Outcomes after MI-TLIF without bone morphogenetic protein: a retrospective study.

OBJECTIVE: Minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) is widely used to treat degenerative lumbar spine conditions, offering reduced tissue trauma, faster recovery, and lower complication rates compared to those with open TLIF. Recombinant human bone morphogenetic protein 2 (rhBMP-2) can enhance fusion and is often used in the interbody space in minimally invasive fusions because of the limited posterolateral surface area for arthrodesis, but it is associated with higher costs and an increased risk of complications. Authors of this study evaluated the outcomes of MI-TL

Achieving "ideal" sagittal alignment does not influence HRQoL outcomes 5 years following PSF for lenke 1 AIS.

INTRODUCTION: Previous studies have emphasized the importance of thoracic kyphosis (TK) restoration and sagittal alignment for long-term outcomes following posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). Although various equations have been introduced to predict the ideal regional alignment of the thoracic or lumbar spine, no study has validated these equations or investigated the influence of achieving these patient-specific ideal regional parameters on patient-reported health-related quality of life (HRQoL) outcomes following PSF for AIS. METHODS: A retrospective co

Multidimensional Management of Degenerative Scoliosis: A Comprehensive Narrative Review Integrating Diagnostic Assessment, Surgical Decision-Making, and Patient-Centered Outcomes.

Degenerative scoliosis (DS) is a common spinal deformity in aging populations and an important contributor to low back pain, neurological symptoms, and functional decline. Unlike adolescent idiopathic scoliosis, DS develops in a previously non-scoliotic spine through progressive asymmetric degeneration of the intervertebral disks, facet joints, and supporting spinal structures. Despite its growing clinical burden, management remains heterogeneous, with decision-making often fragmented across radiographic assessment, treatment planning, and postoperative outcome evaluation. Building on previous

Evaluation of Changes in Spinal Posture Following Fluoroscopy-Guided Transforaminal Epidural Steroid Injection in Chronic Low Back Pain using the DIERS Formetric 4D Spine Posture Analysis System: A Prospective Observational Study.

INTRODUCTION: Chronic low back pain (CLBP) is a common cause of pain, disability, and impaired quality of life. Persistent pain often leads to compensatory postural changes, including pelvic asymmetry, coronal imbalance, and vertebral rotation. Although fluoroscopy-guided transforaminal epidural steroid injection (TFESI) is an established treatment for patients who fail conservative therapy, its effects on spinal posture have not been well studied. This study aimed to evaluate changes in spinal posture following TFESI using the radiation-free DIERS formetric 4D spine posture analysis system, a

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

Association between flexibility assessment based on prone full-spine CT scout view and surgical strategy for old thoracolumbar fracture with kyphotic deformity.

OBJECTIVE: This study aimed to investigate whether spinal flexibility, assessed by the alignment difference between Full-length Spine CT scout view (FLS-CT) and standing radiographs, was associated with the eventual use of three-column osteotomy (3CO) in patients with old thoracolumbar fracture with kyphotic deformity (OTLFK). METHODS: This single-center, dual-surgeon retrospective study included patients with OTLFK who underwent corrective surgery between August 2021 and August 2024. Patients were categorized into the 3CO group and the non-three-column osteotomy (N3CO) group according to whe

Conservative treatment with continued sports participation for early-stage lumbar spondylolysis: An exploratory study of bone union outcomes in adolescent athletes.

BACKGROUND: Conservative treatment for lumbar spondylolysis traditionally requires cessation of sports activities until bone union is achieved. However, prolonged interruption of sports participation can be difficult for adolescent athletes. This study investigated the bone union outcomes of conservative treatment allowing continued sports participation in patients with lumbar spondylolysis. METHODS: Pediatric athletes (≤18 years) diagnosed with very early- or early-stage lumbar spondylolysis between April 2023 and October 2024 were retrospectively reviewed. Patients selected either conservat

Schroth exercises added to bracing for adolescent idiopathic scoliosis: a systematic review and meta-analysis.

BACKGROUND: Bracing is the principal non-operative treatment for skeletally immature adolescents with idiopathic scoliosis. Schroth exercises are often adjunctively prescribed; however, their incremental value over bracing alone remains uncertain. OBJECTIVE: To evaluate the effectiveness, safety, adherence, and treatment burden of Schroth or explicitly Schroth-based exercises added to bracing compared with bracing alone. METHODS: This PROSPERO-registered systematic review and meta-analysis followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) 2020 principle

Exploratory evaluation of spinopelvic and lumbar parameters in adolescent athletes with isthmic spondylolisthesis: feasibility and reliability of upright MRI compared to radiography.

INTRODUCTION: Isthmic spondylolisthesis is prevalent among adolescent athletes (AA). Upright MRI (uMRI) offers a radiation-free alternative to radiographs, combining imaging of the loaded spine with detailed depiction of the adjacent structures. This study evaluates the feasibility and intra-rater measurement repeatability of spinopelvic and lumbar parameter assessment in AA using uMRI and radiography, as well as cross-modality agreement between both imaging techniques. MATERIAL AND METHODS: Lumbar sagittal T2-weighted low-field uMRI and lateral standing radiographs from 38 AA with isthmic sp

Keystone Flap, a Versatile Option for Salvaging Infected Spinal Instrumentation Associated to Soft-tissue Defects: A Novel Option.

Keystone design perforator island flaps (KDPIFs) are fasciocutaneous flaps based on multiple perforators that provide reliable perfusion to large tissue segments. Their design incorporates elements of advancement, rotation, interpolation, and transposition, allowing recruitment of surrounding lax tissue to achieve durable coverage with minimal tension. These characteristics make them particularly useful for reconstruction of complex posterior trunk defects. We present the case of a 23-year-old man with severe neuromuscular scoliosis who underwent posterior spinal instrumentation from T3 to L5

Discordance among skeletal maturity indicators: sources, interpretation, and clinical implications.

Bone age and regional skeletal maturity staging are widely used to identify growth stage, estimate remaining growth, and support orthopedic treatment decisions in children and adolescents. However, skeletal maturity indicators differ in the regions examined, developmental features observed, scoring rules applied, and reference standards used. Consequently, the same child may receive different age estimates or maturity classifications when a different bone group, anatomical region, imaging modality, or assessment method is selected. This narrative review synthesizes evidence from within-image b

Full versus partial correction of pelvic incidence-lumbar lordosis mismatch in lumbar fusion: a systematic review and meta-analysis.

PURPOSE: Restoration of sagittal balance, specifically the mismatch between pelvic incidence and lumbar lordosis (PI-LL), is an important objective in lumbar fusion, particularly for degenerative deformity correction. While a postoperative PI-LL of ≤ 10° is generally considered optimal, the clinical necessity of achieving this threshold versus partial correction (PI-LL > 10°) remains uncertain. This study aimed to systematically review the differences in disability outcomes and mechanical complications between "full" (≤ 10°) versus "partial" (> 10°) PI-LL correction. METHODS: Ovid Embase, Ovi

Dr. John Emmett Hall: a legacy in spine surgery.

The study of biography is important to understand the principles of our profession and the intersections of the lives of the key figures in our spine deformity history. We hope that by telling these stories this history will come alive. The surgeons who came before us, as exemplified by Dr. John Emmett Hall, understood the importance of unwavering principles, developing close professional relationships, the value of travel, and the power of collaboration. The legacy of his full life is summed up by the plaque placed by the nurses in his operating room, at Boston Children's Hospital: "The Dr. J

Aortic elongation with prosthetic graft followed by aortopexy for fixed left main bronchial compression in an adolescent with severe thoracic deformity: A case report.

Airway compression in older children is uncommon and becomes more challenging when thoracic deformities alter aortic anatomy. A 16-year-old girl with severe scoliosis and chronic bedridden status presented with progressive respiratory distress caused by compression of the left main bronchus between the ascending aorta and a right-sided descending aorta. As conventional aortopexy alone was unlikely to achieve adequate decompression, aortic elongation using a prosthetic graft was performed to reposition the descending aorta posteriorly, followed by aortopexy. Postoperative imaging confirmed comp

Scoliosis Induction in the Porcine Spine Using a Growth-Modulating Posterior Tether: Finite Element Model Development and Calibration.

PURPOSE: Predictable correction of early onset scoliosis through mechanical modulation of endochondral growth requires understanding vertebral stress-growth relationships. Here, we develop image-based finite element models (FEMs) to estimate vertebral stress-growth relationships under prescribed loading conditions using a porcine model of spine deformity induced mechanically via a posterolateral tether, assuming that, after initial tether-induced elastic deformation, subsequent deformity develops solely from vertebral endochondral growth against the tether and elastic anatomical structures.
