Lived Experiences of Vertebral Body Tethering: A Qualitative Study of the Patient Journey.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is the most common spinal deformity in children. While posterior spinal fusion is the standard treatment for curves ≥45°, anterior vertebral body tethering (VBT) has emerged as an alternative for some patients. This study explores the experiences of patients who underwent VBT, focusing on decision-making, recovery, and long-term concerns. PATIENTS AND METHODS: Semi-structured interviews were conducted with twenty patients diagnosed with progressive AIS and a spinal deformity less than 60° at the time of surgery. All participants underwent VBT, co

Preoperative pain experience and distribution in relation to mental health in Lenke type 1 and 2 adolescent idiopathic scoliosis: a prospective cross-sectional study.

BACKGROUND: Pain is common in adolescents with idiopathic scoliosis (AIS), yet its distribution is heterogeneous and mechanisms remain incompletely understood. Beyond structural factors, emerging evidence suggests a psychosocial component with links to mental health. This study examined the relationship between preoperative pain and its distribution with mental health in Lenke type 1-2 AIS patients undergoing posterior spinal fusion. METHODS: In this prospective single-center cohort study (July 2023-July 2025), Lenke type 1-2 AIS patients completed the revised SRS-22 (SRS-22r) questionnaire a

Prevalence and characteristics of scoliosis among ethiopian schoolchildren aged 6-15 Years: A school-based cross-sectional study.

BACKGROUND: Scoliosis is a progressive spinal deformity that often develops during childhood and adolescence. In Ethiopia, population-level prevalence data are scarce, and school-based screening, though practical, may overestimate cases without radiographic confirmation. Understanding its distribution and severity is critical for guiding clinical and public health strategies. OBJECTIVES: To estimate the prevalence of scoliosis among Ethiopian schoolchildren, characterize its types and severity, and examine associations with clinical and anthropometric variables. METHODS: A cross-sectional sc

The magnitude of postoperative anterior vertebral body tethering scoliosis correction as measured by inter-screw angulation is dependent on the remaining growth potential at the time of surgery.

BACKGROUND: To assess how skeletal maturity at the time of anterior vertebral body tether (VBT) surgery affects progressive scoliosis correction, we compared the segmental rate of subsequent postoperative scoliosis correction, as measured by inter-screw angulation, based on preoperative Sanders score. METHODS: Thoracic idiopathic scoliosis patients with minimum 2 years follow-up after VBT (2012-2018) had coronal segmental inter-screw angulation measured for each tethered level at first erect (FE) and every 6mo up to ≥ 24mo. Change in screw angulation between 6mo intervals was calculated. Gene

Bone mineral density in pediatric Marfan syndrome patients with scoliosis: a retrospective case-control study using hounsfield units.

PURPOSE: Bone mineral density (BMD) in pediatric Marfan syndrome (MFS) patients remains debated. While dual X-ray absorptiometry for BMD assessment is gold standard, it is unreliable in scoliosis. Hounsfield Units (HU) measurement from lumbar CT scans in pediatric patients is increasingly used. The aim of this study is to evaluate BMD in patients with MFS by comparing HU values with those of adolescent idiopathic scoliosis (AIS) patients scheduled for spinal correction surgery. METHODS: Propensity score matching (1:2, ages 10–19) was performed between MFS and AIS patients based on age, sex, bo

Electroacupuncture-augmented breaststroke vs. breaststroke alone for mild AIS: superior correction in a retrospective cohort.

BACKGROUND: In China, acupuncture is commonly used for adolescent idiopathic scoliosis (AIS), but it requires combination with other therapies. In clinical practice, orthopedic surgeons often recommend swimming for mild AIS patients with Cobb angle <25°. However, owing to the paucity of high-quality evidence and standardized protocols, the efficacy of swimming for AIS remains controversial. The objective of this study was to evaluate the efficacy and safety of breaststroke combined with electroacupuncture vs. breaststroke alone in skeletally immature patients with mild AIS. METHODS: This was

Redefining Mental Health Assessment in Adolescent Idiopathic Scoliosis using PROMIS Psychological Well-Being Measures.

STUDY DESIGN: Prospective. OBJECTIVE: To evaluate the construct validity of PROMIS Positive Affect (PA), Life Satisfaction (LS), and Meaning & Purpose (MP) measures compared to the SRS-22 in AIS patients and determine whether these measures capture distinct aspects of mental health not adequately assessed by traditional instruments. SUMMARY OF BACKGROUND DATA: Traditional AIS mental health assessment focuses on psychopathology, but optimal well-being requires positive psychological functioning too. PROMIS PA, LS, and MP measures need validation in AIS populations and may capture patients who

Redefining Proximal Junctional Kyphosis After Posterior Spinal Fusion for Idiopathic Scoliosis: PJA >12 Is Associated With Increased Pain and Decreased Mobility, But Not Reoperation.

INTRODUCTION: Proximal junctional kyphosis (PJK) occurs at moderate rates following posterior spinal fusion (PSF) in idiopathic scoliosis (IS). However, the impact of preoperative sagittal alignment and the clinical consequences of PJK, particularly effects on Patient-Reported Outcome Measurement Information System (PROMIS) scores, remain underreported. METHODS: A query of an institutional pediatric spinal deformity database identified patients undergoing primary PSF for IS with a minimum 2-year follow-up. Preoperative and latest follow-up radiographs, as well as PROMIS scores, were analyzed.

Minimum 5-Year Clinical and Radiographic Results of Double-Sided Vertebral Body Tethering Applied to Double Adolescent Idiopathic Scoliosis Curves of the First 13 Patients: Important Lessons Learned.

BACKGROUND: Double-sided VBT (dsVBT) (bilateral application to thoracic and lumbar major curves) was reported to provide successful thoracic and thoracolumbar curve corrections in skeletally immature adolescent idiopathic scoliosis (AIS) patients when applied under strict inclusion criteria in the short term. The present prospective cohort study aimed to present at least 5 years' results from the same study population to question the reliability of dsVBT in terms of correctional efficacy, growth modulation, and long-term functional outcomes. METHODS: Thirteen consecutive skeletally immature (

Comparison of Functional Outcomes and Sagittal Balance in Adolescent Idiopathic Scoliosis Corrective Surgery Utilizing Conventional Rods and Patient-Specific Rods: Preliminary Results.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal deformity that arises in otherwise healthy children. Surgical correction for patients with a Cobb angle is usually performed with posterior corrective surgery utilizing rods bent intraoperatively. With the invention of patient-specific rods (PSR), we compared the radiological and functional outcomes of patients operated on with either rod. METHOD: A retrospective cohort study in which AIS patients who underwent posterior corrective surgery from April 2019 until March 2021 in two state hospitals were recruited. The

Radiographic Sagittal Alignment and Kinetic Chain Alterations in Geriatric Patients With Scoliosis: A Case Series.

Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal deformity that originates in adolescence but carries lifelong biomechanical consequences. In geriatric patients, chronic pain associated with a history of AIS is frequently attributed to the scoliotic curve itself. This case series proposes an alternative framework. Pain in this population may originate from long-term kinetic chain dysfunction and accelerated degeneration driven by decades of compensatory biomechanical loading. Two geriatric subjects with a history of AIS and chronic pain presented to a private chiropractic cl

Are Levels of Blood Metal Nanoparticles Elevated After Spinal Fusion and Implantation in Patients With Adolescent Idiopathic Scoliosis?

BACKGROUND: Implantation of titanium alloys (Ti-6Al-4V) or cobalt-chromium-molybdenum alloys (Co-Cr-Mo) for long-segment spinal fixation is the standard surgical treatment for adolescent idiopathic scoliosis (AIS), but long-term presence of the implants in the body may lead to the release of metal nanoparticles. Unlike orthopaedic internal fixation surgeries, spinal implants generally remain in the human body for a lifetime. The release of metal nanoparticles remains a potential clinical issue, but currently there is a lack of strong data on the topic. This study aimed to fill this crucial kno

Opioid Prescribing Patterns For Adolescents Undergoing Posterior Spinal Fusion for Adolescent Idiopathic Scoliosis: A Multi-Site Retrospective Study.

INTRODUCTION: As the opioid crisis has evolved from prescription drug abuse to illicit fentanyl use, research has identified leftover surgical medications as still being a significant source of misuse. While the CDC provides quantitative opioid prescribing benchmarks for adults, discrete numerical targets for the pediatric and adolescent populations remain poorly defined. This study evaluates longitudinal trends in opioid prescribing patterns for adolescents undergoing posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS) across a large, multi-site healthcare system. Spinal energy balance can predict post-operative spine alignment in Lenke 1 Adolescent Idiopathic Scoliosis.

PURPOSE: The preoperative planning of adolescent idiopathic scoliosis (AIS) remains largely debated. We hypothesized that adopting a biomechanical energetic framework could provide valuable insights for exploring the impact of spinal arthrodesis. Using this approach, we conducted a comparative analysis to quantify discrepancies between in silico simulations derived from preoperative radiographs and the actual three-dimensional spinal alignment obtained from post-operative imaging. METHODS: Fifty-two consecutive patients with Lenke Type 1 AIS (mean age: 16 years; mean thoracic Cobb angle: 52°)
