RATIONALE: Lumbar spondylolisthesis (LS) with spontaneous fusion (SF) accompanied by sagittal imbalance (SI) poses a practical decision-making challenge in surgical planning. Here, we present a trauma-associated case of lumbar spondylolisthesis with SF and SI, and outline a reproducible, evidence-informed framework for surgical management.
PATIENT CONCERNS: A 57-year-old patient developed low back pain after trauma. Lumbar computed tomography (CT) showed L4 anterolisthesis with bilateral pars interarticularis defects. Despite standardized conservative treatment, the back pain progressed, with

OBJECTIVE: Anterior cervical disc arthroplasty offers theoretical advantages over fusion such as quicker recovery and decreased biomechanical stress at adjacent levels. However, anterior cervical discectomy and fusion (ACDF) may be beneficial for patients with dynamic instability or advanced spondylosis. In patients with multilevel cervical degenerative disc disease, anterior cervical hybrid arthroplasty-fusion (ACHAF) constructs may allow for personalized approaches, addressing level-specific pathology while offering other theoretical advantages such as requiring a shorter anterior plate (if

INTRODUCTION: cervical spondylosis is a disorder that is characterized by cervical and shoulder pain as the main symptoms. In some cases, it presents with myelopathy, which causes important functional disability in the adult population. There have been many surgical techniques described for treatment, with anterior cervical discectomy and fusion being the gold-standard. Studies have shown that using a cervical mesh as anterior support during a corpectomy allows for a wider decompression than other techniques; this surgical treatment comes with controversy as cage subsidence remains the greates

BACKGROUND: Although total hip arthroplasty (THA) is highly successful, standard cup orientation does not prevent all impingement-related complications. Functional planning integrates spinopelvic motion, but flexed-seated definitions of hip hypermobility remain unclear. We aimed to define clinically relevant flexed-seated femur-pelvis motion thresholds and test their association with simulated impingement and anteversion safe-zone constraints.
HYPOTHESIS: The existence of a pathological hip mobility threshold, independent of other risk factors for impingement with standard cup positioning, wo

BACKGROUND CONTEXT: Existing surgical alignment goals derived from populations with a high pelvic incidence (PI) are not applicable for patients with adult spinal deformity (ASD) and a low PI, who account for a high proportion of Asian populations. The surgical treatment for patients with a low PI is challenging because of their limited pelvic compensation capacity and because there are no criteria to guide corrective spinal deformity surgery in this population.
PURPOSE: To develop and validate a tailored sagittal correction strategy for patients with ASD and a low PI.
STUDY DESIGN/SETTING:

BACKGROUND: The theoretical Roussouly classification describes sagittal spinal profiles based on pelvic morphology. As most reference data originate from Caucasian populations, its applicability to Hispanic populations remains limited. This study aimed to describe sagittal spinal alignment in an asymptomatic Colombian population using the theoretical Roussouly classification.
METHODS: A retrospective cross-sectional study analyzed whole-spine standing radiographs (2020-2024) from asymptomatic adults aged 18-50 years at Clínica Tolima. Spinopelvic parameters were measured using Surgimap, and s

Introduction Chronic low back pain (CLBP) is defined as pain in the lower back that persists for more than three months. It is a common musculoskeletal condition and a major contributor to functional impairment and social limitations among young adults. Hamstring flexibility refers to the ability of the hamstring muscles to stretch to their full length. Since both the hamstrings and lumbar extensor muscles originate from the pelvis, a biomechanical relationship may exist between them. Decreased flexibility or weakness in these muscles may contribute to the development of low back pain. Tight h

BACKGROUND AND PURPOSE: Lumbopelvic rhythm describes the relationship between the lumbar spine and hip motion during trunk bending tasks, while the lumbar lordosis angle represents the normal sagittal curvature of the lumbar spine. This study aimed to investigate the effect of core stabilisation exercises on lumbopelvic rhythm, lumbar lordosis angle, and pain in men with chronic nonspecific low back pain (CNSLBP).
METHODS: This clinical trial was conducted among men aged 30-50 years with CNSLBP in Chaharmahal and Bakhtiari provinces of Iran. Participants (n = 28) were randomly assigned to an

Anterior lumbar interbody fusion (ALIF) at the L5/S1 level is a well-established surgical technique used in the management of degenerative, deformity, and instability-related pathologies of the lumbosacral junction. This region represents a biomechanically significant transition between the mobile lumbar spine and the pelvis, contributing substantially to overall lumbar lordosis while being subjected to considerable shear forces. Successful execution of L5/S1 ALIF requires a comprehensive understanding of both spinal and retroperitoneal vascular anatomy, including the relationship of the iliac

STUDY DESIGN: Retrospective cross-sectional study.
OBJECTIVE: To evaluate the clinical utility of the pelvic inclination angle (PIA) in surgically treated patients with adult spinal deformity (ASD) by examining its relationship with sagittal alignment parameters and patient-reported outcomes.
SUMMARY OF BACKGROUND DATA: Pelvic tilt (PT) is commonly used to assess pelvic compensation in ASD but is influenced by pelvic incidence (PI), limiting its interpretability across patients with different pelvic morphology. The pelvic inclination angle (PIA) has been proposed as a PI-independent paramete

The correct identification of spinal cord structures in magnetic resonance imaging (MRI) plays a vital role in identifying degenerative spondyloarthrosis, osteophytes, osteochondrosis, haemangioma, osteophytes and physiological lordosis malformations. Delineation, which is done physically, is time-consuming and can be varied. We will present a 3D U-Net-based deep learning model of volumetric segmentation of spinal cord MRI data in Digital Imaging and Communications in Medicine (.dcm) format in this research. The model takes advantage of three-dimensional convolutions to learn spatial dependenc

BACKGROUND: Many authors have highlighted the importance of assessing spinal alignment and mobility during total hip arthroplasty (THA), particularly regarding acetabular positioning. However, few studies have investigated the impact of spinal morphology on pelvic mobility from the standing to supine positions at the time of THA, performed via a direct anterior approach. The study aims to analyse this relationship in preoperative patients and its implications for planning acetabular anteversion.
METHODS: This retrospective study included 93 patients who underwent THA between January 2021 and

PURPOSE: The hip-spine relationship is increasingly recognized as clinically relevant in patients undergoing total hip arthroplasty (THA), yet associations between THA and changes in low back pain (LBP), sagittal alignment, and functional disability remain incompletely understood. This prospective study evaluated two-year changes in LBP (primary clinical endpoint) and radiographic alignment, with global sagittal alignment and spinopelvic alignment defined as primary radiographic endpoints, as well as their associations with postoperative pain and disability.

BACKGROUND: Posterior decompression for degenerative lumbar spinal stenosis (DLSS) carries approach-related morbidity. In this study, we hypothesized that multilevel lateral lumbar interbody fusion (LLIF) achieves effective indirect decompression with acceptable safety and patient-reported improvement among DLSS patients without scoliosis.
METHODS: This was a single-institution retrospective case series (2016-2022) of adults with symptomatic spondylotic stenosis and severe disc degeneration without scoliosis (Cobb angle <10°) undergoing multilevel (≥2) LLIF. Patients with concurrent posterior

Background: Cage subsidence after minimally invasive transforaminal lumbar interbody fusion raises revision risk and costs. Intraoperative computed tomography (CT) provides high-resolution, three-dimensional visualization of the endplate-cage interface and serves as a practical-though itself imperfect-reference standard for early subsidence, but it is not available at all institutions. Plain X-ray is widely available and inexpensive, but lower in resolution. The clinically relevant question is therefore not whether CT and X-ray are equivalent, but rather which X-ray protrusion depth measuremen

INTRODUCTION: Stiff-person syndrome (SPS) is a rare autoimmune neurological disorder characterized by progressive muscle rigidity and spasms. While it predominantly affects adults, pediatric cases are exceptionally rare, often leading to delayed diagnosis due to non-specific early clinical manifestations.
CASE REPORT: We report the case of an 11-year-old boy with progressive gait abnormalities and rigidity, initially misdiagnosed as an orthopedic disorder. The patient exhibited a forward-leaning posture, absent lumbar lordosis, and significant muscle tightness, leading to a primary surgical i

Hypochondroplasia is a rare genetic condition that causes disproportionate short stature. This retrospective, real-world, matched-cohort study used electronic health records data from the Clinical Practice Research Datalink Aurum primary care database, linked to Hospital Episode Statistics and Office of National Statistics mortality records, to assess the natural history of hypochondroplasia in England from 1998 to 2019. A total of 549 participants with hypochondroplasia and 2196 matched controls were included. Mean follow-up time was 7.50 years for participants with hypochondroplasia and 11.6

INTRODUCTION: Predicting post-operative changes in thoracic kyphosis (TK) remains challenging in adult spinal deformity (ASD) surgery. This study aimed to quantify both iatrogenic and reciprocal changes in TK following lumbar correction and assess their maintenance at 2-year follow-up.
METHODS: A retrospective analysis was performed on 356 ASD patients treated with posterior instrumentation from T9-11 to the pelvis, with a minimum 2-year follow-up. TK values were compared with normative data. Multivariate analysis identified thoracolumbar parameters associated with reciprocal thoracic changes

STUDY DESIGN: Propensity-matched retrospective cohort.
OBJECTIVE: The objective of this study was to determine whether lumbar disc replacement (LDR) can provide radiographic indirect decompression as effectively as anterior lumbar interbody fusion (ALIF).
SUMMARY OF BACKGROUND DATA: Isolated lumbar foraminal stenosis can often be treated with indirect decompression via ALIF. More recently, LDR has emerged as a motion-preserving treatment option for lumbar disease; however, its ability to provide indirect decompression has not been studied.
METHODS: All patients undergoing 1-level ALIF and L
