Human immunodeficiency virus ocular involvement and retinopathy: Clinical spectrum and management strategies.

This study highlights the significant ocular complications associated with human immunodeficiency virus (HIV), particularly its impact on the retina. Historically, HIV infection frequently resulted in severe retinal infections, such as Cytomegalovirus retinitis. However, with the widespread adoption of highly active antiretroviral therapy, the prevalence of these infections has significantly declined. Despite this progress, unresolved challenges in treatment approaches remain. In addition, HIV is known to cause nonopportunistic ocular conditions, such as optic neuropathy, immune recovery uveit

Risk of uveitis in patients with human immunodeficiency virus: a cohort study through TriNetX database.

BACKGROUND: This multi-institutional, retrospective cohort study using TriNetX database aims to explore the relationship between HIV infection and uveitis. METHODS: HIV patients were propensity-matched to individuals from our non-HIV cohort (1:1 ratio) based on variables such as age (every 5 year), sex, ethnicity, race, and relevant comorbidities. The Cox proportional hazards regression model was utilized to assess the impact of variables on uveitis risk, reporting hazard ratios (HRs) with 95% confidence intervals (CIs). Kaplan-Meier survival analysis and log-rank tests were applied to estima

The relationship between household economic shocks, depression, and elevated stress-responsive biomarkers among adolescent girls and young women in rural South Africa (HPTN 068).

BACKGROUND: Common mental health disorders (CMDs) significantly contribute to the global burden of disease among adolescent girls and young women (AGYW). In South Africa, the high proportion of AGYW experiencing CMDs in part reflects adversity in early life and resulting chronic stress, particularly through exposure to poor economic conditions such as frequent household economic shocks. Yet, longitudinal evidence for the relationship between household economic shocks, CMDs, and chronic stress among AGYW is limited. METHODS: Data are from the HIV Prevention Trials Network (HPTN) 068 study in r

Immuno-informatics analyses of important esophageal cancer associated viruses for multi-epitope vaccine design.

INTRODUCTION: Esophageal cancer (EC) is a highly lethal malignancy characterized by the uncontrolled proliferation of cancerous cells within the esophagus. Despite recent advancements in therapeutic strategies, the prognosis remains poor, underscoring the urgent need for novel preventive and therapeutic approaches. Notably, several oncogenic viruses have been implicated in EC pathogenesis, prompting the exploration of epitope-based vaccines through immunoinformatics. METHODS: Using immunoinformatics and bioinformatics approaches, we designed a novel multi-epitope vaccine targeting viral agent

Applications of CRISPR-Cas-Based Genome Editing Approaches Against Human Cytomegalovirus Infection.

Human cytomegalovirus (HCMV), a globally ubiquitous herpesvirus with the ability to carry out both lytic productive and lifelong latent infections, is a major cause of congenital infections, often leading to intellectual disabilities and neurological disorders. Moreover, HCMV is an opportunistic pathogen commonly found in immunocompromised individuals such as organ transplant recipients, HIV-positive individuals, and cancer patients, causing severe and life-threatening complications. While effective in inhibiting viral lytic infection, current FDA-approved compounds cannot eliminate the latent

Brief Report: Herpesvirus Viremia, ART Response, and Growth Recovery in Kenyan Children Starting Antiretroviral Therapy in Hospital.

BACKGROUND: We recently reported that cytomegalovirus (CMV) viremia was associated with worse clinical outcomes in hospitalized children starting antiretroviral therapy (ART). Here, we interrogate associations between CMV and Epstein-Barr (EBV) viremia and HIV viral suppression, CD4 recovery, and growth after ART initiation. SETTING: In-patient setting, 4 hospitals in Kenya. METHODS: We used regression models to assess associations between CMV and EBV viremia and 6-month growth and immunologic and virologic responses to ART in children aged 2 months to 12 years. RESULTS: Overall, 142 (78%)

Venous thromboembolism associated with infections: a systematic review and meta-analysis.

BACKGROUND: Venous thromboembolism (VTE) is a disease with complex pathophysiology and a significant cause of morbidity and mortality. Infections have been associated with VTE, but the synthesis of the incidence and strength of this association has not been performed. METHODS: A systematic review and meta-analysis were conducted to evaluate the association between infections and VTE. We excluded studies assessing the relationship between VTE and local skin infections, postpartum, malignancies, hypercoagulability syndromes, as well as studies that focused explicitly on orthopedic patients, COV

Stress-responsive biomarkers and sexual behaviors among young women in rural South Africa.

BACKGROUND: Adolescence is a critical period when youth develop their decision-making skills and may engage in their first sexual encounters. Stress during this time can affect decision making; however, limited research has examined the relationship between biological stress correlates and sexual behavior among adolescent girls and young women (AGYW) in high-HIV incidence areas. PURPOSE: To examine whether stress-responsive biomarkers are longitudinally associated with sexual behaviors that are predictive of HIV. METHODS: We used data from a cohort of 897 AGYW nested within the HIV Preventio

Cytomegalovirus-selected blood components in allogeneic stem cell transplantation: A follow-up survey of English transplant centre practices.

BACKGROUND: Cytomegalovirus (CMV) infection remains a major cause of morbidity and mortality after allogeneic stem cell transplantation (aSCT). In 2012, the UK Advisory Committee for the Safety of Tissues and Organs (SaBTO) recommended that CMV-unselected (CMV-U) blood components could be safely transfused to this population without increasing the risk of transfusion-transmitted CMV (TTI-CMV). A 2015 survey of UK transplant centres found that 22.7% of aSCT centres did not follow this recommendation. In response, a subsequent good practice paper addressed concerns regarding the determination of

Human cytomegalovirus UL78 is a nuclear-localized GPCR necessary for efficient reactivation from latent infection in CD34+ hematopoietic progenitor cells.

UNLABELLED: Human cytomegalovirus (HCMV) is a ubiquitous pathogen that persists throughout the lifetime of the host due to the establishment of latency. HCMV encodes four putative G protein-coupled receptors (GPCRs): US27, US28, UL33, and UL78. A definitive role for UL78 in HCMV infection has yet to be elucidated. Utilizing an in vitro CD34+ hematopoietic progenitor cell (HPC) model, we demonstrate that a recombinant virus lacking UL78 protein expression fails to efficiently reactivate from latent infection. Furthermore, we show that UL78 preferentially couples to the Gαi family of G proteins

Clinical Presentation, Genetic Testing, and Outcome of Congenital Nephrotic Syndrome in KwaZulu-Natal, South Africa.

INTRODUCTION: The aetiology of congenital nephrotic syndrome (CNS) is heterogenous with genetic and more rarely infectious or maternal allo-immune disease causes. In resource-replete settings, 60-80% of CNS is attributable to monogenic causes, with other causes much more rarely diagnosed. The relative prevalence of these aetiologies and differences in clinical presentation are understudied in southern Africans. We describe the aetiology, clinical presentation, outcomes, and genetic testing results in black African infants with CNS. METHODS: We enrolled 36 children with CNS from a tertiary ref

The synergistic tapestry: unraveling the interplay of parvovirus B19 with other viruses.

Parvovirus (B19V) is a compact, non-enveloped, spherical virus with a single-stranded DNA genome. In immunocompetent individuals, parvovirus B19V infection is typically asymptomatic or mildly symptomatic. However, in patients with compromised immune systems, it can lead to severe anemia in patients with compromised immune systems; renal transplant recipients taking immunosuppressive therapies often experience B19-induced anemia and red cell aplasia. The coinfections of hepatitis B virus, hepatitis C virus, cytomegalovirus (CMV), HIV, and BK virus with B19V have been reportedly investigated. Th

Unveiling the Hidden Burden of TORCH Infections in HIV Patients: A Residual Serum Seroepidemiological Study in a Korean Metropolitan City.

BACKGROUND: Although human immunodeficiency virus (HIV) is a legally notifiable disease in Korea, surveillance data are reported in aggregate without regional breakdowns. This limits the ability of local governments to design targeted prevention and care strategies. The coronavirus disease 2019 pandemic underscored the importance of region-specific epidemiological data for timely public health responses. TORCH infections-comprising Toxoplasma gondii, Treponema pallidum, rubella virus, cytomegalovirus (CMV), and herpes simplex virus (HSV)-can cause severe complications in immunocompromised indi
