BACKGROUND AND IMPORTANCE: Restoring distal lumbar lordosis at a fused L5-S1 junction is challenging in adult spinal deformity surgery, particularly in revision cases with limited anterior access. Posterior intradiscal osteotomy (IDO) with posterior approach anterior column release (P-ACR) provides a single-posterior approach for mobilizing the anterior column and restoring distal lumbar lordosis without the risks of retroperitoneal exposure. This report details the operative nuances and technical sequence for L5-S1 IDO with P-ACR and summarizes early outcomes from complex revision cases.

Cement discoplasty (CD) was initially described in 2015. This novel treatment involves injecting polymethylmethacrylate (PMMA) into the degenerate disc cavity. This is particularly applicable to elderly patients with disc degeneration and collapse, where restoration of disc height improves lordosis and sagittal balance, treating symptoms of degenerative scoliosis, foraminal stenosis, adjacent segment degeneration, or flatback syndrome, who would otherwise have significant risks for major spine surgery. In all cases, symptoms are associated with intradiscal vacuum phenomenon (IDVP), a radiologi

STUDY DESIGN: Retrospective study using prospectively collected data.
OBJECTIVE: To investigate the impact of the lordosis distribution index (LDI) on proximal junctional kyphosis (PJK) and clinical outcomes in patients with degenerative flatback syndrome (DFS).
SUMMARY OF BACKGROUND DATA: Proper lumbar lordosis (LL) correction is essential for successful outcomes in corrective surgery for DFS. However, the clinical significance of LDI remains debatable, likely due to the lack of a clear definition regarding normal LDI.
METHODS: Patients with DFS who underwent fusion from the low thoracic s

PURPOSE: Understanding the mechanism and extent of preoperative deformity in revision procedures may provide data to prevent future failures in lumbar spinal fusion patients.
METHODS: ASD patients without prior spine surgery (PRIMARY) and with prior short (SHORT) and long (LONG) fusions were included. SHORT patients were stratified into modes of failure: implant, junctional, malalignment, and neurologic. Baseline demographics, spinopelvic alignment, offset from alignment targets, and patient-reported outcome measures (PROMs) were compared across PRIMARY and SHORT cohorts. Segmental lordosis a

The authors outline a review of preoperative, intraoperative, and postoperative considerations surrounding adult spinal deformity. Preoperative management topics include imaging, hemoglobin A1c levels before spine surgery, osteoporotic management, and prehabilitation. Topics surrounding intraoperative management include the use of antibiotics, liposomal bupivacaine, and Foley catheters. The authors also discuss postoperative questions surrounding analgesia, nausea and vomiting, thromboembolic prophylaxis, and early mobilization. Throughout their discussion, the authors incorporate enhanced rec

BACKGROUND: The anterior-to-psoas (ATP) approach to the lumbar spine has been proposed as an alternative to the transpsoas approach for approaching the disc space without dissecting through the psoas muscle, thus decreasing the risk of injury to the lumbar plexus. There are no prior studies that evaluates the clinical application of anterior longitudinal ligament (ALL) release and anterior column realignment (ACR) using the ATP approach. The objective of this study was to describe and evaluate the safety of ACR using an ATP approach with release of both the ALL and bilateral annulus for correc

OBJECTIVE: This study investigated the differences in trunk sway during stair climbing between people with normal spinal alignment and people with flat-back syndrome.
METHODS: Twelve male volunteers with flat-back syndrome (global angle < 20 degrees) and 12 male volunteers with normal spinal alignment (global angle between 20 degrees and 30 degrees) were enrolled. An accelerator was attached to the third lumbar spine and the sway of each participant's trunk was measured during stair climbing.
RESULT: Participants with flat-back syndrome showed significant differences in vector, anteroposteri

Sagittal balance is widely recognized as the primary determinant of optimal outcomes in adult spinal deformity. In adolescent idiopathic scoliosis (AIS), coronal correction risks being obtained at the expense of sagittal malalignment after posterior spinal fusion. Apical lordosis, often underestimated on two-dimensional imaging, is the primary deforming factor in AIS. Failure to restore thoracic kyphosis and lumbar lordosis during posterior spinal fusion contributes to problematic early surgical complications, including proximal or distal junctional kyphosis and failure. Although adolescent pa

BACKGROUND: Hospital-acquired venous thromboembolisms (HA-VTE) are a significant source of morbidity and mortality in spine surgery patients. The purpose of this study was to review HA-VTE rates at our institution and evaluate the prevalence of known risk factors in patients who developed HA-VTE among both neurosurgical and orthopedic spine surgeries.
METHODS: Retrospective chart reviews were conducted of all spine surgery patients from January 1, 2013, to July 31, 2017, to evaluate rates of HA-VTE and prevalence of known HA-VTE risk factors among these patients. Univariate and multivariate l

Spinal fluid overdrainage with cerebellar hemorrhage is a rare complication of spinal surgery that can have severe consequences if not detected quickly. We present the case of a 72-year-old Caucasian female who underwent thoracolumbar fixation for flatback syndrome. Intraoperatively, the patient suffered a dural injury that was repaired. In the immediate postoperative period, the patient's neurological status rapidly deteriorated within an hour and Jackson-Pratt (JP) drain output measured 300 ml of serosanguinous fluid. A stat CT scan revealed cerebellar hemorrhage, pneumocephalus, and tonsill

We present a case report of a patient who had delayed small bowel obstruction secondary to an incarcerated loop of small bowel within an acute lumbar spine fracture. The patient was involved in a rollover motor vehicle accident, resulting in lumbar spine fractures at L2-4. A comminuted fracture of the L3 vertebral body with likely disruption of the anterior longitudinal ligament (ALL) was noted. The patient underwent L1-4 posterior spinal fusion with the introduction of mild lumbar lordosis to prevent future complications of flatback syndrome. On postoperative Day 4, the patient was noted to h

Modern human bipedality is unique and requires lumbar lordosis, whereas chimpanzees, our closest relatives, have short lumbar spines rendering them incapable of lordosis. To facilitate lordosis, humans have longer lumbar spines, greater lumbosacral angle, dorsally wedged lumbar vertebral bodies, and lumbar zygapophyseal joints with both increasingly coronal orientation and further caudal interfacet distances. These features limit modern lower lumbar spine and lumbosacral joint ailments, albeit imperfectly. The more coronal zygapophyseal orientation limits spondylolisthesis, while increasing in

BACKGROUND: Spinopelvic fixation is an integral part of achieving solid fusion across the lumbosacral junction, especially in deformity procedures requiring substantial correction or long-segment constructs. Traditional S2-alar-iliac (S2AI) screw-placement techniques utilize fluoroscopy, increasing operative time and radiation exposure to the patient and surgeon. We describe a novel free-hand technique for S2AI screw placement in patients with adult spinal deformity.
METHODS: We reviewed the records of 45 consecutive patients who underwent spinopelvic fixation performed with use of S2AI screw

A 75-year-old Japanese woman with Parkinson's disease complained of lower back pain and gait disturbance because of iatrogenic flatback syndrome. The preoperative global spinal parameters were as follows: C7SVA, 168 mm; TK, 52°; LL, -0.8°; PI, 57°; PT, 55°; TPA, 60°. We performed 3D gait analysis using a VICON System and calculated the dynamic SVA. Preoperatively, her flexion deformity gradually progressed during walking. The dynamic parameters gradually increased as follows: thoracic SVA, 216-241 mm; lumbar SVA, 53-69 mm; spinal SVA, 270-311 mm. We performed two-stage corrective surgery. Her

The transforaminal lumbar interbody fusion (TLIF) is a well-established 3-column fusion technique that can be used to manage lumbar stenosis, instability, and deformity. Having been in use for more than 20 years, it has evolved into many different renditions. This includes protocols using minimally invasive surgery (MIS) approaches. To avoid the development of flatback syndrome, it is important that a TLIF procedural technique is capable of reproducibly restoring lordosis. This article describes one of many MIS TLIF protocols and presents some of its previously published outcomes.

Flatback syndrome can be a significant source of disability, affecting stance and gait, and resulting in significant pain. Although the historical instrumentation options for thoracolumbar fusion procedures have been commonly regarded as the etiology of iatrogenic flatback, inappropriate selection, or application of modern instrumentation can similarly produce flatback deformities. Patients initially compensate with increased lordosis at adjacent lumbar segments and reduction of thoracic kyphosis. As paraspinal musculature fatigues and discs degenerate, maintaining sagittal balance requires in
