Reduced Lumbar Lordosis in Fibromyalgia: Radiographic Evidence of Lumbar Straightening by L1-L5 Cobb Angle.

BACKGROUND: Reduced lumbar lordosis has been observed in patients with low back pain, but lumbar straightening has not been well characterized in fibromyalgia. We hypothesized that fibromyalgia would be associated with reduced lumbar curvature on standing radiographs. OBJECTIVE: To evaluate lumbar lordosis, quantified by the L1-L5 Cobb angle, in patients with fibromyalgia compared with patients with other rheumatic diseases and low back pain. DESIGN: Retrospective, cross-sectional observational study. SETTING: Rheumatology office practice. PARTICIPANTS: A consecutive series of 245 patients

Achieving "ideal" sagittal alignment does not influence HRQoL outcomes 5 years following PSF for lenke 1 AIS.

INTRODUCTION: Previous studies have emphasized the importance of thoracic kyphosis (TK) restoration and sagittal alignment for long-term outcomes following posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). Although various equations have been introduced to predict the ideal regional alignment of the thoracic or lumbar spine, no study has validated these equations or investigated the influence of achieving these patient-specific ideal regional parameters on patient-reported health-related quality of life (HRQoL) outcomes following PSF for AIS. METHODS: A retrospective co

Successful execution of preoperative lumbar alignment planning: reliability of the L5-S1 anterior lumbar interbody fusion (ALIF).

PURPOSE: Preoperative planning for lumbar fusion involves careful consideration of segmental and regional spinopelvic alignment. However, whether anterior lumbar interbody fusion (ALIF) reliably delivers planned lordosis is incompletely understood. The purpose of this study was to evaluate the ability of L5-S1 ALIF to restore regional L4-S1 and segmental L5-S1 lordosis according to preoperative plan. METHODS: Adult patients who underwent L5-S1 ALIF with posterior fusion by a single surgeon, had a documented preoperative plan, and received UNiD patient-specific rods were included. Demographics

Radiographic Muscle Sarcopenia Affects Regional Sagittal Alignment: A Single-Center Radiographic Retrospective Analysis.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To describe differences in sagittal alignment and compensatory mechanisms in patients with and without sarcopenia. SUMMARY OF BACKGROUND DATA: Sarcopenia refers to the degenerative loss of muscle quantity and quality and impacts the sagittal balance of the spine. Skeletal muscle is essential to maintaining upright posture, and its progressive loss may compromise the compensatory mechanisms that preserve sagittal alignment. Sarcopenia can be diagnosed using thigh muscle thickness measured on a low-dose full-body radiograph. METHODS: Full-b

Ossification of the Nuchal Ligament With Text Neck Syndrome.

Ossification of the nuchal ligament (ONL) is a common but frequently overlooked radiographic finding on lateral cervical spine imaging, characterized by ectopic bone formation within the nuchal ligament, most often at the C4/5 and C5/6 levels. Although usually asymptomatic, ONL has been associated with cervical spondylosis, segmental instability, and, importantly, a markedly increased risk of coexisting ossification of the posterior longitudinal ligament (OPLL), a condition with potential for progressive myelopathy. We conducted a narrative review of the PubMed-indexed literature on the anatom

The Lower Lordotic Arch More Precisely Predicts Pelvic Positioning for Adult Spinal Deformity Patients with an Aligned T4-L1-Hip Axis.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To assess the combined effect of lower lordotic arch (LLA) and thoracolumbar (T4-L1-hip axis) alignment correction on the ability to predict sacral slope (SS) (i.e. pelvic retroversion) in adult spinal deformity (ASD). SUMMARY OF BACKGROUND DATA: The Roussouly framework characterizes the spine through a series of lordotic and kyphotic arches. The LLA should be equivalent to SS, meaning that pelvic positioning should be predicted by lumbar lordosis modifications between the lordotic apex and S1. However, positive T4-L1-hip axis mismatches will dr

A Spine Surgeon's Learning Curve With the Minimally Invasive L5 to S1 Lateral ALIF Surgical Approach: Perioperative Outcomes and Technical Considerations.

BackgroundLateral anterior lumbar interbody fusion (L-ALIF), or oblique lumbar interbody fusion at L5 to S1 (OLIF51), is a minimally invasive approach between the iliac vessels that provide indirect decompression, alignment restoration, and lordosis. While supine ALIF exposure has historically been performed by vascular surgeons, minimally invasive L-ALIF can be performed by spine surgeons familiar with lateral approaches. The L-ALIF is a relatively newer approach, and the objective of the present study was to assess the learning curve of the L-ALIF approach by comparing the pre- and postopera

Standalone Anterior Lumbar Interbody Fusion L5-S1 for Single-Level Degenerative Disc Disease: Implant Parameters Influencing Radiological Results.

Introduction Degenerative disc disease at the L5-S1 level is a common condition and is often associated with chronic low back pain and, in some cases, radicular symptoms. Anterior lumbar interbody fusion (ALIF) is intended to restore segmental and global lumbar lordosis (LL), improve sagittal alignment, and achieve decompression. The primary objective of this study was to examine the association between implant-related parameters and postoperative radiological outcomes following standalone L5-S1 ALIF. A secondary exploratory objective was to assess how closely postoperative LL at the L4-S1 and

Machine learning models for predicting treatment outcomes in chronic non-specific back pain patients undergoing lumbar extension traction.

Conservative treatment for chronic non-specific low back pain (CLBP) includes lumbar extension traction (LET) to re-align lumbar lordosis (LL). This study explores the use of machine learning (ML) models to predict post-treatment outcomes in patients with CLBP undergoing LET and how these predictions can support clinical decision-making. We utilized a retrospective database of 431 consecutive patients with uncomplicated CLBP. Post-treatment variables predicted included LL, NRS pain score, and Oswestry Disability Index (ODI). Input model variables included pre-treatment LL, sacral base angle (S

Apelin-13 induces lordosis behavior in estradiol-primed ovariectomized rats via the activation of multiple protein kinases in the ventromedial hypothalamus.

The present study aimed to determine whether apelin-13 facilitates lordosis behavior through the activation of specific hypothalamic kinases and to identify which signaling pathways mediate this response. Because lordosis is a well-established neuroendocrine indicator of female sexual receptivity, it served as an appropriate behavioral model to detect peptide-induced facilitation. Thus, we examined the role of protein kinase A (PKA), protein kinase C (PKC), mitogen activated protein kinase (MAPK), or Src kinase (Src), in the facilitation of lordosis behavior following bilateral intrahypothalam

Evaluation of Spino-Pelvic Parameters in Asian Patients Undergoing Total Hip Arthroplasty.

INTRODUCTION: Emerging research emphasizes the critical relationship between the hip and spine affecting implant positioning in total hip arthroplasty (THA). While spino-pelvic parameters and mobility patterns have been well described in Western populations, corresponding data from Asian patients undergoing THA remain limited. The primary objective of this study was to evaluate preoperative spino-pelvic parameters in Asian patients undergoing THA. The secondary objectives were to classify these patients using the hip-spine classification system and to compare the observed distribution with exi

Simultaneous prone transpsoas interbody fusion and osteotomies for severe deformity correction: multi-institutional retrospective review.

BACKGROUND: The increasing prevalence of adult spinal deformity (ASD) highlights the need for effective treatment strategies. While minimally invasive spine surgery has improved outcomes, patients with severe deformity often require more extensive correction. This study evaluates the outcomes of combining prone transpsoas (PTP) lumbar interbody fusion with posterior column osteotomies (PCO) or pedicle subtraction osteotomies (PSO) for severe ASD correction. METHODS: This retrospective, multicenter cohort study evaluated pre- and postoperative changes in 15 patients with severe ASD [minimally

Sagittal spinal alignment and segmental mobility in collegiate baseball players with low back pain: A cross-sectional study.

BACKGROUND: Low back pain is common among baseball players and may impair their performance and increase the risk of absence from competition. Abnormalities in the sagittal alignment of the spine and reduced trunk function have been suggested as risk factors, but evidence in collegiate players with skeletal maturity remains limited. This study investigated differences in sagittal alignment of the spine, segmental mobility, trunk flexibility, and isometric trunk muscle strength between collegiate baseball players with and without low back pain. METHODS: This cross-sectional study included colle
