Lower limb compensation in adult spinal deformity: can we identify different patterns?

OBJECTIVE: Lower limbs can play a major compensating role for sagittal malalignment; however, little is known about the different types of compensation. This study aimed to identify different patterns of lower limb compensation and to determine which parameters may affect the recruitment of knee flexion versus hip extension. METHODS: This study included adult spinal deformity (ASD) patients with full-body X-rays in erect position from a multicentric prospective database. All parameters were measured at baseline: demographics, clinical scores and radiographic parameters: pelvic parameters, pel

Relationship between pelvic incidence and hip, pelvic, and lumbar motion during squatting in healthy individuals with high and low pelvic incidence.

Pelvic incidence (PI)—the sacral angle relative to the pelvis—is a posture-independent spinopelvic parameter influencing pelvic and lumbar alignment. Because PI affects sagittal spinal configuration, it may reflect susceptibility to posterior pelvic tilt and reduced femoral head coverage. However, its influence on pelvic, lumbar, and hip motion during dynamic tasks is unclear. We examined whether individuals with high and low PI have differing time-series hip, pelvic, and lumbar angles during squatting. Twenty-six healthy male individuals were classified into high (≥ 53.8°) and low (≤ 43.1°) P

Sagittal Alignment Reciprocal Changes After Thoracolumbar/Lumbar Anterior Vertebral Body Tethering.

Background/Objective: The anterior vertebral body tethering (AVBT) technique, which preserves spinal mobility and avoids possible fusion problems in adolescent idiopathic scoliosis (AIS) patients, continues to be increasingly used in spine surgery. The study aims to report the early-to-early-mid postoperative radiological results of thoracolumbar/lumbar AVBT on sagittal alignment, and the second aim is to compare AVBT with selective thoracic fusion (STF) and non-selective fusion (NSF) groups in AIS patients. Methods: Patients with a diagnosis of AIS were retrospectively evaluated in the study.

CT-Based Analysis of Rod Trace Length Changes During Posterior Spinal Correction in Adult Spinal Deformity.

Background: In adult spinal deformity (ASD) surgery, appropriate rod length determination is crucial, as excessive cranial rod length can lead to skin problems, especially in thin elderly patients if proximal junctional kyphosis (PJK) develops. In adolescent idiopathic scoliosis (AIS), correction is primarily performed in the coronal plane, and rod length changes are relatively predictable. Moreover, PJK is uncommon in AIS, making excess rod length rarely a clinical concern. In contrast, ASD correction involves more complex three-dimensional realignment, including restoration of lumbar lordosi

The Efficacy of Prone Single-Position Lateral Lumbar Interbody Fusion for Symptomatic Cranial Adjacent Segment Degeneration.

Background/Objectives: Following lumbar fusion procedures, adjacent segment degeneration (ASD) at cranial levels presents as a well-documented long-term complication, manifesting through recurrent pain, neurological deficits, and progressive functional decline. The prone single-position technique for lateral lumbar interbody fusion (PSP-LLIF) streamlines surgical workflow by eliminating the need for intraoperative patient repositioning; however, comprehensive evidence supporting its clinical and radiological effectiveness in managing cranial ASD remains insufficient. Material and Methods: This

In vivo prediction of intervertebral disc strains and segmental kinematics from clinical MRI during lumbar extension.

INTRODUCTION: Excessive intervertebral disc (IVD) strains and vertebral body motions are associated with lower back pain (LBP). Quantifying these strains and motions may aid in predicting the success of candidate LBP treatments and enable better prediction of pre-operative instability and post-operative implant failure, but cannot currently be obtained in routine clinical assessment. Thus, the aim of this study was to evaluate the feasibility of utilising clinical measures of spinal alignment, IVD geometry, and disc degeneration to predict in vivo IVD strains and vertebral translations. Types of Pelvic Fixation and Their Influence on Pelvic Incidence Change After Surgery for Adult Spinal Deformity: A Retrospective Analysis.

STUDY DESIGN: Retrospective analysis. OBJECTIVE: This study will compare post-operative pelvic incidence (PI) modification by types of pelvic-fixation. BACKGROUND: In adult spinal deformity (ASD), restoring sagittal spinal alignment which is usually based on PI is essential to improve the patient's quality of life. While PI was postulated to be a constant value, recent literature reported that the latter can change with pelvic fixation after ASD surgery. METHODS: This is a retrospective multicenter study of 423 patients who have undergone ASD surgery between 2012 and 2022. These patients we

Short- and long-acting GLP-1 receptor agonists decrease female sexual behaviors in experienced rodents.

Beyond the metabolic effects induced by long- and short-acting glucagon-like peptide-1 receptor (GLP-1R) agonists, they modulate reward-related behaviors by acting on brain regions including lateral septum (LS) and neural pathways involving the nucleus tractus solitarii (NTS). While the short-acting GLP-1R agonist exendin-4 (Ex-4) reduces sexual behaviors in sexually naïve male mice, the effects of long- versus short-acting GLP-1R agonists on natural rewards, such as sexual and social behaviors, in females remain unexplored. In the current study, we investigate the effects of both long-acting
