Persistence of Bacterial Dysbiosis, Cervicovaginal Human Papillomaviruses, and Inflammation in Women Living With HIV-1 After a Year of Antiretroviral Therapy.

BACKGROUND: The cervicovaginal microbiome may affect susceptibility to human immunodeficiency virus (HIV) -1 and can in turn influence the prevalence and clinical course of HIV-1 and other sexually transmitted diseases. Because the determinants, immunological correlates, and clinical effects of the dysbiosis observed in women with HIV-1 (WWH) remain elusive, we evaluated the vaginal immunological milieu, cervicovaginal microbiome, and prevalence of human papillomaviruses (HPVs) in antiretroviral-naive WWH over their first year of antiretroviral therapy (ART). METHODS: 83 ART-naive Ugandan WWH

Limited impact of HIV infection and immune status on the gut mycobiota.

BACKGROUND: A growing body of literature connects the gut mycobiota to several diseases, including HIV infection. In contrast to bacteria, fungi represent a smaller fraction of the gut microbiota and pose several technical challenges, including low biomass, environmental contamination and primer-associated biases. Characterizing the gut mycobiota in HIV infection is still at an early stage. METHODS: We characterized the gut mycobiota in untreated people living with HIV (PWH, n = 69), PWH on antiretroviral therapy (PWHA) for at least 2 years (n = 14) and people without HIV (PWoH, n = 48) using

VIBRANT: A phase 1 randomized trial of multi-strain vaginal L. crispatus live biotherapeutic products in people with bacterial vaginosis.

Bacterial vaginosis (BV) is characterized by high microbial diversity. High recurrence rates following antibiotics may stem from poor recolonization by protective Lactobacillus species. This phase 1 randomized trial in the United States and South Africa evaluated two vaginally delivered live biotherapeutic products (LBPs) containing multiple Lactobacillus crispatus strains. After metronidazole treatment for BV, participants received either a placebo or 3 or 7 days of active LBPs. LBP strains were detected by metagenomics in 66.1% (47/71) of participants in the active arms in the first 5 weeks.

Assessing the Impact of Female Genital Mutilation/Cutting on Genital Inflammation and Microbiota Among Kenyan Female Sex Workers.

PROBLEM: Female genital mutilation/cutting (FGM/C) is harmful to physical, mental, and reproductive health, though the effect of this practice on a woman's HIV susceptibility is poorly understood. Despite the known associations of FGM/C with short-term vaginal epithelial damage, neither genital inflammation nor the genital microbiome have been explored in women who have undergone FGM/C. In this study we compare the genital immune milieu and microbiome among female sex workers (FSWs) by FGM/C status, hypothesizing that these biological factors are dysregulated in women who have undergone FGM/C,

Genital inflammation screening for predicting sexually transmitted infections and bacterial vaginosis: an updated cost analysis from the GIFT study in Madagascar, South Africa, and Zimbabwe.

BACKGROUND: Sexually transmitted infections (STIs) and bacterial vaginosis (BV) are significant public health issues, particularly in sub-Saharan Africa, and are associated with genital inflammation and increased HIV acquisition risk. A substantial proportion of these infections are asymptomatic, limiting the effectiveness of syndromic management. The Genital InFlammation Test (GIFT), a novel rapid point-of-care (POC) test, was developed to detect elevated inflammatory biomarkers associated with genital inflammation. The first-in-field prototype of the GIFT device was evaluated in a multicente

Positive Nucleic Acid Amplification Test Results Are Rarely Indicative of a Viable Rectal Chlamydia trachomatis Infection Among Men With Anal Exposures Other Than Receptive Anal Sex.

In 2019-2022, we enrolled men who have sex with men who did not engage in receptive anal sex but had other anorectal exposures (e.g., rimming). Of 29 men who tested positive for rectal chlamydia by nucleic acid amplification testing, only 10 (34.5%) were viable by culture or viability PCR.

Increasing HIV Testing During Gonorrhea and Chlamydia Evaluations in Urgent Care and Emergency Departments: A Large Health System Initiative.

BACKGROUND: HIV testing opportunities are often missed during urgent care (UC) and emergency department (ED) evaluations for sexually transmitted infections. We implemented a multimodal diagnostic stewardship intervention to increase HIV testing in UC and ED gonorrhea (GC)/chlamydia (CT) testing encounters ("HIV co-testing"). METHODS: We conducted a pre-/post implementation study across 26 UCs and 22 EDs in our health system (April 2022-March 2024). The intervention included system-wide simultaneous implementation of provider and patient education and a link-to-care program for newly diagnose

High in vitro activity of the novel antimicrobial gepotidacin against Neisseria gonorrhoeae isolates in eight WHO Enhanced Gonococcal Antimicrobial Surveillance Programme countries in three WHO regions, 2021-24.

OBJECTIVES: The spread of ceftriaxone-resistant Neisseria gonorrhoeae is threatening the last option for gonorrhoea treatment, ceftriaxone. Gepotidacin, the first-in-class triazaacenaphthylene bacterial topoisomerase type IIA inhibitor, recently showed non-inferiority compared to ceftriaxone-azithromycin for treatment of uncomplicated urogenital gonorrhoea in a Phase 3 randomized controlled trial. We evaluated the in vitro susceptibility to gepotidacin in clinical gonococcal isolates (n = 2912), including 125 (4.3%) ceftriaxone-resistant isolates, collected 2021-24 in eight WHO Enhanced Gonoco

Culture and Identification of Neisseria gonorrhoeae: A Narrative Review of Current Laboratory Methods.

BACKGROUND: Neisseria gonorrhoeae (GC) culture remains critical for gonorrhea treatment and surveillance, as antimicrobial susceptibility testing currently depends on culture-based methods. Here we aim to describe how GC culture and identification methods have changed since they were last described by the US Centers for Disease Control and Prevention. METHODS: We performed a systematic literature search to address the questions, "Have culture methods for GC changed since they were last described in 2009," "What biochemical tests can be used for the identification of GC," "What is the utility

Review of Methods for Detecting Antimicrobial Resistance of Neisseria gonorrhoeae.

BACKGROUND: Neisseria gonorrhoeae (GC) rapidly develops antimicrobial resistance, complicating treatment and surveillance. Although interest in resistance-guided therapy is increasing, most infections are treated empirically. Culture-based antimicrobial susceptibility testing (AST) remains essential but is limited by the fastidious nature of GC. Molecular AST shows promise but is constrained by incomplete validation of resistance determinants. METHODS: We conducted a systematic review of phenotypic and molecular AST methods for GC using predefined search strategies across Medline, Embase, Coc
