We developed and externally validated a hierarchical deep learning pipeline that automates cervical sagittal measurements, explicitly addressing C7 obscuration on lateral radiographs. The model combines a global keypoint detector with C2/C7 specialists localized via a multilayer perceptron to refine landmarks on high‑resolution patches. Trained on 5604 images and tested internally and on a challenging external cohort enriched for C7 obscuration (82%), it achieved excellent agreement with ground truth. Externally, intraclass correlation coefficients (ICCs) were 0.97 for lordosis (mean absolute

Background/Objectives: Adult degenerative scoliosis (ADS) is a spinal disease causing pain and reduced mobility, often occurring with degenerative lumbar spinal stenosis (DLSS). While fusion stabilizes the spine, it has drawbacks like loss of motion and adjacent segment degeneration. Minimally invasive techniques, such as tubular unilateral laminotomy for bilateral decompression (tULBD), provide a less invasive alternative, but their impact on ADS with DLSS is underexplored. This study examines the short-term effects of navigated tULBD on radiological and clinical outcomes in this patient popu

BACKGROUND: Although patients with THA may increase their use of hip flexion to compensate for loss of spinal motion after lumbar fusion, no studies have directly examined this relationship. Furthermore, the association between the number of fused levels on postoperative compensatory hip motion is not clear, making it difficult to anticipate these changes prior to lumbar fusion.
QUESTIONS/PURPOSES: (1) Is lumbar fusion after THA associated with resting spinopelvic alignment or compensatory changes at the hip? (2) Is the number of fused spinal segments associated with an increase in hip compen

Background Isthmic spondylolisthesis is commonly associated with back pain and neurological symptoms. The primary localised kyphotic deformity at the level of the spondylolisthesis is counterbalanced by increased lordosis across the lumbar spine. Spondylolisthesis reduction and fusion corrects the initial deformity and may also restore lumbar lordosis and sacral slope. In this study, we aimed to investigate how lumbar lordosis normalises following short-segment transforaminal lumbar interbody fusion (TLIF) for isthmic spondylolisthesis. Methodology In total, 54 consecutive patients from a sing

Motor vehicle collisions (MVCs) are a frequent cause of cervical spine injury and may result in persistent neurological and structural sequelae in a subset of patients. Many individuals may improve with standard active management. Complex presentations involving sagittal malalignment and suspected segmental instability remain clinically challenging. The relationship between cervical alignment, dynamic motion behavior, and neurological status continues to be explored within contemporary spinal biomechanics literature. This case report describes a 28-year-old woman who presented with persistent

STUDY DESIGN: Retrospective cohort.
OBJECTIVE: This study aims to assess the effects of CT-based subsidence on lumbar sagittal alignment and patient-reported outcome measures (PROMs) after transforaminal lumbar interbody fusion (TLIF), and to determine if subsidence and cage properties are independent predictors of postoperative changes in regional lordosis.
SUMMARY OF BACKGROUND DATA: Subsidence is a well-known complication following interbody fusion and has previously been associated with recurrence of preoperative symptoms and higher reoperation rates. There is incomplete evidence to char

STUDY DESIGN: Retrospective analysis.
OBJECTIVE: To describe normative cervical sagittal alignment and to propose a classification system to guide clinical assessment and surgical planning.
BACKGROUND CONTEXT: Optimizing alignment is a key goal of adult cervical deformity (ACD) surgery. The purpose of this study was to understand normative alignment utilizing an asymptomatic adult cohort and to formulate a classification system that would help identify the spinal regions contributing to the cervical deformity.
MATERIALS AND METHODS: A total of 468 asymptomatic adults (18-80 yr) from five co

PURPOSE: The purpose of this study is to evaluate the impact of preoperative LDI on postoperative sagittal alignment and surgical/patient-reported outcomes (PROMs) in a degenerative cohort. METHODS: Patients who underwent 1–2 level elective primary lumbar fusion (2014–2020) for degenerative disease were retrospectively identified. Preoperative/postoperative radiographs were reviewed for global, upper (L1-L3) and lower (L4-S1) LL, pelvic-incidence (PI), PI-LL mismatch, and pelvic tilt (PT). PI-LL mismatch and PT were assessed based on the literature. LDI was calculated as lower LL/global LL. Pa

BACKGROUND: Pathogenic ACAN variants ('aggrecanopathies') are increasingly recognised as a non-syndromic cause of skeletal dysplasias and short stature. Unlike many other aetiologies, ACAN-related disorder is reportedly associated with advanced bone age due to early epiphysial fusion, although presentation can vary. This study aimed to further characterise skeletal findings in paediatric and adult patients.
METHODS: A retrospective study of confirmed ACAN-related disorder cases from a tertiary genetics centre in England was performed. Data collected included patient demographics, antenatal ul

Thoracic wall deformity (TWD) is reported in several cat breeds. The Burmese and Bengal cats appear predisposed, suggesting a genetic background. Thoracic changes in Bengal kittens were investigated using whole-body computed tomography, macro- and microscopic examination of the rib cage. By using a questionnaire, breeding performance and kitten mortality among Bengal cats in Sweden during a two-year period was analyzed. TWD comprised a spectrum of symmetrical and asymmetrical rib cage changes. Macroscopically, lordosis, flat chest and pectus excavatum were frequent findings. When asymmetric, t

BACKGROUND: Dislocation remains a common reason for revision after total hip arthroplasty (THA), and adverse spinopelvic mobility is increasingly recognized as a major contributor to instability. Whether its prevalence differs between surgical approaches is unclear. This study compared adverse spinopelvic mobility and associated risk factors in patients who sustained a dislocation after a posterior approach (PA) or a direct anterior approach (DAA), and evaluated whether implant positioning differed between groups.
METHODS: A retrospective analysis was performed on 133 patients with post-opera

PURPOSE: To evaluate both radiographic and patient-reported outcomes after surgical correction of severe scoliosis in osteogenesis imperfecta (OI) patients with minimum 2-year follow-up. The main research question was whether modern multimodal surgical approaches translate into meaningful improvements in both deformity correction and quality of life measures.
METHODS: This retrospective analysis of prospectively collected data included 35 OI patients (mean age 12.2 ± 3.7 years) who underwent posterior spinal fusion for scoliosis at a single institution between 2013 and 2022. OI type 3 was mos

BACKGROUND: Lumbar degenerative diseases are prevalent spinal disorders among the elderly population worldwide, significantly impairing their quality of life. Both minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) and uniportal endoscopic posterior lumbar interbody fusion (Endo-PLIF) are established minimally invasive interbody fusion modalities for the management of these conditions. However, MIS-TLIF is associated with limitations such as soft tissue compression induced by dilator tubes and a comparatively prolonged postoperative recovery period. In the present study, we a

BACKGROUND: Double-sided VBT (dsVBT) (bilateral application to thoracic and lumbar major curves) was reported to provide successful thoracic and thoracolumbar curve corrections in skeletally immature adolescent idiopathic scoliosis (AIS) patients when applied under strict inclusion criteria in the short term. The present prospective cohort study aimed to present at least 5 years' results from the same study population to question the reliability of dsVBT in terms of correctional efficacy, growth modulation, and long-term functional outcomes.
METHODS: Thirteen consecutive skeletally immature (

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal deformity that arises in otherwise healthy children. Surgical correction for patients with a Cobb angle is usually performed with posterior corrective surgery utilizing rods bent intraoperatively. With the invention of patient-specific rods (PSR), we compared the radiological and functional outcomes of patients operated on with either rod.
METHOD: A retrospective cohort study in which AIS patients who underwent posterior corrective surgery from April 2019 until March 2021 in two state hospitals were recruited. The
