Initial Radiographic Outcomes of Titanium Cage Implants in Anterior Cervical Discectomy and Fusion: A Retrospective Review.

Introduction Anterior cervical discectomy and fusion (ACDF) is a common surgical treatment for degenerative diseases and trauma in the cervical spine. The procedure aims to restore and maintain lordosis in the cervical spine and increase intervertebral disc height; ultimately leading to vertebral body fusion and spinal stabilisation. Polyetheretherketone cages are commonly utilised; however, new implant technologies such as 3D-printed titanium cages have emerged. There are few published reports regarding titanium cages, and therefore, this study evaluated the initial radiological outcomes of t

Restoring segmental lumbar lordosis after failed previous fusion at the same level.

One of the major challenges in revision spinal surgery lies in restoring segmental lordosis at the level of previous surgery. We have developed a surgical technique that can help to avoid the need for a long surgical procedure, reduce the extent of spinal instrumentation, and eliminate the need for multiple stages of surgery in order to significantly restore lumbar lordosis after a failed previous operation. In this article, we provide our single institutional surgical experience including main indications and contraindications, a step-by-step surgical technique description. Segmental lordosis

Impact of lumbar pedicle subtraction osteotomy (PSO) level on global alignment and proportion (GAP) score in revision adult iatrogenic flatback spinal deformities.

PURPOSE: To explore the impact of different lumbar pedicle subtraction osteotomy (L-PSO) levels on Global Alignment and Proportion (GAP) scores. METHODS: Adults at a single center who underwent lumbar PSOs with revision instrumentation [thoracolumbar junction (T9-L1) to pelvis] and a minimum 2-year follow-up were reviewed. The patients were divided by level of PSO (L2, L3, L4, and L5) and compared with respect to demographic and surgical data, sagittal parameters, GAP scores, and mechanical complications requiring revision operations. RESULTS: 152 patients (average age 64.4 ± 10.6 years, ave

Association of age-adjusted pelvic incidence minus lumbar lordosis correction with long-term radiographic and clinical outcomes in adult spinal deformity surgery.

OBJECTIVE: Adequate correction of sagittal spinal malalignment is crucial in adult spinal deformity (ASD) surgery. Although the age-adjusted pelvic incidence (PI)-lumbar lordosis (LL) scheme has been widely used for ideal sagittal correction, its association with long-term surgical outcomes is not well documented. Therefore, the authors aimed to evaluate the association of age-adjusted PI-LL correction with long-term radiographic and clinical outcomes. METHODS: The authors included patients who underwent ≥ 5-level fusion including the sacrum for ASD between 2012 and 2023. The patients were ca

Factors Associated With Postoperative Kyphosis and Loss of Range of Motion After Cervical Disc Replacement.

STUDY DESIGN: Single-center retrospective study. OBJECTIVE: To evaluate the risks associated with postoperative kyphosis and loss of range of motion after cervical disc replacement (CDR). SUMMARY OF BACKGROUND DATA: One of the main benefits of CDR is that it maintains physiological range of motion (ROM) and lordosis while achieving decompression. However, some patients experience loss in segmental ROM or postoperative segmental kyphosis. This study analyzes the radiographic outcomes of these patients. METHODS: Adult patients who underwent CDR were included. The cohort was divided into patie

Lessons From Recurrent Dropped Head Syndrome After Inadequate Short Fixation: A Case Series.

Dropped head syndrome (DHS) is a condition in which the head falls forward due to dysfunction or atrophy of the cervical extensor muscles. It is more commonly observed in elderly women and significantly affects horizontal gaze and activities of daily living (ADL). For cases in which conservative treatment is ineffective, surgical corrective fixation is considered; however, indications and standardized procedures have not yet been fully established. We retrospectively analyzed five cases of DHS treated surgically at our institution and examined the efficacy of corrective fixation and potential

Prone transpsoas (PTP)-LLIF approach versus standard LLIF in patients undergoing surgical revision following adult spinal deformity correction surgery.

INTRODUCTION: Both the prone transpsoas (PTP) single-position lateral lumbar interbody fusion (LLIF) and standard (dual-position) LLIF enable minimally invasive (MIS) indirect decompression and deformity correction. Existing studies comparing the utility of these approaches for adult spinal deformity (ASD) correction have been limited to small, single-center series. No study has investigated revision single-position PTP-LLIF versus standard LLIF in a large cohort of ASD patients. METHODS: A multi-center cohort of all patients undergoing PTP-LLIF or standard LLIF revision for ASD between 2013

Vosoritide (Voxzogo) for Achondroplasia: A Review of Clinical and Real-World Evidence.

Achondroplasia is the most common genetic skeletal dysplasia, caused by activating mutations in the FGFR3 gene that impair endochondral ossification and result in disproportionate short stature. Vosoritide (VOXZOGO®), a C-type natriuretic peptide analog, is the first targeted therapy approved for achondroplasia and acts by antagonizing FGFR3 signaling to promote bone growth. This review evaluates current clinical evidence on the efficacy, safety, and long-term potential of vosoritide in children with achondroplasia. A structured literature search of PubMed and Embase was conducted using the te

Development of thoracic spine kyphosis and lumbar spine lordosis in the growing child from birth to adulthood: protocol for a systematic review.

INTRODUCTION: Thoracic kyphosis, a posterior curvature of the thoracic spine and lumbar lordosis, an anterior curvature of the lumbar spine, are essential components of spinal alignment that facilitate a biomechanically efficient upright stance. Understanding the normal growth of the spine and its constituents, especially during periods of rapid growth, is important as deviations from normal growth are thought to be initiating factors of spinal conditions such as scoliosis. While in the coronal plane the spine is straight, there is variability in the reported normative values of sagittal spina

Analysis of sagittal alignment changes in the spine-pelvis joint in sitting and standing positions after long- or short-segment fixation to the pelvis for lumbar degenerative diseases.

BACKGROUND: The spine, pelvis, and joints maintain sagittal balance, which is often disrupted in lumbar degenerative diseases. While preoperative changes in sagittal alignment are well studied, postoperative adaptations, particularly following spinal fixation extending to the pelvis, are less understood. Therefore, the present study aimed to examine sagittal changes in the spine, pelvis, and joints in sitting and standing positions after short- or long-segment posterior spinal fixation extending to the pelvis. METHODS: This cross-sectional study analyzed patients who underwent long- or short-

Durability in symptomatic and radiographic outcomes after standalone anterolateral minimally invasive surgery for adult spinal deformity.

PURPOSE: Adult spinal deformity (ASD) impacts patients' quality-of-life. Minimally invasive surgery (MIS) can adequately treat carefully selected patients with ASD. Stand-alone lateral interbody fusion has been found effective in the treatment of low-grade degenerative spondylolisthesis, degenerative lumbar scoliosis as well as other degenerative spine conditions. However, the efficacy of anterolateral interbody fusion as an alternative approach to open spinal deformity correction has not been fully explored. Herein, the outcomes of patients who underwent standalone MIS anterolateral fusion we

The Impact of Opening Wedge High Tibial Osteotomy on Spinal Alignment.

Introduction Flexion contracture of the knee has been reported to induce forward trunk inclination and pelvic retroversion, whereas the progression of pelvic retroversion may further exacerbate knee joint symptoms, suggesting a close relationship between the knee and spinal alignment. The purpose of this study was to investigate the effects of lower limb alignment changes after opening wedge high tibial osteotomy (OWHTO) on spinopelvic sagittal alignment. Methods We retrospectively analyzed 34 knees that underwent OWHTO for medial compartment knee osteoarthritis between 2023 and 2025. Standing
