Is the lumbodorsal fascia necessary?

The role of the lumbodorsal fascia is generally neglected in spine biomechanics. Yet it is perhaps the most important structure insuring the integrity of the spinal machinery. The viscoelastic property of its collagen has a direct impact on the way the muscles are used and forces are channeled from the ground to the upper extremities. As a controller of the forces distribution between muscles and fascia, lordosis is the prime candidate for rehabilitation in the event of injury.

Lumbar lordosis in osteoporosis and in osteoarthritis.

The curvature of the lumbar spine and the risk of developing either osteoporosis (OP) or osteoarthritis (OA) are influenced by many common factors. The aim of this study is to determine whether lumbar lordosis is different between patients with either disease and healthy persons. A cross-sectional, blinded, controlled design was implemented. One hundred and twelve postmenopausal women were evaluated for bone mineral density as well as undergoing spinal radiography. Lordosis measurement was performed with Cobb's method. The sample was divided in four groups: patients with OP (n = 34, L1-L5 = 40

[The influence of thoracic kyphosis on sagittal balance of the lumbosacral spine in thoracic idiopathic scoliosis patients].

OBJECTIVES: To evaluate the influence of thoracic kyphosis to sagittal alignment and balance of the lumbosacral vertebrae in thoracic adolescent idiopathic scoliosis patients. METHODS: Standing posteroanterior and lateral x-rays of a cohort of 55 patients with thoracic adolescent idiopathic scoliosis were obtained. The patients were classified according to their thoracic kyphosis, the first group TK < 10 degrees and the second group 10 degrees < or = TK < or = 40 degrees . The following parameters were measured: lumbar lordosis (LL), upper and lower arc of lumbar lordosis, sagittal vertical a

Influence of trunk muscle co-contraction on spinal curvature during sitting reclining against the backrest of a chair.

Today, many office workers frequently adopt a relaxed or slumped sitting posture for many hours, and often people tend to spend their leisure time reclining against the backrest of a chair while sitting for a long time, as when watching television. While sitting, the pelvis rotates backwardly, and lumbar lordosis is flattened. Simultaneously, the load on the intervertebral discs and spine increases. Sitting in a slumped position is known to increase disc pressure even more, and to aggravate chronic low back pain (CLBP). Therefore, it is very important to teach workers and often people about th

Juvenile galactosialidosis with attacks of neuropathic pain and absence of sialyloligosacchariduria.

Galactosialidosis (MIM 256540) is an autosomal recessive lysosomal storage disease caused by a defect of the protective protein/cathepsin A. Increased amounts of urinary sialic acid-rich oligosaccharides are considered to be an essential diagnostic marker of the disease. We here report a patient with atypical clinical features who consistently has excreted normal amounts of sialyloligosaccharides in the urine. The boy started to have attacks of neuropathic pain associated with hyperesthesia around 1(1/2) years of age. From 4 years of age when his vision was first tested, the patient developed

Female sexual behavior, estrous cycle and gene expression in sexually dimorphic brain regions after pre- and postnatal exposure to endocrine active UV filters.

The developing female brain represents a potential target for estrogenic environmental chemicals because it depends on estrogen but is exposed to low endogenous estrogen levels, thus facilitating competition by exogenous estrogen receptor (ER) agonists. We investigated effects of two estrogenic UV filters, 4-methylbenzylidene camphor (4-MBC) and 3-benzylidene camphor (3-BC). 4-MBC has been detected in human milk, indicating potential exposure of fetus and infant. The two chemicals were administered in chow to rats of the parent generation before mating, during pregnancy and lactation, and to t

Sagittal alignment as a risk factor for adjacent level degeneration: a case-control study.

This study examined whether sagittal alignment, preexisting adjacent level degeneration, and smoking predispose patients to adjacent segment degeneration following lumbar fusion. Fifty-one patients with adjacent segment degeneration were identified and matched with control patients based on age, sex, level, and date of index surgery. Preexisting adjacent level degeneration and sagittal alignment through the fusion and from L1-S1 were determined before and after initial surgery. Patients with adjacent segment degeneration had significantly less lordosis through the fusion and lumbar spine follo

[The psycho-social impact of the locomotor deformations in school-aged child].

Depending on their severity, congenital or acquired, the deformations of the locomotor system affect more or less the child's mental status. Adolescence is the time when their impact is most important. When the physical, mental or functional suffering become a disability, professional and psycho-social integration problems occur. The role of the team treating such a patient is to orientate him toward a specific job corresponding to his abilities, while the social services must support him in achieving this goal. The psychotherapist is the most important person in his fight against depression a

Estradiol enhances sociosexual behavior and can have proliferative effects in ovariectomized rats.

Although estradiol (E(2)) may have some beneficial effects as a treatment for menopause symptoms, E(2) also has trophic effects that can increase vulnerability to some cancers, such as breast cancer. In the present study, a model to investigate the concomitant behavioral and proliferative effects of E(2) was developed. First, the effects of different duration of chronic E(2) exposure (2 vs 6 months), or no such exposure, on proliferation (tumor incidence and weight, uterine weight) in adult, ovariectomized (OVX) rats was determined. Second, the effects of different dosages of E(2) (0.03 or 0.0

Comparison of anterior cervical foraminotomy vs arthroplasty for unilateral cervical radiculopathy.

BACKGROUND: Preservation of segmental motion and avoidance of adjacent segment degeneration are common theoretical advantages of cervical arthroplasty and ACF. In patients with unilateral cervical radiculopathy, both procedures have shown good clinical results; but there are currently no proven comparative biomechanical results. This study was designed to compare the biomechanical effect of cervical arthroplasty and ACF and to propose optimal inclusion criteria for each surgery. METHODS: Among the patients with unilateral cervical radiculopathy, 15 patients who underwent arthroplasties using

Neurological symptoms and deficits in adults with scoliosis who present to a surgical clinic: incidence and association with the choice of operative versus nonoperative management.

OBJECT: Adults with scoliosis often present with neurological symptoms and deficits. However, the incidence of these findings and how they may affect treatment decisions have not been clearly defined. The purpose of this study was to quantify the prevalence of neurological symptoms and deficits in adults with scoliosis presenting to a surgical clinic, and to assess for an association between these factors and the decision to pursue operative treatment. METHODS: In this study, the authors document the Oswestry Disability Index (ODI), radiographic findings, and the incidences of back pain, neur

Change of cervical balance following single to multi-level interbody fusion with cage.

Postoperative limitation of cervical movement and adjacent disc degeneration are major causes of postoperative neck pain after anterior cervical discectomy and interbody fusion (ACDF). We present a retrospective study of dynamic radiographic change following ACDF with cages. We performed ACDF in 50 patients with cervical degenerative diseases, divided into three groups based on the level of interbody fusion (Group A: one-level; Group B: two-level; Group C: three-level). Preoperative and postoperative flexion-extension X-rays were acquired and cervical range of motion (ROM), lordosis, fusion ra
