European spine journal : official publication of the European Spine Society, the European Spinal Deformity Society, and the European Section of the Cervical Spine Research Society2026eng
PURPOSE: Population aging has increased both the prevalence and clinical impact of adult spinal deformity (ASD). Sagittal and regional parameters (e.g., SVA, PI-LL, LL-TK) guide assessment and treatment planning, but it remains unclear which indices are most closely associated with sagittal imbalance and functional disability. This study therefore evaluated the associations between regional and global spinopelvic parameters and clinical status in an adult outpatient cohort with chronic low back pain, and compared the ability of the LL-TK and PI-LL indices to discriminate concurrent sagittal im

PURPOSE: To evaluate the association of parity and menopausal timing with spinopelvic alignment in postmenopausal women and whether pelvic incidence-lumbar lordosis (PI-LL) difference statistically mediates the association between parity and low back pain.
METHODS: This single-centre cross-sectional study included 239 postmenopausal women aged ≥ 45 years. Standardised standing lumbosacral radiographs were used to measure pelvic incidence, sacral slope, pelvic tilt, lumbar lordosis, and PI-LL difference. PI-LL mismatch was defined as ≥ 10°. Pain severity was assessed using a visual analogue sc

Study DesignRetrospective cohort study.ObjectivesTo determine whether thoracic compensation (TC) assessed against the deformity burden is associated with proximal junctional kyphosis (PJK) after lower thoracic fusion for adult spinal deformity (ASD).MethodsWe reviewed 343 patients fused from T9-L1 to the sacrum or pelvis with a pelvic incidence (PI) minus lumbar lordosis (LL) mismatch above 20°. Expected thoracic kyphosis (TK) was derived from the PI-LL mismatch using published equations, and a measured TK below it defined TC. PJK was defined as a proximal junctional angle of 20° or more, or r

BACKGROUND Osteoporotic vertebral compression fractures (OVCFs) are correlated with thoracolumbar sagittal imbalance associated with paravertebral muscles (PVM) atrophy and fatty infiltration (FI). This study investigated PVM degeneration and its correlation with spinopelvic parameters in patients with OVCFs. MATERIAL AND METHODS This retrospective case-control study enrolled 50 patients with OVCFs and 50 controls with lumbar degeneration, frequency-matched for age (±5 years). Complete sex matching was not achieved. Primary comparisons were adjusted for age, sex, BMI, and lumbar bone mineral d

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs.
OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain.
DESIGN: Retrospective, cross-sectional observational study.
SETTING: Rheumatology office practice.
PARTICIPANTS: A consecutive series of 245 patients

➢ Standing full-spine radiographs are mandatory for cervical deformity evaluation; key alignment targets include cervical sagittal vertical axis (cSVA) of ≤40 mm, T1 slope minus cervical lordosis (T1S - CL) of <15°, and a chin-brow vertical angle (CBVA) between -10° and +10° relative to the true vertical. The achievement of these thresholds is independently associated with better patient-reported outcomes, whereas values outside these ranges are associated with worse disability and quality-of-life scores.➢ The Kim-International Spine Study Group (ISSG) morphology-based classification, which id

➢ Cervical sagittal alignment is associated with patient-reported outcomes, with higher T1 slope-cervical lordosis mismatch and greater C2-C7 sagittal vertical axis correlating with greater disability and worse health-related quality of life.➢ Cervical alignment must be interpreted alongside global sagittal balance, as cervical posture interacts with thoracic and spinopelvic alignment and compensatory mechanisms.➢ Both anterior and posterior cervical fusion can influence sagittal alignment, but optimal outcomes depend on alignment-conscious planning tailored to deformity morphology and spinal

INTRODUCTION: Previous studies have emphasized the importance of thoracic kyphosis (TK) restoration and sagittal alignment for long-term outcomes following posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). Although various equations have been introduced to predict the ideal regional alignment of the thoracic or lumbar spine, no study has validated these equations or investigated the influence of achieving these patient-specific ideal regional parameters on patient-reported health-related quality of life (HRQoL) outcomes following PSF for AIS.
METHODS: A retrospective co

INTRODUCTION: Chronic low back pain (CLBP) is a common cause of pain, disability, and impaired quality of life. Persistent pain often leads to compensatory postural changes, including pelvic asymmetry, coronal imbalance, and vertebral rotation. Although fluoroscopy-guided transforaminal epidural steroid injection (TFESI) is an established treatment for patients who fail conservative therapy, its effects on spinal posture have not been well studied. This study aimed to evaluate changes in spinal posture following TFESI using the radiation-free DIERS formetric 4D spine posture analysis system, a

OBJECTIVE: This study aimed to investigate whether spinal flexibility, assessed by the alignment difference between Full-length Spine CT scout view (FLS-CT) and standing radiographs, was associated with the eventual use of three-column osteotomy (3CO) in patients with old thoracolumbar fracture with kyphotic deformity (OTLFK).
METHODS: This single-center, dual-surgeon retrospective study included patients with OTLFK who underwent corrective surgery between August 2021 and August 2024. Patients were categorized into the 3CO group and the non-three-column osteotomy (N3CO) group according to whe

PURPOSE: Restoration of sagittal balance, specifically the mismatch between pelvic incidence and lumbar lordosis (PI-LL), is an important objective in lumbar fusion, particularly for degenerative deformity correction. While a postoperative PI-LL of ≤ 10° is generally considered optimal, the clinical necessity of achieving this threshold versus partial correction (PI-LL > 10°) remains uncertain. This study aimed to systematically review the differences in disability outcomes and mechanical complications between "full" (≤ 10°) versus "partial" (> 10°) PI-LL correction.

BACKGROUND: Questions about the surgical corridor through the psoas muscle and the retroperitoneal space have persisted since the introduction of lateral lumbar interbody fusion (LLIF) in the lateral decubitus position. The prone transpsoas (PTP) approach maintains or restores lordosis and allows single-position access to the anterior and posterior columns, but the position of visceral structures during an actual PTP case has not been characterized.
METHODS: Retrospective single-arm observational cohort of 51 consecutive adults undergoing LLIF-PTP by 4 fellowship-trained surgeons at 3 sites.

BACKGROUND: High-performance swimming requires specific physical attributes to optimize propulsion and minimize drag. While start, turn, and underwater phases are critical, comprehensive comparisons of physical characteristics between elite and sub-elite swimmers remain limited. This study aimed to investigate whether differences exist in physical characteristics related to performance between high-performance competitive swimmers (Olympic trial qualifiers) and average competitive swimmers.
METHODS: Thirty-three male competitive swimmers were enrolled in this prospective observational study.

BACKGROUND: Impaired spinopelvic function contributes to instability after total hip arthroplasty (THA). It remains unclear whether preoperative spinopelvic function affects patient-reported-outcomes (PROs) in patients undergoing THA. We hypothesized that preoperative pathological spinopelvic function is associated with worse postoperative PROs at 2-years-follow-up (2Y-FU) after THA.
METHODS: A follow-up study of a prospective cohort undergoing primary THA was performed. PROs at 2Y-FU included Hip-Disability-and-Osteoarthritis-Outcome-Score (HOOS), Harris-Hip-Score (HHS), and Subjective-Hip-V

PURPOSE: Preoperative planning for lumbar fusion involves careful consideration of segmental and regional spinopelvic alignment. However, whether anterior lumbar interbody fusion (ALIF) reliably delivers planned lordosis is incompletely understood. The purpose of this study was to evaluate the ability of L5-S1 ALIF to restore regional L4-S1 and segmental L5-S1 lordosis according to preoperative plan.
METHODS: Adult patients who underwent L5-S1 ALIF with posterior fusion by a single surgeon, had a documented preoperative plan, and received UNiD patient-specific rods were included. Demographics

STUDY DESIGN: Retrospective cohort study.
OBJECTIVE: To describe differences in sagittal alignment and compensatory mechanisms in patients with and without sarcopenia.
SUMMARY OF BACKGROUND DATA: Sarcopenia refers to the degenerative loss of muscle quantity and quality and impacts the sagittal balance of the spine. Skeletal muscle is essential to maintaining upright posture, and its progressive loss may compromise the compensatory mechanisms that preserve sagittal alignment. Sarcopenia can be diagnosed using thigh muscle thickness measured on a low-dose full-body radiograph.
METHODS: Full-b
