Microvasculopathy in spinal muscular atrophy: The endothelium as a target of SMN deficiency and an amplifier of disease.

Spinal muscular atrophy (SMA) is caused by deficiency of the ubiquitous survival motor neuron (SMN) protein. Degeneration of anterior horn motor neurons defines the disease, but many organs are affected, including heart, liver, kidney, and bone. The vascular compartment is among them: digital necrosis in infants with the fewest SMN2 copies, capillary rarefaction in SMA mice, and decreased von Willebrand factor with fibrinogen extravasation in human post-mortem spinal cord. Whether these reflect endothelial injury by SMN deficiency itself or capillary adaptation to atrophic, denervated tissue h

Dynamic force sensing by bone delays aging and extends lifespan.

Physical activity promotes health primarily by decelerating the aging process. Bone is one of the tissues capable of sensing mechanical force and can adapt to different physical demands. However, its role in whole-body aging is unknown. We found that reduced mechano-sensing in osteocytes, caused by deletion of the mechanosensory channel, Piezo1, accelerated whole-body aging in mice. Dynamic tibial axial loading (DTAL) in mice at 20-24 months of age extended both healthspan and lifespan (9.4%/79.5 days). Furthermore, DTAL reversed bone marrow immunosenescence and induced anti-aging and protecti

Development of Enterovirus D68 3C Protease Inhibitors with In Vivo Efficacy in a Neurological Infection Model.

Enterovirus D68 (EV-D68) is a non-polio enterovirus that primarily causes respiratory illness but can also lead to severe disease, including acute flaccid myelitis. No approved vaccine or antiviral is available. Here, we report the rational design and structure-property optimization of dipeptidyl EV-D68 3C protease (3Cpro) inhibitors. Structure-guided optimization and prodrug modification identified Jun13133 as a potent antiviral active against multiple contemporary EV-D68 isolates and EV-A71, with high selectivity. Computational modeling supported a stable substrate-mimetic binding mode consi

Precision Nanomedicine for Traumatic Neural Injury: Functional Paradigms and Translational Frameworks.

Traumatic neural injuries, including traumatic brain injury (TBI), spinal cord injury (SCI), and peripheral nerve injury (PNI), impose a substantial global burden. These injuries converge on oxidative stress, neuroinflammation, regulated cell death, demyelination, and extracellular matrix remodeling, while diverging in timing, cellular effectors, and regenerative capacity. Current management still fails to arrest secondary injury or restore functional neural circuitry. Nanomedicine integrates targeted delivery, microenvironmental regulation, and regenerative support, yet existing strategies an

Investigating potential molecular mechanisms linking 6PPD-quinone exposure to Intervertebral Disc Degeneration: A network toxicology, molecular docking, and molecular dynamics simulation study.

As an emerging pollutant derived from tire rubber, 6PPD-quinone (6PPD-Q) has raised public health concerns due to widespread environmental exposure; however, its potential toxicological effects on avascular tissues such as intervertebral discs remain unclear. This study integrated network toxicology, molecular docking, and molecular dynamics (MD) simulation to investigate the potential mechanisms linking 6PPD-Q exposure to intervertebral disc degeneration (IVDD). ADMET toxicological predictions indicated high model-predicted probabilities for SR-MMP and SR-ARE activation. Through multidimensio

Bacteria inside kidney tumors associate with myeloid inflammation and disease progression: A TRACERx Renal study.

BACKGROUND: Intratumoral bacteria are increasingly recognized as active contributors to neutrophil-driven inflammation and disease progression in solid tumors. Yet, in clear cell renal cell carcinoma (ccRCC), where myeloid inflammation is a key feature of aggressive disease, live bacterial presence, their intratumoral distribution, and interaction with the tumor microenvironment remain unexplored. METHODS: We genetically characterized the microbiome of primary ccRCC tumors across two independent cohorts (TRACERx: 616 samples, 143 patients; The Cancer Genome Atlas [TCGA]: 511 samples and patie

Control of M. tuberculosis infection requires CD28 to generate lung-homing T cells and CTLA-4 to limit immunopathology.

Mycobacterium tuberculosis (Mtb)-specific CD4 T cells differentiate into protective lung parenchyma-homing and non-protective intravascular subsets, but the signals that instruct these fates are poorly understood. Here we show that CD28 co-stimulation regulates host protection during Mtb infection. Cd28-/- mice are susceptible to Mtb infection, displaying higher bacterial loads and reduced survival compared to wild type mice. Cd28-/- Mtb-specific T cells expand normally but are skewed towards the lung intravascular phenotype. CD4 T cells primed in the absence of CD28 co-stimulation in B7 defic

18F-Fluorodeoxyglucose PET/CT in Tuberculosis and Other Granulomatous Diseases (except Sarcoidosis and Fungal Disease).

Granulomatous inflammation consists mainly of activated macrophages, epithelioid cells, and lymphocytes, which have high glucose metabolism and therefore show increased 18F-fluorodeoxyglucose (18F-FDG) uptake on PET/computed tomography (CT). This makes FDG PET/CT useful in detecting and mapping granulomatous disease, including clinically silent or extrapulmonary involvement. It is particularly crucial in tuberculosis, where it can help identify active disease, guide biopsy, assess treatment response, and detect dissemination. FDG PET/CT also has applications in granulomatous vasculitis such as

Near-infrared-activated CuTA@PDA@PTH nanozymes remodel the osteoarthritic microenvironment for coordinated cartilage protection and subchondral bone repair.

Osteoarthritis (OA) is a progressive whole-joint disorder involving cartilage degeneration, oxidative stress, synovial inflammation, and subchondral bone remodeling. Current treatments mainly provide symptomatic relief and rarely restore joint microenvironmental homeostasis. Here, we developed a multifunctional near-infrared (NIR)-responsive CuTA@PDA@PTH nanozyme, termed CPP, for OA therapy. CPP was constructed through copper-tannic acid coordination, polydopamine coating, and PTH 1-34 incorporation, integrating antioxidant catalytic activity, photothermal responsiveness, and peptide-mediated

18F-fluorodeoxyglucose Positron Emission Tomography with Computed Tomography in Pediatric Non-oncological Conditions: A Comprehensive Review.

This article examines the expanding role of 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography with computed tomography (PET/CT) in pediatric non-oncological conditions, moving beyond its established oncologic use. It outlines radiation dosimetry principles unique to children-emphasizing the As Low as Reasonably Achievable principle, reduced tracer activity, and low-dose CT protocols-given their heightened radiosensitivity. In pediatric neurology, FDG PET/CT aids evaluation of drug-refractory epilepsy (temporal and extratemporal foci) and neuronal migration/proliferation anomalies l

Acute activation of autophagy enables growth plate regeneration following radiation-induced injury.

PURPOSE: Radiation injury to growth plates commonly leads to skeletal late complications including short stature, limb length-discrepancy, and scoliosis/kyphosis in pediatric oncology patients. We aimed to understand the acute responses of direct growth plate irradiation that result in skeletal late complications. MATERIALS AND METHODS: We first established an in vivo murine model of focal growth plate irradiation that recapitulates the clinical development of skeletal late complications and used it to explore the responses of growth plate chondrocytes within the first 72 hours of radiation e

AX-35 Targets the Cytochrome bc1 Complex in Mycobacterium abscessus and Reveals a Novel Resistance Mechanism.

In patients with cystic fibrosis, nontuberculous mycobacteria are opportunistic, intracellular and extracellular pulmonary pathogens that contribute to deterioration of lung function and, ultimately, morbidity from respiratory failure. Among nontuberculous mycobacterial respiratory pathogens, members of the Mycobacterium abscessus complex pose additional treatment complications as they are notoriously drug-resistant. As part of our routine screening against mycobacterial species, one of the lead antituberculosis compounds, the cytochrome bc1 inhibitor AX-35, was found to be active against isol

The impact on scoliosis development and the need for bracing after posterior fossa decompression for Chiari Malformation type I and de-tethering for tethered cord syndrome: a retrospective study of 171 patients.

PURPOSE: Scoliosis is frequently associated with CM1 and TCS, but its course following neurosurgical treatment remains incompletely understood. The aim of this study was to describe scoliosis development following posterior fossa decompression for CM1 or de-tethering for TCS. A secondary aim was to describe postoperative brace treatment and its association with scoliosis progression. METHODS: This was a retrospective study including children operated consecutively from the age of one to eighteen years for CM1 or TCS. We included patients with and without preoperative scoliosis who had a last

Tau isoform mapping across tissues and biofluids reveals a signature distinguishing Alzheimer's disease from amyotrophic lateral sclerosis.

Blood phosphorylated tau 217 (p-tau217) accurately detects Alzheimer's disease (AD) pathology but is also elevated in amyotrophic lateral sclerosis (ALS), likely reflecting peripheral (big) tau released from muscle. Assays selectively measuring brain-derived (BD-) tau and BD-p-tau217 are therefore needed. We analysed serum BD-tau and p-tau217 in ALS (n = 139), AD (n = 101) and controls (n = 75) using direct immunoassays and after BD-Tau immunoprecipitation (BD-Tau-IP), and tau species in brain (n = 16), spinal cord (n = 11) and muscle (n = 10) by targeted mass spectrometry. False-Negative Ligamentum Teres Tendon Sampling for the Detection of Systemic Amyloidosis.

This case report involves a 60-year-old man with bilateral carpal tunnel syndrome, hip osteoarthritis, and lumbar spinal stenosis who underwent biopsies of the ligamentum teres, ligamentum flavum, and carpal flexor tenosynovium. Amyloid deposits were identified in the ligamentum flavum and flexor tenosynovium, but not the ligamentum teres. Amyloid was typed by mass spectrometry, cardiac scintigraphy was negative, and genetic testing remains pending. This supports that ligamentum flavum and carpal flexor tenosynovium biopsies may have better diagnostic value for the early diagnosis of amyloidos

Suzetrigine (Journavx) as a Non-Opioid Adjuvant for Cancer-Associated Pain: A Case Series.

Opioids remain central to managing cancer-associated pain (CAP) but are often limited by constipation, nausea, sedation, and concerns about maladaptive use amid the ongoing opioid epidemic. Suzetrigine (Journavx), a first-in-class, non-opioid, selective NaV1.8 sodium channel inhibitor, was approved by the U.S. Food and Drug Administration in January 2025 for moderate-to-severe acute pain; its role in chronic pain syndromes such as CAP has not been established. This case series describes two patients with CAP of differing pathophysiology started on suzetrigine as an off-label adjuvant analgesic
