PURPOSE: The Iberian Registry of Ocular Syphilis aims to describe the epidemiology, clinical manifestations and treatment outcomes of ocular syphilis in Spain and Portugal. This re-emerging condition, associated with rising global syphilis rates and HIV coinfection, presents with diverse ocular and systemic features. The study seeks to fill knowledge gaps regarding incidence, presentation and therapeutic response.
METHODS: This multicentre, observational cohort study includes patients aged ≥18 years with newly diagnosed ocular syphilis confirmed by treponemal and nontreponemal serological tes

BACKGROUND: The global incidence of syphilis has been rising, and atypical clinical presentations are increasingly recognized. Human immunodeficiency virus (HIV)-induced immune changes can alter syphilis progression and complicate diagnosis. Otosyphilis is a rare but potentially reversible cause of sensorineural hearing loss (SNHL). This study investigates otologic involvement in patients co-infected with syphilis and HIV.
METHODS: Medical records of patients diagnosed with both syphilis and HIV infection at Etlik City Hospital between January 2023 and June 2025 were retrospectively reviewed.

INTRODUCTION: Syphilis is a leading cause of adverse pregnancy outcomes, excessively affecting sub-Saharan Africa. Early diagnosis and treatment are crucial to prevent mother-to-child transmission. The aims of the present study were to assess prevalence and epidemiological and clinical correlates of active syphilis diagnosed using serological [treponemal and non-treponemal (lipoidal)] assays and the research-use-only (RUO) Aptima Treponema pallidum assay [Hologic; transcription-mediated amplification (TMA) assay] among pregnant women attending four antenatal care facilities in Nchelenge, Zambi

BACKGROUND: Syphilis cases in the United States have surged over the past decade, and cases are highest among gay, bisexual, and other men who have sex with men (GBMSM). The Centers for Disease Control and Prevention recommends doxycycline postexposure prophylaxis (doxy PEP) for GBMSM recently diagnosed with bacterial sexually transmitted infections to mitigate this burden. Potential drawbacks include increased antimicrobial resistance and microbiome disruption. Here, we estimate doxy PEP coverage levels required to eliminate sustained community transmission among GBMSM while balancing the dra

Hospitalization provides an opportunity to improve the Hepatitis C Virus (HCV) cascade of care among people who inject drugs. Little is known about whether this potential opportunity is utilized. We conducted a retrospective chart review at four U.S. academic medical centers among patients hospitalized between 1/1/2018-3/31/2022 with ICD-10 diagnosis codes for both opioid use disorder and acute bacterial or fungal infection. Electronic medical records were reviewed manually to confirm injection drug use-related infection for inclusion in the study. Data abstracted from medical records included

BACKGROUND: Preexposure prophylaxis (PrEP) prevents HIV, although some have hypothesized it may lead to increases in behaviors that put men who have sex with men at risk for other sexually transmitted infections (STIs). However, data on the association between PrEP use and STI incidence are conflicting and prone to biases.
SETTING: New York City, 2014-2023.
METHODS: We used serial cross-sectional data from the New York City National HIV Behavioral Surveillance study among men who have sex with men. Log-linked Poisson regression with robust standard errors was used to determine linear trends

BACKGROUND: The vaginal microbiome plays a vital role in maintaining reproductive and urogenital health in females. In healthy women, it is dominated by Lactobacillus species, including L. crispatus, L. gasseri, L. iners, and L. jensenii. These species provide strong protection against sexually transmitted infections (STIs) and other gynecological diseases. They produce lactic acid, which helps maintain a low vaginal pH, supporting epithelial integrity, modulating the immune system, and preventing colonization by bacterial pathogens. Dysbiosis, or an imbalance in microbial communities, is link

HIV-1 and Chlamydia trachomatis (CT) are two significant sexually transmitted pathogens that frequently co-infect individuals. However, the mechanisms by which these two obligate intracellular pathogens interact at the cellular level remain elusive, particularly in macrophages, where persistent infections can occur. In this study, we demonstrate that CT generates inclusions in macrophages of murine and human origins. We also show that both HIV-1 and CT reciprocally reduce the growth and replication of each other within co-infected human macrophages, irrespective of whether the viral or bacteri

Lipoarabinomannan (LAM) is a useful biomarker for detection of M. tuberculosis infection and disease. Related antigens can be detected in urine samples of TB patients by combinations of monoclonal antibodies (mAbs) directed against specific epitopes expressed in LAM. While sensitive for samples from patients with active TB disease who have HIV-1 co-infections, these assays are less effective for other populations, and there is therefore a need for more sensitive antibodies that can improve the sensitivity of these assays. Here we characterize the antigen and epitope specificities, sequence div

UNLABELLED: No regulatory agency-approved nucleic acid amplification tests (NAATs) exist for detecting Chlamydia trachomatis (CT) or Neisseria gonorrhoeae (NG) in conjunctival samples, necessitating validation, including performance analysis. This is challenging due to case rarity and the absence of a gold standard. This retrospective case series evaluated the performance of the Roche cobas CT/NG NAAT on conjunctival samples using microbiological and clinical data. Clinical records of patients with positive CT/NG NAAT conjunctival samples processed in our laboratory between 2013 and 2023 were

UNLABELLED: Haemophilus parainfluenzae is increasingly recognized as a reservoir of antimicrobial resistance in urogenital infections. We conducted a retrospective laboratory-based study including all H. parainfluenzae isolates collected at a tertiary hospital in Barcelona between 2018 and 2022. Antimicrobial susceptibility testing was performed for all isolates, and whole-genome sequencing was conducted for multidrug-resistant (MDR) and capsulated strains. A total of 529 isolates from 504 individuals were analyzed (258 respiratory, 223 urogenital, and 48 other sources). Overall, 85/529 (16.1%

INTRODUCTION: Although HIV pre-exposure prophylaxis (PrEP) has been rapidly scaled up in Argentina to reach key populations with high HIV burden, evidence on real-world persistence is lacking. Understanding who remains engaged in care, and why others discontinue, is essential to ensure the effectiveness of PrEP. This study evaluated oral PrEP persistence and identified demographic and behavioural factors associated with discontinuation in a cohort of individuals initiating PrEP in Buenos Aires.
METHODS: We performed a retrospective cohort study including all users initiating daily or event-dr

BACKGROUND: HIV testing is an integral component of prevention, reducing undiagnosed infection and promoting early treatment. Indigenous gay, bisexual and other men who have sex with men (GBM) are at increased risk of HIV and sexually transmitted infections (STIs), but may experience difficulties accessing clinical services. At-home blood-based self-collection may offer an alternative that bypasses such barriers. This study aimed to explore uptake of dried blood spots (DBS) among Indigenous Māori GBM in Aotearoa New Zealand.
METHODS: The Sex and Prevention of Transmission Study is a national

BACKGROUND: Group sex events (GSEs) have potential implications for transmission and acquisition of HIV and sexually transmitted infections (STI).
METHODS: We analyzed survey data from Project DETECT, a Centers for Disease Control and Prevention-funded study evaluating point-of-care tests. Our analytical sample included cisgender gay, bisexual, and other men who have sex with men (cisGBMSM) seeking HIV/STI testing who reported at least one anal sex partner in the past three months. We defined recent GSE as a sexual encounter in the past three months involving more than two people. Poisson reg

BACKGROUND: Quarterly sexually transmitted infection (STI) screening is recommended for gay, bisexual, and other men who have sex with men (gbMSM) using HIV pre-exposure prophylaxis (PrEP). However, frequent in-person testing can pose engagement barriers and increase costs, prompting interest in risk-based screening strategies to individualize testing frequency.
METHODS: We evaluated the extent to which two common sexual risk assessment tools, the I Want the Kit (IWTK) survey and the HIV Incidence Risk Index for MSM (HIRI-MSM), predict bacterial STI acquisition among gbMSM using PrEP. Partici

BACKGROUND: Trimeric envelope-proximal domains in viral class I fusion proteins are conserved targets of broadly neutralizing antibodies (bNAbs), but it has proven difficult to develop vaccines against those targets. The HIV-1 gp41 membrane-proximal external region (MPER) is one such target. Induction of a neutralizing response likely depends on the immunogen having a close-to-native structure.
METHODS: Native sequence MPER was displayed on genome-reduced bacteria as a coiled-coil homotrimer using a Haemophilus influenzae Hia trimeric autotransporter. Vaccine designs incorporated additional f

BACKGROUND: As syphilis cases increase across the U.S., the emergency department (ED) has emerged as a critical venue for screening. However, little is known about the current ED screening landscape. This study aims to evaluate the national landscape of ED syphilis screening, identify barriers to implementation, and characterize existing infrastructure that may support expansion of syphilis screening in the ED.
METHODS: Electronic surveys were distributed via email to 281 institutions with emergency medicine residency programs. Survey responses were also solicited from institutions both with

BACKGROUND: This study evaluated health system readiness in Malawi for elimination of mother-to-child transmission (EMTCT) of syphilis against World Health Organization (WHO) validation criteria by identifying key health system bottlenecks. It also developed a replicable toolkit to assess bottlenecks to syphilis EMTCT in other countries.
METHODS: A mixed-methods design integrated quantitative and qualitative data. A National Clinic Readiness Survey assessed coverage indicators for prevention of maternal and congenital syphilis at 58 health centers. Exit surveys included 9513 mothers at antena

BACKGROUND: Syphilis diagnosis in resource-limited settings relies on syndromic management and treponemal-based antibody screening tests that cannot distinguish active infection from past-treated infection, both of which can lead to overtreatment. Point-of-care (POC) tests that detect active infection could reduce unnecessary treatment.
METHODS: We developed an agent-based model of co-transmitting syphilis and human immunodeficiency virus, calibrated to Zimbabwe using population-based survey data (ZIMPHIA) and UNAIDS estimates. We modeled five scenarios involving two hypothetical new POC diag
