Post-TB care in the UK: a national survey of existing practice.

BACKGROUND: Tuberculosis (TB) survivors experience high mortality and long-term morbidity, contributing substantially to the global TB burden. In the UK, where TB incidence is rising, the scale of post-TB health needs is unknown and current guidelines do not recommend follow-up. We conducted the first nationwide survey of UK TB services to assess approaches to post-TB care. METHODS: We conducted a digital survey between February and May 2025 across National Health Service TB services in all four nations, targeting specialist clinicians. The questionnaire captured data on types of post-TB morb

Efficacy and Safety of Glutathione Supplementation in Patients with HIV Infection and HIV-Tuberculosis Co-Infection.

Glutathione (GSH), the most abundant intracellular non-protein thiol, is a central regulator F redox homeostasis, immune function, and mitochondrial integrity. In human immunodeficiency virus (HIV) infection, persistent oxidative stress and impaired precursor availability result in sustained glutathione deficiency, contributing to immune dysfunction, inflammation, and disease progression despite effective antiretroviral therapy. This redox imbalance is further exacerbated in HIV-tuberculosis co-infection, where compounded inflammatory and metabolic stress increases susceptibility to opportunis

Declining prevalence of AIDS-defining illness among newly diagnosed people living with HIV in northern Thailand: a 10-year review.

BACKGROUND: HIV infection and AIDS remain serious global health problems. In Thailand, previous studies have revealed that nearly half of newly diagnosed people living with HIV (PLWH) presented with AIDS-defining illnesses (ADIs). However, data regarding the burden of ADIs among PLWH in northern Thailand have been limited. METHODS: This retrospective cohort study analyzed 777 newly diagnosed PLWH between January 1, 2012, and December 31, 2022 at a tertiary care hospital in northern Thailand. RESULTS: The study found that 29.0% of patients presented with ADIs at diagnosis, although this preva

The impact of cytomegalovirus coinfection on tuberculosis in the mouse model.

BACKGROUND: Tuberculosis (TB) remains a major global health challenge, with 10.6 million new cases and 1.3 million deaths annually. HIV coinfection is a well-known risk factor in high-burden regions, but emerging evidence suggests that other viral infections, including cytomegalovirus (CMV), may also impact TB pathogenesis. CMV is a ubiquitous herpesvirus capable of lifelong persistence, and its potential role in modulating TB progression remains unclear. METHODS: In this study, we established a murine coinfection model in C57BL/6 mice to investigate the effects of latent murine CMV (MCMV) on

Mapping facility-based tuberculosis preventive treatment provision in Ethiopia: a geospatial analysis.

BACKGROUND: Tuberculosis (TB) remains a major public health challenge in Ethiopia, despite being a preventable disease. TB preventive treatment (TPT) is a critical intervention to prevent the progression from latent TB infection to active disease, particularly among household contacts of TB patients and people living with HIV due to weakened immunity. However, the initiation and completion rates of TPT at subnational and local levels have not been thoroughly investigated. This study aims to map facility-based TPT initiation and completion rates among household contacts of TB across Ethiopia. M

Component Clinical and Cost Outcomes Complementing Value of Information: Case Study of a Tuberculosis Diagnostics Clinical Trial.

PURPOSE: Conventional value-of-information (VOI) analysis bases decisions on a composite cost-effectiveness measure that aggregates clinical and cost outcomes. We sought to highlight potentially informative tradeoffs by estimating and reporting the "disaggregated" components, allowing stakeholders-such as clinicians, patients, trial funders, policymakers, and trial networks-to identify what drives the value of additional research. METHODS: We used a microsimulation to project the value of a hypothetical trial comparing mortality associated with different tuberculosis screening strategies amon

Towards tuberculosis elimination in people with HIV on antiretroviral therapy: evidence from a two-decade nationwide cohort in Spain.

BACKGROUND: Tuberculosis (TB) incidence among people with HIV (PWH) has fallen in many high-income settings, yet residual risk persists, and tools to identify those still vulnerable are limited. This study aimed to evaluate TB incidence trends, identify risk factors, and develop a predictive score in PWH in Spain. METHODS: We conducted a retrospective nationwide cohort study using the Spanish AIDS Research Network (CoRIS). Adults initiating antiretroviral therapy (ART) between 2004 and 2023 were followed from ART start until incident TB, death, or censoring. Incidence rates were expressed per

The role of environmental contamination in the dynamics of HIV-TB co-infection with control strategies: Caputo fractional-order approach.

This study investigates the effectiveness of integrated environmental and medical interventions in controlling HIV-TB co-infection. A Caputo fractional-order modeling framework is developed to capture memory and nonlocal effects inherent in chronic infections, while explicitly incorporating environmental reservoirs of Mycobacterium tuberculosis as an additional transmission pathway. The mathematical well-posedness of the model is established through existence, uniqueness, positivity, boundedness, and Ulam-Hyers stability analysis. Numerical solutions are obtained using an Adams-Bashforth-Moult

Diagnostic Yield of Tongue Swab- Compared to Sputum-Based Molecular Testing for Tuberculosis in Four High-Burden Countries.

BACKGROUND: Tongue swabs are a promising alternative specimen for tuberculosis (TB) diagnosis. Although test specificity exceeds 98%, sensitivity is lower than sputum-based molecular testing. We investigated whether the use of tongue swabs could increase sample availability, resulting in similar diagnostic yield. METHODS: In this cross-sectional study (July 2024-January 2025), we screened consecutive people with presumptive TB at health centers in the Philippines, Vietnam, Uganda, and Zambia. Participants were asked to provide tongue swabs and referred for routine sputum collection. Tongue sw

Differentiated HIV Service Delivery vs Conventional Care: Tuberculosis Preventive Therapy Outcomes for People Living With HIV in Sub-Saharan Africa.

INTRODUCTION: Differentiated service delivery (DSD) models, which reduce provider visits and offer varied ART delivery methods for people with HIV, are expanding across sub-Saharan Africa. However, their impact on services beyond ART, including the uptake and completion of tuberculosis preventive therapy (TPT), remains unclear. METHODS: Using the RE-AIM framework, we analyzed data from Opt4TPT, a longitudinal cohort study examining TPT delivery in South Africa and Zimbabwe. We constructed multivariate logistic regression models to evaluate the association of receiving ART from a DSD model wit

Direct detection of unamplified DNA from Mycobacterium tuberculosis using modified gold nanoparticles.

INTRODUCTION: Tuberculosis (TB), caused by Mycobacterium tuberculosis (M. tuberculosis, MTB), remains a significant global health threat, especially in resource-limited settings. The absence of rapid, affordable, and clinically useful diagnostic methods poses a significant obstacle to global TB elimination efforts. Standard diagnostic methods are either time-consuming or dependent on sophisticated laboratory infrastructure. MATERIALS AND METHODS: This study developed and optimized a novel gold nanoparticle (AuNP)-based colorimetric biosensor for the direct, amplification-free detection of M.

Assessment of drug therapy problems and associated factors among PLWHA and tuberculosis admitted to two referral hospitals in Lusaka, Zambia.

BACKGROUND: Managing the simultaneous treatment of HIV/AIDS and tuberculosis (TB) increases the risk of adverse reactions, drug interactions, and challenges to treatment adherence. To address these complexities, a multidisciplinary approach is crucial for preventing, identifying, and resolving drug therapy problems (DTPs) in these patients. Therefore, this study aimed to assess the magnitude and associated factors of DTPs in hospitalized people with tuberculosis and those living with HIV/AIDs at two referral hospitals. METHODS: We conducted a prospective observational study between January 202
