Stable low-grade degenerative spondylolisthesis does not adversely affect clinical outcome of full-endoscopic decompression in patients with spinal stenosis.

BACKGROUND AND OBJECTIVES: The optimal surgical management of lumbar spinal stenosis (LSS) in the presence of stable low-grade degenerative spondylolisthesis remains a subject of ongoing debate. This study aimed to evaluate the clinical outcomes and radiological stability of Lumbar full-endoscopic unilateral laminotomy with bilateral decompression (LE-ULBD) in this patient population. Specifically, we sought to determine whether this minimally invasive approach compromises spinal stability or leads to progression of spondylolisthesis over time. MATERIALS AND METHODS: A total of 36 patients di

Obturator Dislocations Following Direct Anterior Approach Total Hip Arthroplasty: Two Case Reports With Analysis of Implant and Spinopelvic Factors.

Dislocation remains a potential complication after total hip arthroplasty (THA), even with the anterior approach, which is generally associated with lower instability rates. Obturator dislocation, although well-documented in native hips following high-energy trauma, is extremely rare in the context of THA and has not been previously described in patients with preoperative spinopelvic assessment. We report two cases of obturator dislocation following direct anterior approach THA in young female patients without medical comorbidities. Both dislocations occurred during extreme hip flexion combine

Multiple Rod Construct for Occipito-Cervical Fusion: Overcoming Challenges With Enabling Technologies.

Introduction Occipito-cervical fusion is a well-established procedure for numerous cranio-vertebral junction (CVJ) anomalies. Owing to the complex biomechanics at play in this region, rigid internal fixation with pedicle screws and rods is needed to provide stability for fusion and good clinical outcomes. However, the rates of implant failure are still worrisome, especially in individuals with compromised anatomy and physiology. We describe a method for using multiple rod constructs for instrumentation across the CVJ, making the construct biomechanically stable, achieving high fusion rates, an

The correlation between loss of cervical lordosis and the severity of ligament ossification in patients with cervical ossification of posterior longitudinal ligament: a cross-sectional observational study.

BACKGROUND: Both genetic and environmental (e.g., mechanical stress) factors have been considered multifactorial components contributing to the development and progression of ossification of posterior longitudinal ligament (OPLL). Importantly, loss of cervical lordosis may change the mechanical stress on posterior longitudinal ligament. However, few studies have discussed the relationship between the cervical sagittal alignment and OPLL. The aim of this study is to quantify the cervical sagittal alignment and severity of ligament ossification in patients with cervical OPLL and to analyze their

[Clinical efficacy of anterior cervical mortise-tenon corpectomy and fusion for the treatment of ossification of the posterior longitudinal ligament].

OBJECTIVES: For the segmental ossification of cervical posterior longitudinal ligament (OPLL) accompanied by spinal cord compression, anterior cervical corpectomy and fusion (ACCF) can achieve direct decompression through removal of most of the involved vertebral body. However, reconstruction-related complications such as titanium mesh cage subsidence, loss of cervical lordosis, and poor bone fusion are common after surgery. This study aims to investigate the clinical efficacy of a novel anterior cervical mortise-tenon corpectomy and fusion (ACMTF) technique, designed based on principles of bi

Sagittal Alignment as a Determinant of Pain and Functional Recovery after Lumbar Surgery: A Systematic Review and Meta-Analysis.

BACKGROUND: Sagittal alignment is increasingly recognized as a key determinant of functional recovery after lumbar spine surgery; however, its role in degenerative lumbar disease remains variably reported. While alignment principles are well-established in adult spinal deformity, their applicability to routine degenerative procedures has not been quantitatively synthesized. METHODS: A systematic review and meta-analysis of 23 original clinical studies (1,837 adults) was performed. Eligible studies reported postoperative sagittal or spinopelvic parameters-pelvic incidence (PI)-lumbar lordosis

L1-L5 anterior lumbar interbody fusion: anatomical and technical considerations: a narrative review.

BACKGROUND AND OBJECTIVE: In the aging population, degenerative disc and facet joint disease is a common occurrence, and it frequently results in disability. Anterior lumbar interbody fusion (ALIF) surgeries offer effective treatments by stabilising the segment inducing the painful motion, providing indirect decompression of neural elements, restore lordosis and deformity correction. With direct visualization ALIF allows thorough discectomy and endplate preparation, with large surface area implants without the need for direct neural retraction which offers advantages over other techniques. The

Radiographic Evaluation of a Modular Anterior Lumbar Interbody Fusion (ALIF) Cage: Subsidence and Segmental Lordosis.

INTRODUCTION: Anterior lumbar interbody fusion (ALIF) is an established intervention for degenerative pathology and sagittal plane corrections. Despite advances in implant technology, challenges such as vertebral endplate damage, implant subsidence, and loss of segmental lordosis remain. A modular ALIF cage, designed with an endplate-first insertion technique, aims to protect endplates, reduce insertional trauma, and enable precise restoration of disc height and lordosis. The purpose of this study was to evaluate early radiographic outcomes after implantation of a modular ALIF device, specific

Comparative Accuracy of Preoperative Planning Technologies in Spinal Deformity Surgery: A Systematic Review.

Preoperative surgical planning is central to achieving optimal spinal alignment in spinal deformity (SD) surgery; however, the extent to which different planning modalities accurately translate preoperative goals into surgical outcomes remains incompletely characterized. This systematic review evaluates the accuracy and clinical usefulness of existing preoperative planning tools in SD surgery. A systematic search of MEDLINE, Embase, and the Cochrane Library was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Studies repor

Artificial intelligence-driven data expansion for the validation of spinopelvic parameter correlations in asymptomatic subjects.

BACKGROUND: The study of spinopelvic alignment in asymptomatic individuals is essential for understanding physiological sagittal balance and establishing reference values for spinal deformity assessment. However, the availability of large datasets of healthy subjects is limited by ethical, logistical, and radiation-related constraints. Artificial intelligence (AI)-based synthetic data generation may represent a promising strategy to overcome these limitations. METHODS: Full-spine standing radiographs from 123 asymptomatic subjects were retrospectively analyzed. Demographic characteristics and

Advancements in sagittal alignment parameters and management of adult spine deformities.

Adult spinal deformity (ASD) is increasingly recognized as a condition in which sagittal alignment plays a role in pain, disability, and mechanical failure. Historically, sagittal correction strategies relied on global radiographic parameters such as sagittal vertical axis (SVA) and pelvic incidence-lumbar lordosis (PI-LL) mismatch. While these measures demonstrated important associations with health-related quality of life, they incompletely account for compensated deformities, segmental mechanics, and directionality of correction. Over the past decade, alignment planning has evolved toward m
