Ultrasound estimated connective tissue proportion is lower after 6 months of restorative neurostimulation therapy: an exploratory analysis.

PURPOSE: To assess whether ultrasound-estimated connective tissue proportion in the human lumbar multifidus muscle is changed by restorative neurostimulation, and whether changes are associated with clinical outcomes. METHODS: Fifty individuals with mechanical chronic low back pain (cLBP) who were scheduled to receive a neurostimulation device to evoke contraction of the multifidus muscle via stimulation of the medial branch of the dorsal ramus at L2 were invited to participate. Connective tissue features of the multifidus and muscle cross-sectional area were extracted from transverse B-mode

Discussing pain increases at the point of exercise prescription: a clinical commentary for persistent musculoskeletal conditions.

BACKGROUND: Patients with persistent musculoskeletal pain frequently perform prescribed exercise without direct supervision. When exercise increases symptoms, patients may be uncertain whether to continue, modify the session, or seek advice. A prescription that specifies exercises and dosage but provides no guidance about possible symptom aggravation may therefore leave an important gap in preparation. PURPOSE: This commentary proposes that, when prescribed exercise may increase pain, the possible response should be discussed at the point of prescription and that clinicians should prepare pat

F-Wave-Induced Repetitive Discharges: Expanding the Spectrum of Stimulus-Induced Repetitive Discharges.

INTRODUCTION/AIMS: The clinical significance of stereotyped post-F-wave discharges remains uncertain. We aimed to characterize F-wave-induced repetitive discharges (FIRDs) and their associated clinical and electrodiagnostic findings. METHODS: We searched the Mayo Clinic electrodiagnostic database (January 1997-December 2025) using "F-wave" and "repetitive discharges" and reviewed retrieved waveforms. FIRDs were defined by repetitive discharges following F-wave activation, reproducible morphology within a train, and regular or semiregular firing. RESULTS: The study included 20 patients with F

Illuminating Antarctic Microbial Dark Matter: In situ Cultivation Reveals Rare Taxa with Broad Resistome and Virulence Phenotypes.

Glacial forelands in Maritime Antarctica are rapidly expanding due to deglaciation, thereby increasing ecological connectivity among previously isolated microbial reservoirs. From a One Health perspective, this process is relevant because environmental microorganisms represent long-standing reservoirs of antibiotic resistance and virulence-associated traits. However, current knowledge is mainly limited to dominant, fast-growing taxa recovered by conventional cultivation, leaving the functional potential of slow-growing and rare Antarctic bacteria largely unexplored. Here, we compared tradition

Halotolerant Brucella intermedia PM63 enhances growth, ion homeostasis, antioxidant defense, and salt stress tolerance in rice.

Soil salinity limits rice growth through osmotic stress, ionic imbalance, and oxidative damage. In this study, halotolerant plant growth-promoting bacteria were isolated from Peganum harmala collected at the Khewra Salt Mine, Pakistan, and one of the best isolates, Brucella intermedia PM63, was evaluated for its ability to improve salt tolerance in rice. Ten bacterial isolates were screened for salt tolerance and plant growth-promoting (PGP) traits, and three selected strains were characterized by phenotypic tests, FTIR, SEM, 16S rRNA sequencing, and PCR detection of functional genes. PM63 tol

Agri-food connectivity drives the resurgence and geographic expansion of human brucellosis in China.

Agri-food systems are increasingly recognised as drivers of zoonotic disease emergence, yet the mechanisms by which modern food production, trade, and distribution reshape pathogen transmission remain poorly quantified. Here we integrate two decades of nationwide surveillance, epidemiological modelling, and phylogenomic analyses of 4215 Brucella genomes from 79 countries and regions to elucidate the drivers of brucellosis re-emergence in China. Nationwide, human cases increased by 282.6% between 2005 and 2023, far exceeding the 8.1% increase in sheep inventory. This decoupling indicates a fund

Unveiling Equine Pathologies from an Iron Age Ritual Deposit: Health, Management and Biomechanical Stress in the Protohistoric Horses of Casas del Turuñuelo (Badajoz, Spain).

Equids played a central economic and symbolic role in Iberian Iron Age society, yet little is known about their health and management during the protohistoric period. This study examines the unusually well-preserved equid assemblage from the Tartessian site of Casas del Turuñuelo (Badajoz, Spain), dating from the 5th century BCE with the aim of reconstructing patterns of pathology and biomechanical stress. Thirty-one individuals were subjected to macroscopic paleopathological assessment following established diagnostic criteria and, for a subset of seven individuals, computed tomography (CT) o

S1 Dual Outer Diameter Screws Accelerate Early Fusion Compared with Conventional Pedicle Screws in Posterior Lumbosacral Interbody Fusion.

INTRODUCTION: Achieving successful lumbosacral interbody fusion is challenging due to the unique anatomical characteristics of the sacrum. The predominantly cancellous bone and short pedicles limit screw purchase, which frequently leads to S1 screw loosening and pseudarthrosis, resulting in persistent pain and poor outcomes. Therefore, the S1 Dual Outer Diameter (DOD) screw, featuring a dual-zone thread design with greater thread depth and sharper edges proximally and reduced thread depth with blunter edges distally, was developed to maximize screw purchase in cancellous and cortical bones. A Characterisation of Spinal Abnormalities in a Cohort with Neurofibromatosis Type 1 with Dural Ectasia.

INTRODUCTION: Dural ectasia is a known manifestation of Neurofibromatosis type 1 (NF1). Despite its potential to cause severe complications such as spinal instability, further characterisation of its associated spinal abnormalities remains clinically important. This is a retrospective review reporting the characteristics and prevalence of spinal abnormalities in patients with concurrent NF1 and dural ectasia. METHOD: The data was collected from a period of 5 years from a cohort of 132 patients with dural ectasia and NF1. The data was collected from multidisciplinary team meeting reports, and

Relationship between paraspinal muscle degeneration and the global alignment and proportion (GAP) score: cross-sectional study in middle-aged and older adults.

BACKGROUND: The paraspinal muscle, a crucial component of the stable sagittal balance of the spine, is closely associated with the structural degeneration of ligaments. The global alignment and proportion (GAP) score can predict spinal pelvic ligament degeneration and examine the overall sagittal balance of the spine. This study aimed to determine the correlation between the paraspinal muscles and the GAP score using imaging evaluation and to investigate better methods for evaluating the sagittal balance of the spine compared to spinal pelvic parameters. METHODS: This study enrolled 61 patien

Biomedical functionalities and teratogenic risks of bioengineered ZnO nanoparticles in post-fertilization zebrafish (Danio rerio) embryos.

Zinc oxide nanoparticles (ZnO NPs) are widely utilized in several commercial applications. Although their toxicity and associated oxidative stress have been extensively studied, the toxicity they induce during embryonic development remains largely unknown. In this study, ZnO NPs were fabricated via a green synthesis approach using the leaf extract of Aegle marmelos (Am@ZnO NPs), and their developmental toxicity toward post-fertilized zebrafish embryos was investigated. The synthesized Am@ZnO NPs exhibited a UV-Vis absorbance peak at 372 nm. X-ray diffraction (XRD) analysis confirmed the presen
