Sagittal spine length moderately correlates with pulmonary function for the timing of definitive fusion in growth-friendly graduates with early onset scoliosis.

PURPOSE: Severe spinal curvature in early onset scoliosis (EOS) impairs pulmonary development and function. The treatment requires imaging to assess spine growth and pulmonary function. The purposes of this study were to determine if radiographic sagittal spinal length (SSL) correlates with pulmonary function testing (PFT) and to compare SSL with T1-T12 coronal height (CH) to guide definitive fusion in EOS patients. METHODS: Growth-friendly graduate EOS patients who underwent definitive spinal fusion and had postoperative PFT data were identified in a multi-center database. CH and SSL measure

Outcomes and complications of bipolar fusionless constructs for early-onset scoliosis: a systematic review.

PURPOSE: Early-onset scoliosis (EOS) is a spinal deformity manifesting before age ten, often linked to neuromuscular conditions. Traditional Growing Rod (TGR) techniques, while effective, are associated with high complication rates and require frequent surgeries. Bipolar fixation techniques, which utilize ilio-sacral screws and sliding rods, have emerged as a less invasive alternative, aiming to preserve spinal growth. This systematic review evaluates the safety and efficacy of bipolar fixation in predominantly neuromuscular EOS and mixed pediatric cohorts reported in the current literature, e

Does pelvic fixation impact reoperation outcomes for neuromuscular scoliosis surgery? A 10-year matched cohort analysis.

PURPOSE: To report revision surgery rates in neuromuscular scoliosis (NMS) patients undergoing posterior spinal fusion with or without pelvic fixation. METHODS: The PearlDiver database identified patients who underwent posterior spinal fusion for NMS from 2015 to 2021. Two cohorts-patients with pelvic fixation and those without-were matched for age, gender, number of levels fused, and wheelchair use. Primary outcomes were rates of subsequent posterior spinal fusion and instrumentation, and spinal wound debridement. Secondary outcome assessed subsequent posterior spinal fusion rates after furt

Effect of mixing dissimilar metals on serum metal ion levels following spinal deformity surgery.

PURPOSE: The increased use of hybrid spinal implants combining dissimilar metals raises concerns for galvanic corrosion, which can affect metal ion release and implant longevity. This study examines the variation in post-fusion metal ion concentrations among different hybrid implant constructs. METHODS: In this prospective multicenter observational study, patients undergoing elective surgery for idiopathic or neuromuscular scoliosis received either cobalt-chromium (CoCr/Ti) or stainless steel (SS/Ti) rods with titanium screw implants. Blood metal ion concentrations were measured preoperativel

Intraoperative neurophysiological monitoring changes during surgical correction of pediatric neuromuscular scoliosis: a single-institution report.

PURPOSE: Intraoperative neurophysiological monitoring (IONM) is a tool that can improve safety during spinal surgery. Patients with neuromuscular scoliosis (NMS) have inherent neural axis or primary muscle pathology that may present challenges to neuromonitoring. An understanding of IONM event rates, patterns, and outcomes would better prepare surgeons to counsel patients and families. METHODS: IONM data from pediatric spine deformity correction procedures in patients with neuromuscular scoliosis at a single quaternary care center between 2012 and 2022 were reviewed to determine cases with mo

Dual vs. single surgeon in spinal deformity correction: a meta-analysis.

BACKGROUND CONTEXT: Spinal deformity correction is technically demanding and often associated with extended operative time, substantial blood loss, and elevated complication rates. Dual-attending surgeon (DS) models have emerged as a strategy to enhance operative efficiency and patient safety, though published outcomes remain inconsistent. PURPOSE: To evaluate the impact of a DS approach compared with a traditional single-attending surgeon (SS) strategy on perioperative and postoperative outcomes in spinal deformity surgery. STUDY DESIGN/SETTING: Meta-analysis of comparative studies assessin

Reducing Transfusions in Neuromuscular Spinal Fusion: A National Surgical Quality Improvement Program-Pediatrics Quality Improvement Project.

BACKGROUND: Pediatric spinal fusion can result in substantial blood loss, particularly in patients with scoliosis related to underlying neuromuscular (NM) disease. Transfusion is frequently required but carries risks including transfusion reactions, alloimmunization, volume overload, and infection. LOCAL PROBLEM: The American College of Surgeons National Surgical Quality Improvement Program-Pediatrics (ACS NSQIP-P) is a nationally validated, risk-adjusted national outcomes registry. At our institution, ACS NSQIP-P data identified our institution as a high outlier in transfusion for neuromuscu

The Traffic Light Protocol: Preventing the 90° 'Point of No Return' Through Risk-Stratified Spinal Surveillance in Children with Cerebral Palsy.

Background: Cerebral palsy (CP) is the leading cause of permanent physical disability in children. Although hip surveillance is a global standard, spinal surveillance remains inconsistent, often leading to reactive rather than proactive management of neuromuscular scoliosis. This study aims to establish an international consensus on a risk-based spinal surveillance protocol. Methods: A three-round modified Delphi process was conducted in 2024 with 15 international pediatric spine surgeons, identified through purposive sampling. The process adhered to CREDES standards and focused on establishin

Health-State Utility Values in CP Patients Following Deformity Surgery: Are We Now Ready for Cost-Utility Analysis in This Patient Population?

Background: Cost-utility analysis (CUA) is frequently used by reimbursement agencies and national advisory bodies to make informed decisions on whether or not to reimburse surgical interventions. Health state preferences (utilities) are a key component in valuing health outcomes in that they are used in calculating quality-adjusted life-years (QALY). Unfortunately, disease-specific HRQoL measures commonly lack the preference weights necessary to produce health-state utility values for use in CUA. A solution to this problem is to map a disease-specific quality-of-life measure to a generic prefe

Orthopedic treatment of neuromuscular scoliosis: Benefits of biomechanically optimized trunk orthoses using the neuroBrace system as an example.

Neuromuscular diseases, categorized by muscle tone into hypotonic and hypertonic groups, frequently result in spinal deformities. Both groups exhibit similar patterns of spinal deformities, typically presenting as kyphoscoliosis or lordoscoliosis, especially in the thoracolumbar region. Trunk orthoses play an important role in the treatment of neuromuscular scoliosis. They can slow disease progression, stabilize the torso, and improve respiration. In this context, the literature often mentions the Boston brace and the Milwaukee brace, and so-called flexible trunk orthoses. These are almost alw

Predictors for postoperative gastrointestinal complications in pediatric non-idiopathic scoliosis patients.

PURPOSE: Non-Idiopathic scoliosis (NIS) patients reportedly have higher postoperative complication rates compared to those with adolescent idiopathic scoliosis (AIS). However, there are limited data on the prevalence of gastrointestinal (GI) complications postoperatively, and the preoperative indicators that may predict them. This study aimed to determine the incidence and risk factors of postoperative gastrointestinal (PG) complications after neuromuscular/syndromic scoliosis corrective surgery. METHODS: 163 NIS patients underwent spine deformity corrective procedure between 2005 and 2020. R

Surgical management of scoliotic deformities related to spina bifida: complication rates, radiographic parameters, and quality of life.

PURPOSE: Spina bifida is a serious condition with numerous comorbidities, leading to severe scoliotic deformities. Several surgical techniques can be considered for the correction of scoliosis but may be associated with complications. Our hypothesis is that the isolated anterior approach significantly reduces the rate of infectious complications. The primary objective of our study is to define the rate of complications according to the surgical technique. Secondary objectives are radiographic results and quality of life scores. METHODS: Thirty-six patients aged less than 18 years with spina b
