Brain mechanisms supporting movement-evoked chronic low back pain: an observational brain imaging study.

BACKGROUND: Chronic low back pain (cLBP) is the most prevalent chronic pain condition and the leading cause of disability worldwide. Acute movement-evoked pain strongly predicts cLBP exacerbation, reflecting maladaptive amplification of nociceptive input, yet the neural mechanisms supporting movement-evoked cLBP flares remain poorly characterised. METHODS: A total of 114 participants with cLBP performed the cLBP-evoking leg raise test during perfusion-based arterial spin labelling functional MRI. Leg raise test-based pain intensity and unpleasantness ratings were assessed with an 11-point vis

Fast Analysis of RNA from Transcription and Rho-Dependent Termination Assays Using Automated Capillary Electrophoresis.

Conventional analysis of RNA products synthesized by bacterial RNA polymerase (RNAP) in in vitro transcription assays relies on radioactive labeling, followed by separation of labeled RNAs by denaturing urea-polyacrylamide gel electrophoresis (PAGE). This approach provides excellent resolution across a wide range of RNA lengths (from 2 nt to several hundred nucleotides), but the use of radioisotopes requires specialized facilities, strict waste-disposal procedures, and is often restricted. To circumvent these limitations, we adapted a standard in vitro transcription assay for use with the Agil

State of the global tuberculosis epidemic: burden, progress towards WHO End TB Strategy targets and systemic challenges.

Mycobacterium tuberculosis remains the leading cause of death from a single infectious pathogen globally despite decades of effective chemotherapy. In 2024, an estimated 10·7 million people developed tuberculosis, including approximately 620 000 people living with HIV (PLHIV), and tuberculosis caused an estimated 1·23 million deaths overall, including approximately 150 000 deaths among PLHIV. Men accounted for more than half of the cases, and children represented a substantial burden, reflecting ongoing transmission and diagnostic gaps. Approximately a quarter of the world's population has bee

Minimal supply chain and forecasting testing volumes for clinical bacteriology laboratories in resource-limited settings for individual patient care: a costing study.

BACKGROUND: Inadequate access to clinical bacteriology testing in low-resource settings compromises individual patient care and contributes to antimicrobial resistance. Funders, policy makers, and health administrators working to expand bacteriology testing in sub-Saharan Africa require evidence-based guidance. The aim of this study was to assemble a costed consensus list of minimum equipment and consumables required to implement and operate a functional clinical bacteriology laboratory in secondary and tertiary care hospitals in sub-Saharan Africa, to guide funders and health administrators.

Advances in tuberculosis diagnostics and biomarkers, 2020-26: scientific evidence, recommendations, gaps, and developmental priorities.

Major advances in tuberculosis diagnostics and biomarker research from 2020 to 2025 have informed updated WHO policy guidance, consolidating recommendations for the detection of tuberculosis disease and drug resistance within a single framework. These recommendations align screening, diagnosis, and drug-resistance testing across populations and specimen types. Digital chest radiography, increasingly supported by computer-aided detection, is now a core WHO-recommended screening and triage tool, improving case detection and referral for confirmatory testing at scale. Decentralised molecular diag

Advances in development of tuberculosis vaccines and preventive drug therapies: current pipeline, evidence, and access.

Tuberculosis remains the leading cause of death from a single infectious agent globally. The WHO Global Tuberculosis Report 2025 indicates that progress towards the WHO End TB Strategy targets remains off track despite modest declines in incidence and mortality. In 2024, an estimated 10·7 million people developed tuberculosis and 1·23 million died from the disease. In this Series paper, we examine evidence published between Jan 6, 2020, and Jan 6, 2026, to provide an updated overview of tuberculosis vaccines and tuberculosis preventive treatment (TPT). As of June 4, 2026, 18 tuberculosis vacci

Advances in tuberculosis treatment: novel regimens, new drug pipelines, host-directed therapies, and updated management guidelines.

Tuberculosis treatment has undergone its most profound transformation since the launch of the standardised DOTS strategy in 1994. Since 2020, a series of pivotal randomised trials, including TB-PRACTECAL, ZeNix, Nix-TB, endTB, BEAT-TB, SHINE, and Study 31/A5349, have redefined the management of both drug-susceptible and drug-resistant tuberculosis. These studies have enabled shorter, fully oral regimens with improved efficacy and safety across adult and paediatric populations, including people with HIV, and have driven major updates to WHO treatment guidelines. Despite these advances, tubercul

Diagnostic performance and testing costs of pooled testing for molecular detection of tuberculosis in seven countries: a prospective, observational, diagnostic accuracy study.

BACKGROUND: Globally in 2024, only 54% of people diagnosed with tuberculosis received an initial WHO-recommended rapid molecular diagnostic test. Pooled testing could expand access to molecular detection using existing laboratory capacity. We aimed to evaluate the performance of pooled tuberculosis testing in a multicentre diagnostic accuracy study. METHODS: In this prospective diagnostic accuracy study, we enrolled individuals with presumptive tuberculosis through facility-based case finding (district hospitals and primary health centres) and community-based case finding in Bangladesh, Brazi

Coronal curvature angle and asymmetrical blood vessel distribution in intervertebral discs from aged cadavers: a pilot study.

PURPOSE: Intervertebral disc degeneration (IVDD) is a major cause of chronic spinal pain and increases with age. Although spinal deformities contribute to asymmetric IVDD, vascular asymmetry within the intervertebral disc (IVD) remains unclear. This study investigated the relationship between differences in vascularity between the concave and convex sides of the IVD and the coronal curvature angle. METHODS: The study comprised two parts: Study A and Study B. Study A included 15 IVDs (C7-T1, T1-T2, and T7-T8; five discs per level) obtained from five cadavers (two females). Study B included 53

ROS-scavenging microspheres loaded with extracellular vesicles for intervertebral disc degeneration therapy.

Intervertebral disc degeneration (IVDD), primarily driven by oxidative stress and inflammation, significantly impacts patient quality of life. Current therapies lack efficacy, highlighting the need for novel treatment strategies. This study investigates the protective effect of antioxidant hydrogel microspheres containing black phosphorus (BP) nanosheets and extracellular vesicles (EVs), fabricated using a microfluidic technology-based delivery system, designated as EVs@BPMS. In vitro analyses of EVs@BPMS revealed that BP nanosheets enhanced the antioxidant capacity of the hydrogel microsphere

Multimodel RBE and Dose-Averaged LET Optimized Carbon Ion Radiation Therapy for Sacral Chordoma: Results of the First Year of Clinical Application.

PURPOSE: Carbon ion therapy (CIRT) optimization, driven by purely dosimetric and single-relative biological effectiveness (RBE) model may result in suboptimal solutions, particularly when considering dose-averaged LET (LETd) within the tumor. This study reports on the early outcomes of a clinical optimization strategy combining compliance with multi-RBE and explicit LETd escalation in a cohort of 10 patients with sacral chordoma treated with CIRT. METHODS AND MATERIALS: Treatment plans, optimized with dosimetric objectives to a prescribed RBE-weighted dose (DRBE) of 73.6 Gy(RBE), using the lo

VERIDAH: solving enumeration anomaly-aware vertebra labeling across imaging sequences.

OBJECTIVES: Although the identification of enumeration anomalies has potential clinical implications for chronic back pain and operation planning, the thoracolumbar junction is poorly assessed and rarely described in clinical reports. Additionally, even though multiple deep-learning-based vertebra labeling algorithms exist, there is a lack of methods to automatically label enumeration anomalies. MATERIALS AND METHODS: We introduce "Vertebra Identification with Anomaly Handling" (VERIDAH), a novel vertebra labeling algorithm based on multiple classification heads combined with a weighted verte

Can Pillow Use Prevent Postdural Puncture Headache? A Critical Review.

Postdural puncture headache (PDPH) is a recognized complication of neuraxial anesthesia that follows unintentional dural puncture during epidural techniques or intentional dural puncture during spinal anesthesia and other neuraxial procedures. It has traditionally been managed with a period of mandatory supine bed rest and pillow restriction, a practice grounded in the historical belief that recumbency limits ongoing cerebrospinal fluid (CSF) leakage, thereby preventing headaches. This "no-pillow" instruction remains widely taught and practiced, including among clinicians, despite an absence o

Duplex ultrasound as an initial imaging clue to spontaneous isolated celiac artery dissection: A case report.

Spontaneous isolated celiac artery dissection is uncommon and is usually confirmed with computed tomography angiography (CTA). Duplex ultrasound may provide an initial clue when the lesion and its flow abnormalities are directly visualized. A 56-year-old woman with untreated hypertension presented with acute persistent epigastric pain radiating to the back. Color Doppler ultrasound at the celiac artery origin demonstrated an intimal flap separating the true and false lumens, with an adjacent hypoechoic component without detectable flow. Spectral Doppler sampling within the dissected segment sh
