The Impact of NEAT1 on Inflammatory Responses and Processes in Arthropathic Diseases: Latest Findings and Perspectives.

Arthropathies are joint-related disorders with diverse etiologies but standard clinical and radiographic features. There is growing evidence that long non-coding RNAs (lncRNAs) play a crucial role in regulating the development and progression of certain diseases. Nuclear-enriched abundant transcript 1 (NEAT1) is one of these lncRNAs. It is common and helps control transcription and epigenetics, especially during inflammation. This narrative review presents current data on NEAT1's role in inflammation and its contribution to the development of arthropathies, including psoriasis, rheumatoid arth

Persons with high Hemophilia Joint Health Scores have reduced functional range of motion during simulated sports activities.

BACKGROUND: Joint bleeds for persons with hemophilia are characteristic and can lead to hemophilic arthropathy, primarily in the ankles, knees, and elbows. The Hemophilia Joint Health Score (HJHS) is a validated physical exam measure of hemophilic arthropathy but does not take into account functional range of motion (ROM). OBJECTIVES: To examine the association between HJHS severity and functional ROM during simulated sports activities in persons with hemophilia compared with healthy controls. METHODS: Using motion capture analysis, functional ROM at the ankle, knee, and hip was measured in

Hemophilia's Overlooked Female Face.

Hemophilia is no longer regarded solely as a male disorder; female carriers are increasingly attracting the attention of the scientific community due to the common, albeit overlooked, bleeding tendency associated with their condition. Moreover, women can, in rare cases, experience disease severity comparable to that of hemophilic males. The overall bleeding phenotype in females is complicated by gynecological and obstetric bleedings, which do not always correlate directly to their factor levels. As a patient population, they remain significantly underdiagnosed and inadequately treated due to a

Enhanced Intracellular Stability and Translation Efficiency of mRNA Drugs by a 2-arm mRNA Platform.

The poly(A) tail deadenylation and transient protein expression of messenger RNA (mRNA) extremely hinder its therapeutic potential in genetic diseases, from which it follows that improving RNA stability and translation efficiency has emerged as a critical priority. In this study, we construct a 2-arm mRNA via a streamlined modular assembly approach, characterized by a unique topology formed through the dimerization of two mRNA 3' poly(A) tails. This distinctive architecture exhibits the capacity for efficiently recruiting poly(A)-binding proteins (PABPs) to activate the eIF3-eIF4F complex and

FVIIIa Mimetics: New Approaches and Next-Generation Initiatives.

Emicizumab has revolutionized hemophilia A care, yet limitations regarding the "ceiling" of hemostatic efficacy (equivalent to mild hemophilia) and global access persist. This review critically examines two distinct paradigms shaping the future of care: Innovation and Access. Regarding innovation, we synthesize the latest clinical data on next-generation FVIIIa mimetics. Mim8 (denecimig) demonstrated superiority over prior prophylaxis in the FRONTIER 2 phase 3 study, offering flexible weekly to monthly dosing options. Effect of Electrical Stimulation of the Quadriceps Muscles in the Management of Haemophilic Knee Arthropathy.

Background Hemarthrosis is the most common disabling manifestation of haemophilia, and recurrent knee bleeding leads to haemophilic arthropathy with pain, muscle wasting, and functional limitation. Evidence on the added benefit of electrical stimulation (ES) in conjunction with therapeutic exercise for haemophilic knee arthropathy is limited, particularly in low-resource settings. So, this study was planned to evaluate the effect of quadriceps ES as an adjunct to isometric quadriceps exercise (IQE) on pain, muscle strength, muscle bulk, bleeding events, and functional status in patients with h

Physical activity patterns in people with haemophilia with and without arthropathy: a cross-sectional study using wearable sensors.

BACKGROUND: In haemophilia, recurrent joint bleeding often results in joint destruction, which in turn leads to reduced physical activity (PA) and decreased quality of life (QOL). While maintaining high PA levels is important in the care of people with haemophilia (PwH), it remains unclear whether PwH without joint disease can maintain adequate activity levels. AIM: This descriptive cross-sectional study sought to compare moderate-to-vigorous physical activity (MVPA) levels among 19 PwH with haemophilic arthropathy (HA), 12 PwH without HA, and 15 non-PwH. METHODS: A total of 46 males wore a

Biomarkers as Diagnostic and Prognostic Tools for Hemophilic Arthropathy.

Repeated joint bleeds cause hemophilic arthropathy, which can develop despite regular prophylaxis with clotting factors. Therefore, recognizing ongoing or subclinical bleeds is an important element of assessing the therapeutic needs of patients with hemophilia. Joint bleeds lead to the remodeling of the synovia and subsequent osteochondral damage if bleeding persists or recurs. These processes can be demonstrated by biomarkers in the synovial fluid, blood, or urine, in addition to conventional imaging. Biomarkers that are upregulated in patients with hemophilia in the context of joint bleeds a

EVALUATION OF THE RADIOISOTOPIC SYNOVECTOMY PROTOCOL IN PATIENTS WITH HEMOPHILIA.

INTRODUCTION: Hemophilia is an inherited hemorrhagic disorder in which the most common musculoskeletal manifestation is intra-articular hemorrhage. Successive hemarthroses result in chronic synovitis, joint surface destruction, and chronic hemophilic arthropathy. When conservative treatment cannot halt an already established synovitis, removing the synovium may interrupt the process of joint destruction, and radioisotopic synovectomy (RS) becomes one of the treatments of choice. OBJECTIVE: To evaluate the safety and effectiveness of the RS protocol in reducing the incidence of hemarthrosis in

Delayed diagnosis of hemophilia A presenting as chronic joint swelling: a case report.

BACKGROUND: Chronic joint swelling in children usually requires differential diagnosis for juvenile idiopathic arthritis (JIA). Hemophilic arthropathy, despite its rarity, may be an important differential entity, and its identification is challenging in the absence of significant bleeding history. CASE PRESENTATION: A 2-year-old boy admitted to our hospital with a two-month history of recurrent left knee swelling. The patient was initially diagnosed with JIA based on the MRI findings of synovitis and joint effusion. No personal or family history of bleeding disorders was noted. The vital diag

rAAV8 encapsidated HMR-001/z enables high efficiency hepatic transduction and restores hemostasis in hemophilic mice.

Hemophilia A (HA), an X-linked bleeding disorder caused by factor VIII (FVIII) deficiency, is primarily managed with exogenous therapeutic agents; however, this treatment approach remains burdensome and fails to provide durable hemostatic control. Adeno-associated viral (AAV) vectors enabling endogenous FVIII expression have emerged as promising alternatives to address these limitations, but existing vectors show limited transduction efficiency and declining activity over time. Here, we report the development and preclinical evaluation of two bioengineered AAV8 vectors, HMR-001 and its codon-o

When and How to Start Prophylaxis in Children with Hemophilia.

BACKGROUND: Hemophilia A and B are X-linked inherited bleeding disorders resulting from factor VIII or IX deficiency and are associated with substantial morbidity beginning in early childhood. Recurrent musculoskeletal bleeding can lead to hemophilic arthropathy and long-term disability. Intracranial hemorrhage, especially in early infancy, remains a life-threatening complication with high risks of mortality and neurological sequelae. As treatment decisions in early life decisively influence lifetime outcomes, the optimal timing and mode of prophylaxis are central aspects of pediatric hemophil

Bleeding Frequency and Its Association With Joint Health and Functional Independence in Hemophilia: A Cross-Sectional Study Using the Hemophilia Joint Health Score (HJHS) and Functional Independence Score in Hemophilia (FISH).

Background and aim Hemophilia is a genetic bleeding disorder defined by a deficiency of coagulation factor VIII or IX, which results in recurrent bleeding episodes that frequently involve the joints. Repeated hemarthrosis results in progressive joint damage and functional impairment. The Hemophilia Joint Health Score (HJHS) and Functional Independence Score in Hemophilia (FISH) are standardized tools used to assess joint status and functional ability in patients with hemophilia. This study aimed to assess joint health and functional independence in individuals with hemophilia using the HJHS an

BACKGROUND: Epidural anesthesia is generally avoided in patients with hemophilia due to the risk of spinal or epidural hematoma. However, with careful factor replacement and perioperative management, neuraxial techniques may be feasible in selected cases. Mechanical complications, such as catheter twisting, are rare but can mimic serious neurological events, requiring careful differential diagnosis.
