of cases resulting from reactivation of Mycobacterium tuberculosis (Mtb) infection. New vaccines that would prevent TB in adults are under development, but the potential health impact of a program prioritizing non-U.S.-born persons and persons with medical comorbidities, including persons living with HIV (PLWH), has not been evaluated. METHODS: We developed a deterministic compartmental transmission model that simulates Mtb infection, transmission, and progres

The pleural tuberculosis-associated microenvironment promotes HIV-1 persistence by impairing CD8+ T cell-mediated viral control.

Mycobacteriumtuberculosis (Mtb), the causative agent of tuberculosis (TB), is the most common coinfection in people living with HIV-1 (PLWH). This coinfection is associated with accelerated HIV-1 disease progression and reduced survival. However, the immunological and virological mechanisms driving this progression are not completely understood. To address this knowledge gap, using pleural effusion samples from PLWH and TB, we investigated how the HIV-1 genetic landscape and the anti-HIV-1 immune response are impacted by a TB-associated microenvironment. Our results revealed an enrichment of g

Beyond the lungs: A case series of extrapulmonary tuberculosis.

BACKGROUND: Globally, Tuberculosis (TB) is the leading infectious cause of death with more than 10 million cases and 1 million deaths from the disease annually. TB can present as a primary extrapulmonary infection without pulmonary symptoms or infection often mimicking other pathologies, thereby making its clinical manifestations uniquely difficult to diagnose in the Emergency Department. OBJECTIVE: We present five patients with rare cases of TB in an effort to underscore the importance of the EM physician keeping extrapulmonary infection in their differential. DISCUSSION: Extrapulmonary TB

Evaluation of five fixation methods for mandibular sagittal split osteotomy in a significant advancement with counterclockwise rotation: a finite element study.

BACKGROUND: Diabetes is one of the non-communicable diseases (NCDs) which is rising significantly across sub-Saharan African (SSA) countries and posing a threat to the social, economic, and cultural fabric of the SSA population. The inclusion of NCDs into the post-2015 development agenda along with the global monitoring framework provides an opportunity to monitor progress of development programmes in developing countries. This paper examines challenges associated with dealing with diabetes within the development agenda in SSA and explores some policy options. DESIGN: This conceptual review d

Gut microbiota dysbiosis and the gut-lung axis: links to asthma and its common comorbidities.

INTRODUCTION: Almost from the beginning of the HIV epidemic in 1981, an association with tuberculosis (TB) was recognized. This association between HIV and TB co-infection has been particularly evident amongst prisoners. However, despite this, few studies of TB in prisons have stratified results by HIV status. Given the high prevalence of HIV-positive persons and TB-infected persons in prisons and the documented risk of TB in those infected with HIV, it is of interest to determine how co-infection varies amongst prison populations worldwide. For this reason we have undertaken a systematic revi

Gastroduodenal Kaposi Sarcoma Without Cutaneous Lesions in an Immunosuppressed Patient.

Kaposi sarcoma (KS) is an angioproliferative neoplasm that develops as a result of infection with human herpesvirus 8 (HHV-8). The disease most commonly appears as painless, nonpruritic, violaceous papules, plaques, or nodules on the skin, but it can also affect lymph nodes, mucosal surfaces, and internal organs. A 53-year-old man from Guinea-Bissau with membranous nephropathy who was being treated with prednisolone and cyclophosphamide presented with abdominal pain, early satiety, anorexia, and fever. His symptoms emerged while he was taking antibiotics for a recent respiratory tract infectio

Unexplained Giant Esophageal Ulceration in a Pregnant Woman With Severe Immunosuppression.

Esophageal ulceration is a well-recognized cause of morbidity in patients with advanced human immunodeficiency virus (HIV) infection, most commonly from opportunistic infections such as cytomegalovirus, herpes simplex virus, or Candida species. Idiopathic esophageal ulceration (IEU) is a diagnosis of exclusion associated with profound immunosuppression, typically with CD4 counts <50 cells/mm³. Its occurrence during pregnancy, particularly in congenital HIV infection, is exceedingly rare and presents unique diagnostic and therapeutic challenges. We report a 32-year-old pregnant woman at 21 week

Mycobacterium avium complex (MAC) elimination slopes from sputum are potential treatment response biomarkers in patients with HIV and pulmonary MAC.

BACKGROUND: Lack of quantitative and reproducible treatment response biomarkers is slowing the development of better therapy regimens for pulmonary Mycobacterium avium complex (P-MAC) disease. We hypothesised that steeper bacterial elimination rate slopes (BERSs) would be associated with faster mycobacterial cure and more favourable outcomes. METHODS: We retrospectively compared serial sputum culture (SSC) trajectories and outcomes of 29 P-MAC patients, of whom 26/29 (90%) had concurrent HIV infection, treated with guideline-based therapy (GBT) versus non-GBT regimens at a tertiary hospital i

Renal Trajectories Across Histopathological Phenotypes in Treated HIV: A Multicenter Biopsy-Based Observational Study.

BACKGROUND: Despite widespread antiretroviral therapy (ART), kidney disease in people living with HIV remains clinically and biologically heterogeneous. Persistent immune imbalance-reflected by CD4/CD8 inversion-often coexists with virologic suppression, yet its relevance within biopsy-defined renal phenotypes is incompletely understood. We aimed to characterize clinicopathological phenotypes and longitudinal renal trajectories in treated people living with HIV using the KDIGO histopathological framework. Effects of the COVID-19 Pandemic on Tuberculosis Outcomes in the United States: A Bayesian Analysis.

BACKGROUND: Tuberculosis (TB) notifications and deaths in the United States fluctuated substantially during the coronavirus disease 2019 (COVID-19) pandemic. We analyzed multiple data sources to understand the factors contributing to these changes and estimated future TB trends. METHODS: We identified 4 mechanisms potentially contributing to observed TB trends during 2020-2023: immigration, respiratory contact rates, rates of accurate diagnosis and treatment initiation, and mortality rates for persons experiencing TB disease. We employed a Bayesian approach to synthesize evidence on how these
