Knowledge, attitudes and behaviours in relation to doxycycline post-exposure prophylaxis use among attendees and healthcare workers at two sexual health services in Ireland.

OBJECTIVES: Doxycycline post-exposure prophylaxis (doxyPEP) has been shown to reduce syphilis and chlamydia incidence among gay, bisexual and other men who have sex with men (gbMSM) and transgender women (TGW). However, evidence among other populations remains limited. This study aimed to assess knowledge, attitudes, willingness and use of doxyPEP among sexual health clinic attendees and healthcare workers in Dublin. METHODS: Data were collected via two anonymous surveys in two sexual health services for service users and healthcare workers. Surveys were collected between January and March 20

Long-Term Serologic Outcomes Following Treatment of Syphilis in People With HIV: A Prospective Cohort Study.

BACKGROUND: This study aimed to evaluate the factors associated with rate and time to serological response, serofast status, and seroreversion in people with HIV (PWH) with a first diagnosis of syphilis. METHODS: The primary endpoint was serological response as determined by a 4-fold decline in rapid plasma regain (RPR) titer or seroreversion at 12 months. A secondary endpoint was cumulative serological response by 24 months of follow-up. Survival and multivariate analysis were used to compare different associated factors. RESULTS: A total of 184 PWH had a first syphilis episode in 642 perso

Bell's palsy as the first manifestation of neurosyphilis in an immunocompetent and HIV-negative patient: a challenging diagnosis.

Acute isolated peripheral facial nerve palsy is mainly idiopathic. In a minority of cases, however, it may result from other disorders that complicate the diagnostic process. Atypical clinical features, temporal evolution, and associated signs or symptoms are crucial in guiding diagnostic suspicion. In this case, we discuss a 57-year-old woman with acute unilateral facial weakness. The clinical course, together with the combination of imaging and laboratory studies, ultimately led to the final diagnosis of neurosiphilis. In the antibiotic era, the number of patients with the classic form of ne

Burden and clinical impact of 'neglected' transfusion-transmitted infections in Cameroon: A systematic review and meta-analysis.

BACKGROUND: Blood supply is a public health challenge in African countries. In Cameroon, blood selection guidelines focus on four viral and bacterial pathogens (HIV, hepatitis B and C viruses, Treponema pallidum) associated with transfusion-transmitted infections (TTIs). Other pathogens, often endemic (e.g., Plasmodium spp.), are not routinely screened in blood banks and are not included in blood safety guidelines or surveillance, despite their potential for transfusion transmission. MATERIALS AND METHODS: Here, we conducted a systematic review and meta-analysis of prevalence, determinants, a

Incidence and risk factors of C. trachomatis, N. gonorrhoeae and syphilis among a cohort of urban Canadian gay, bisexual and other men who have sex with men, 2017-2023: informing the potential impact of doxycycline prophylaxis.

OBJECTIVES: Doxycycline as post-exposure prophylaxis (doxy-PEP) has emerged as an efficacious strategy to reduce Chlamydia trachomatis (C. trachomatis), Neisseria gonorrhoeae (N. gonorrhoeae) and syphilis (sexually transmitted infections (STIs)) among gay, bisexual and other men who have sex with men (GBM). There is a need to identify prescribing criteria that maximise the number of STIs averted while minimising excessive use. DESIGN: In this prospective longitudinal cohort study with repeated measures and biobehavioural data collection, participants completed a questionnaire and tested for S

A Staff-Directed Electronic Medical Record Alert to Increase Chlamydia Screening: A Randomized Clinical Trial.

IMPORTANCE: Chlamydia is the most commonly reported sexually transmitted disease (STD) in the US. Screening sexually active females 24 years or younger is recommended to prevent pelvic inflammatory disease and its sequelae; however, screening remains suboptimal. OBJECTIVE: To assess whether an automated electronic medical record alert directed at medical assistants who are preparing patients for office visits increases chlamydia screening. DESIGN, SETTING, AND PARTICIPANTS: The STD Testing in Outpatient Practices (STOP STDs) Study was a cluster randomized clinical trial conducted from August

Modelling assessment of epidemiological strategies based on self-screening tools.

BACKGROUND: To reach 2030 targets, the World Health Organization specifically recommends test-and-treat strategies to eliminate HIV and bacterial STI. Mathematical models identified the required coverages for the success of these interventions. However, they appear out of reach for current testing protocols based on medical recommendations. Still, promoting voluntary testing can effectively increase testing rates if individuals perceive high benefits associated to frequent testing. Design of multi-disease voluntary testing kits remains a key strategic tool to reach various epidemiological goal

Global Survey on the Management of Multidrug-resistant Gram-negative Bacilli: Divergence of Practice Between High-Income Countries and the Rest of the World.

BACKGROUND: Multidrug-resistant gram-negative bacterial (MDR GNB) infections are associated with high mortality. The evidence base for the management of these infections is weak. Major international bodies have recently offered guidance on managing these infections. Their impact on real-life practice is unknown. METHODS: We conducted an online survey utilizing a 123-item questionnaire among healthcare professionals managing patients with serious MDR GNB infections via national societies and professional networks. RESULTS: A total of 1320 respondents from 76 countries completed the survey. Th

Impact of reduced-frequency monitoring among users of HIV pre-exposure prophylaxis on sexually transmitted infections and associated care: secondary outcomes of a randomized controlled trial.

BACKGROUND: Oral HIV pre-exposure prophylaxis (PrEP) users are generally screened for sexually transmitted infections (STIs) every 3 months. The need for such frequent screening is debatable. We compared STI diagnoses and associated care between PrEP users who underwent 6-monthly versus 3-monthly PrEP monitoring. METHODS: We conducted a secondary analysis of the EZI-PrEP study- a 2 × 2 factorial randomized controlled trial examining 6-monthly versus 3-monthly and online versus in-clinic PrEP monitoring among men who have sex with men (MSM) and transgender or gender-diverse persons in the Neth

HIV and Syphilis Testing Among Patients Tested for Gonorrhea and Chlamydia in Emergency Departments.

INTRODUCTION: Sexually transmitted infections (STIs), including HIV and syphilis, are increasing. In 2023, there were over 2.4 million reported cases of chlamydia, gonorrhea, and syphilis in the United States, a 32.5% increase from 2014. Emergency departments (EDs) are vital touchpoints for STI testing, yet HIV and syphilis testing among patients undergoing Neisseria gonorrhoeae (NG) and Chlamydia trachomatis (CT) testing is suboptimal. We aimed to determine testing frequency and to identify factors associated with HIV and syphilis co-testing among ED patients undergoing NG/CT testing. METHOD

Cervicovaginal cytokine profiles and their associations with colposcopic findings in adolescent girls and adult women in South Africa: an exploratory cross-sectional study.

BACKGROUND: Colposcopy is used to evaluate cervical inflammation and detect precancerous and other cervical lesions; however, access remains limited in many low-resource settings. Identification of cervicovaginal immune biomarkers associated with specific colposcopic findings may provide insight into mucosal immune alterations linked to increased susceptibility to sexually transmitted infections (STIs) and human immunodeficiency virus (HIV), although these associations do not imply diagnostic or predictive utility in a cross-sectional context. METHODS: In this exploratory cross-sectional stud

Genital tract infections, antiretroviral treatment and HIV-1 viral load in early postpartum women: : implications for sexual and breastfeeding HIV transmission.

BACKGROUND: Maintaining viral suppression in the postpartum period is critical for preventing HIV transmission to sexual partners and breastfed infants. Despite systemic viral suppression, genital HIV shedding may still occur in some women. We examined the association between bacterial vaginosis (BV) and sexually transmitted infections (STIs) and genital HIV shedding in postpartum women. METHODS: In this cross-sectional postpartum study, we hypothesised that BV and other genital tract infections would modulate HIV viral load (VL) in the genital tract. Postpartum women with HIV, aged < 25 years
