A parametric analysis of interbody fusion cages placement: A finite elements approach comparing lumbar lordosis of bullet and steerable banana cages.

INTRODUCTION: Improper cage placement during spinal interbody fusion surgeries could lead to numerous post-operative complications. Biomechanical factors of such improper placement may result in loss of lumbar lordosis, foraminal stenosis, subsidence, and altered stress distribution to the tissues adjacent to the cage. OBJECTIVE: The aim of the present study is to compare three different lumbar interbody cage designs, placed posterior, middle, and anterior, and their biomechanical effect on the aforementioned studied parameter. METHODOLOGY: Cages and MRI-based lumbar spine models were develo

Sensitivity and Specificity of Elliptical Modeling and Sagittal Lumbar Alignment Variables in Normal vs. Acute Low Back Pain Patients: Does Pelvic Morphology Explain Group Lordotic Differences?

Background/Objectives: The lumbar lordosis (LL) is influenced by pelvic morphology, the unique dimensional characteristics of the pelvis. We investigated the sensitivity and specificity of lumbar sagittal radiographic alignment variables to discriminate between normal controls and acute low back pain (ALBP) patients. Methods: A total of 50 normal controls (29 men; mean age of 27.7 ± 8.5 years) with no history of low back pain and 50 ALBP patients (29 men; 28.1 ± 8 years of age) were compared. Radiographic variables included three measures of LL, a b/a elliptical modeling ratio, sacral base ang

Kinematic quantification of lever positioning manipulation under different working conditions and analysis of related individual factors: an observational cross-sectional study.

ABSTRACT: Lever positioning manipulation (LPM) is an innovative traditional Chinese medicine (TCM) spine manipulation developed from the conventional lumbar extension manipulation, and its clinical efficacy has been validated. Currently, patients exhibit significant individualized characteristics, and the effectiveness of the manipulation is closely related to the direction, angle, duration, and individual factors during its application. Therefore, it is imperative to conduct quantitative research on the operational parameters of LPM under different working conditions and analyze relevant indi

Expandable titanium interbody cage with adjustable height and lordosis for anterior cervical discectomy and fusion: a clinical and radiological study.

BACKGROUND: Anterior cervical discectomy and fusion (ACDF) is a widely used approach for cervical pathologies. However, achieving optimal sagittal alignment with static implants remains challenging. Expandable titanium cages (ETC) may offer the advantage of precise alignment adjustments. This study presents our early clinical experience with ETC in ACDF. METHODS: Between 2019 and 2023, we performed a retrospective analysis at Weill Cornell Medicine, Department of Neurosurgery, New York-Presbyterian Hospital, examining patients who underwent ACDF with ETC. Hospital records, imaging, and pre- a

The effect of smoking on the radiological and functional outcomes of anterior cervical discectomy and fusion with anchored standalone cages.

BACKGROUND: Anchored standalone cages for anterior cervical discectomy and fusion (ASC-ACDF) are easier to implant but have higher subsidence rates compared to anterior plating constructs. Smoking is a known risk factor for subsidence, however, there are conflicting reports on its effect on functional outcomes. This study aims to evaluate the impact of smoking on radiological and functional outcomes of patients undergoing ASC-ACDF. METHODS: Patients who underwent primary single- and double-level ASC-ACDF between 2015-2022 were reviewed. Inclusion: diagnosis of cervical myelopathy, radiculopat

The Lower Lordotic Arch Is a Driver of Pelvic Positioning and Inflection Point Migration in Adult Spinal Deformity: A Retrospective Cohort Analysis.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To evaluate the relationship between the lower lordotic arch (LLA) and other parameters of spinopelvic alignment in adult spinal deformity (ASD). SUMMARY OF BACKGROUND DATA: Lordotic and kyphotic curves maintain a geometrically balanced relationship defined by inflection points and apices along the spine. One proposed relationship suggests that pelvic positioning should be intimately related to the LLA, defined as the Cobb angle between the lordotic apex and L5. METHODS: We reviewed data of 304 adult thoracolumbar fusion patients between 2014 a

Occipito-Cervical Fusion Using Screw Rod Plate System in Craniocervical Pathologies: A Prospective Cohort Analysis of Long-Term Functional and Radiological Outcome With Minimum Two Years of Follow-Up.

INTRODUCTION: Occipito-cervical fusion is a standard treatment for craniovertebral junction (CVJ) instability and myelopathy. The technique has evolved from onlay bone grafting with halo immobilisation to rigid internal fixation with pedicle screws and rods construct, allowing early mobilisation and improving fusion rates. We describe our experience with the posterior-only surgical technique using enabling technologies, avoiding facet joint exposure and its associated complications. METHODOLOGY: A total of 19 patients who underwent occipito-cervical fusion for CVJ pathologies were evaluated.

Validation of a novel artificial intelligence model (SpinePose) to automatically and accurately predict spinopelvic parameters using scoliosis radiographs in an external cohort.

OBJECTIVE: SpinePose was developed in 2024 as a novel artificial intelligence (AI) tool to automatically predict spinopelvic parameters with high accuracy and without the need for manual entry. The authors' published results demonstrated excellent performance comparable to a fellowship-trained spine surgeon with more than 15 years of experience. To date, there have not been any studies that have externally validated the performance of AI-based spinopelvic parameter measurement tools on data acquired from other institutions. To assess the generalizability of SpinePose, the authors report its pe
