Synovial chondromatosis of the temporomandibular joint.

Synovial chondromatosis is a benign, chronic, progressive synovial metaplasia that will not resolve spontaneously. Although it is non-neoplastic, the condition can easily be mistaken histologically as malignant. One Chinese male patient with synovial chondromatosis of the temporomandibular joint is described. Awareness of this rare lesion is important to prevent overdiagnosis and radical treatment. Treatment consists of synovectomy and removal of loose bodies within the joint space.

Treatment of primary Sjögren's syndrome with hydroxychloroquine.

Sjögren's syndrome is an autoimmune disease characterized by lymphocytic infiltration of the salivary/lacrimal glands, autoantibody production, and polyclonal hyperglobulinemia. In view of the efficacy and relative safety of hydroxychloroquine in other autoimmune disorders, the potential benefit of hydroxychloroquine (200 mg per day for 12 months) in 10 patients with Sjögren's syndrome was evaluated. Changes in levels of total immunoglobulin, antibody against Sjögren's syndrome-associated antigen B, rheumatoid factor, and in vitro production of immunoglobulin in the serum were evaluated. For c

Monocytoid B-cell lymphoma. Clinicopathologic study of 21 cases of a unique type of low-grade lymphoma.

The morphologic and immunologic features of three cases of an unusual and distinct B-cell lymphoma were recently described and termed monocytoid B-cell lymphoma (MBCL) because of the striking resemblance of the neoplastic cells to reactive monocytoid B-lymphocytes. The morphologic spectrum and the clinical behavior of MBCL were investigated in a series of 21 patients. This study indicates that patients with MBCL usually present with lymphadenopathy and Stage I or II disease. MBCL also occurs at extranodal sites including the salivary gland. Because four of the patients with MBCL had Sjögren's

Respiratory manifestations of connective tissue disease.

Pleuropulmonary disease is a common feature of the following connective tissue diseases: systemic lupus erythematosis (SLE), rheumatoid arthritis (RA), progressive systemic sclerosis (PSS), mixed connective tissue disease (MCTD), polymyositis/dermatomyositis (PM/DM), and Sjogren's syndrome (SS). Features common to most of these disorders include pleurisy with effusion and interstitial lung disease. Pleural effusions caused by SLE and RA have certain characteristics on pleural fluid analysis that aid in diagnosis, but infection and other causes of effusion must be excluded. Interstitial lung in

Viruses and salivary gland disease: are there associations?

Viruses can cause sialadenitis and may be associated with other diseases of salivary glands, particularly immunologically mediated and neoplastic lesions. The evidence that such an association with Sjögren's syndrome is causal is reviewed here and shown to be fairly tenuous at present.

Computed tomography of the sternum.

The author presents his experience with computed tomography (CT) in the diagnosis of sternal anomalies. The normal anatomy of the sternum and the peristernal soft tissues are described. The embryology, pertinent to the understanding of congenital sternal anomalies, is reviewed. The technique used to image the sternum is briefly explained. Variants of the sternum, traumatic changes, iatrogenic alterations, inflammation, and neoplastic lesions are illustrated. It is concluded that CT offers definite advantages in the evaluation of presumed sternal pathology and should be the examination of choic

Septic arthritis of the temporo-mandibular joint--a problematic diagnosis.

A rare case of chronic septic arthritis of the temporo-mandibular joint is described, which presented as a pre-auricular swelling several weeks after a blow to the chin. Although the history indicated an inflammatory process precipitated by trauma, the clinical findings suggested a neoplastic aetiology. The treatment and problems in making an early diagnosis in this condition are discussed.

Long term mortality after a single treatment course with X-rays in patients treated for ankylosing spondylitis.

Mortality up to 1 January 1983 has been studied in 14,106 patients with ankylosing spondylitis given a single course of X-ray treatment during 1935-54. For neoplasms other than leukaemia or colon cancer, mortality was 28% greater than that of members of the general population of England and Wales, and this increase is likely to have been a direct consequence of the treatment. The proportional increase reached a maximum of 71% between 10.0 and 12.4 years after irradiation and then declined. There was only a 7% increase in mortality from these tumours more than 25.0 years after irradiation and o

Osteochondrosis of the temporomandibular joint presenting as an apparent parotid mass.

A mass in the preauricular area usually indicates the presence of a neoplastic or inflammatory process within the parotid gland. Osteochondrosis is an unusual disease process affecting large joints, particularly the knee. Rarely, the temporomandibular joint (TMJ) may be affected. The disease process involves the synovial lining of the TMJ and is commonly ascribed to a benign neoplastic process or metaplasia, although trauma and inflammation have also been implicated. This process can result in single or multiple loose cartilage bodies (joint mice) within the joint cavity. Symptoms are usually

Radioimmunoassay of tumor necrosis factor in serum.

We present a double-antibody radioimmunoassay for determination of the concentration of tumor necrosis factor (TNF) in serum. TNF in serum competes with a fixed amount of 125I-labeled TNF for the binding sites of specific rabbit antibodies. The bound TNF is precipitated with Sepharose-bound anti-rabbit IgG, then centrifuged, and the radioactivity of the pellets is counted. The detection limit of the assay is 7 ng/L (B0-3 SD). Bound radioactivity in the range of 10% to 90% of the B0 counts corresponds to TNF concentrations of 26 to 10,000 ng/L. Of 40 sera from healthy subjects, 21 (53%) contain

[Magnetic resonance imaging of the knee. Preliminary study of non-neoplastic osteoarticular pathology].

Magnetic Resonance Imaging (MRI) has been applied to musculoskeletal pathoanatomy and has proved to be useful in the detection and characterization of knee pathology. Thirty-two acutely injured knees and 8 normal knees were examined. The images were obtained in the Diagnostic Centre RMRC of Naples on a 0.5 T superconductive magnet system, using a surface coil and a spin-echo pulse sequence (SE 600/28 ms). The examined limb was immobilized and bent at 8-10 degrees, extrarotated for the examination of the anterior cruciate ligament (ACL) only. Images were obtained on a 256 x 256 matrix and had a

[Evaluation of hyperuricemia caused by fructose in a status of altered uric acid metabolism].

It's reported that oral administration of fructose in normal and gouty patients causes a rise in serum uric acid (see "References": Stirpe et al.). The changes in serum uric acid after administration of fructose to gouty patients and to children of gouty patients were significantly different from the changes observed in normal subjects, both for the extent and for the duration of hyperuricaemia. In this study the authors verify if the fructose-induced hyperuricaemia can help to identify gouty patients from people with different metabolic alteration of serum uric acid levels (i.e. chronic leuka

Computed tomography of the spine: an update and review.

Computed tomography (CT) has proven to be an important diagnostic imaging technique for the evaluation of a variety of spinal disorders. The method is useful for identifying and characterizing acute fractures in traumatized patients and can accurately quantify spinal canal encroachment. Intervertebral disc disease and facet joint osteoarthritis are readily demonstrated by CT in both the cervical and lumbar regions. The technique can reliably distinguish between infection and neoplastic disease in the spine and is helpful for the guidance of percutaneous biopsy. A variety of congenital and deve

Systemic lupus erythematosus sera antilymphocyte reactivity: detection of antibodies to Tac-antigen positive T cell lines.

Sera obtained from patients with systemic lupus erythematosus (SLE) were tested for their reactivity to cell lines derived from cutaneous T-cell lymphoma (CTCL), or adult T cell leukaemia (ATL), and with other cell lines, by indirect immunofluorescence method. Approximately one half of SLE sera reacted with the surface antigens of HUT-102 cells, a cell line from CTCL, which constitutively expresses Tac antigen. The titre tended to be higher in the active than in the inactive stage. These positive sera also reacted with other neoplastic or normal T cell lines having Tac antigen. SLE sera reacti

Expression of a cross-reactive idiotype on rheumatoid factor in patients with Sjogren's syndrome.

Primary Sjogren's syndrome (SS) is a systemic autoimmune disorder characterized by lymphocytic infiltration of salivary and lacrimal glands. These patients have evidence of marked B cell hyperactivity, including the production of autoantibodies such as rheumatoid factor (RF) and an increased frequency of non-Hodgkin's lymphoma. We now demonstrate that RF from 12/15 SS patients contains a cross-reactive idiotype (CRI) on their kappa light chain defined by a monoclonal antibody (MoAb 17-109) and immunoblotting. This CRI was associated with immunoglobulin (Ig) A-RF, and to a lesser extent with Ig

Differential diagnosis of neoplastic and rheumatic diseases in children.

The early phase of the disease in 30 children referred to this Clinic with the diagnosis of rheumatic disease and whose condition was ultimately diagnosed as malignancy was analysed. A correct diagnosis was established after a period of 1-22 months. thus acute leukemia, non-Hodgkin's lymphoma, Hodgkin's disease and solid tumours were diagnosed in 13, 7, 3, 7 consecutive patients. It was demonstrated that in an early stage of neoplastic diseases, signs may occur which are typical also of rheumatic diseases. Fever, arthralgia or arthritis, hepatosplenomegaly, lymphadenopathy and signs of cardiac

[Chemotaxis and collagen].

The authors evaluate the chemotactic properties of collagen and collagen derived fragments, according to literature datas. In experimental conditions, collagen can be chemotactic for human blood monocytes, fibroblasts and various malignant cells but not polymorphonuclear. The meaning of these facts and their possible role in the genesis of osteo-articular diseases are also discussed.

[Neoplasias of the scapula--rare causes of chronic shoulder-arm syndrome].

The diseases most frequently resulting in a chronic shoulder-hand syndrome are definitely of a post-traumatic nature and are later - after a varying period - often combined with degenerative changes. The tendency to injury is enhanced by the particularly great mobility of the shoulder joint. Inflammatory changes - e.g. of bacterial, rheumatic origin - are much rarer. The authors present two patients with rare neoplastic lesions in the region of the shoulder-blade and show how the disease was identified via various differential diagnostic methods.
