Neuromuscular scoliosis: a case of the pediatric patient in the adult ICU.

Neuromuscular scoliosis can be a problem in children with underlying neuromuscular conditions such as cerebral palsy, spina bifida, and muscular dystrophy. A comprehensive preoperative assessment is essential to provide comprehensive postoperative care. Surgical procedures to correct neuromuscular scoliosis include anterior spinal fusion, posterior spinal fusion, or a combined anterior-posterior spinal fusion. Postoperative problems can include respiratory failure, hemodynamic instability, neurovascular compromise, and pain control. With an understanding of the developmental status of these pa

Wound infection after spinal fusion in children with cerebral palsy.

One hundred and seventy-two children with cerebral palsy were operated on for neuromuscular scoliosis by spinal fusion with unit rod instrumentation between January 1988 and June 1996. There were 15 (8.7%) postoperative wound infections (seven deep, eight superficial) in 15 patients (five males, 10 females) who had a mean age of 13.9 years. The mean follow-up after diagnosis of infection was 3.3 years (range, 1-7.2). Twelve of the 15 infected cases, including all seven deep infections, occurred in the distal portion of the incision. In 14 patients, the wound infections were diagnosed within th

Curve patterns and etiologies of scoliosis: analysis in a university hospital clinic in Taiwan.

BACKGROUND: The curvature characteristics and etiologies of scoliosis have been well documented in many countries and among different races but have seldom been reported for Asians. We aimed to determine the etiology frequency and curvature characteristics of scoliotics in Taiwan. MATERIALS AND METHODS: From July 1994 through December 1997, 648 new patients with scoliosis were examined. A whole spine standing radiographic study was taken for each patient. The data were collected prospectively for analysis. RESULTS: Idiopathic scoliosis was the leading type in our series (86%), followed by co

Results and morbidity in a consecutive series of patients undergoing spinal fusion for neuromuscular scoliosis.

STUDY DESIGN: A retrospective clinical and radiographic review. OBJECTIVES: To provide current data on the results and complications of patients who have undergone spinal fusion for neuromuscular scoliosis at a center with physicians experienced in these types of cases. SUMMARY OF BACKGROUND DATA: The reported complication rate in the management of neuromuscular scoliosis ranges from 44% to 62% in the recent literature. This literature is that of 1991 or earlier reflecting operative techniques of the mid-1980s, and it has been used to argue against the efficacy of neuromuscular spinal fusion

Spinal cord monitoring in operations for neuromuscular scoliosis.

We reviewed retrospectively the role of monitoring of somatosensory spinal evoked potentials (SSEP) in 99 patients with neuromuscular scoliosis who had had operative correction with Luque-Galveston rods and sublaminar wiring. Our findings showed that SSEP monitoring was useful and that a 50% decrease in the amplitude of the trace optimised both sensitivity and specificity. The detection of true-positive results was higher than in cases of idiopathic scoliosis, but the method was less sensitive and specific and there were more false-negative results. In contrast with the findings in idiopathic

Rib-vertebral angle asymmetry in idiopathic, neuromuscular and experimentally induced scoliosis.

The concave and convex rib-vertebral angle (RVA) at levels T2-T12 was measured on AP radiographs of 19 patients with right convex idiopathic thoracic scoliosis and 10 patients with major thoracic right convex neuromuscular scoliosis. The difference between the angles on the concave and the convex sides, the RVAD, was calculated. The RVAs were also measured on radiographs from three animal groups in which spinal curves had been induced experimentally in a variety of ways. Group 1 comprised 16 rabbits that had been subjected to selective electrostimulation of the latissimus dorsi, the erector sp

Revision spine surgery in children with cerebral palsy.

Ten children with neuromuscular scoliosis and pelvic obliquity had revision spinal instrumentations and fusions performed at an average age of 14.7 years. The initial spinal deformity in all children was the result of static encephalopathy with nine children diagnosed as having spastic cerebral palsy without another specific diagnosis and one child diagnosed as also having Noonan syndrome. All 10 children had a revision performed after the first surgery, and two of the 10 children underwent a second revision because of failure of rod cross-links. The indications for revision surgery were sympt

Clenbuterol, a beta2-adrenoceptor agonist, reduces scoliosis due to partial transection of rat spinal cord.

Injury to the spinal cord often results in abnormal lateral curvature of the spine, or scoliosis, that is associated with neuromuscular weakness. The lateral curvature of the spine is thought to be a consequence of insufficient or asymmetrical loading of the vertebrae. To study neuromuscular scoliosis, an animal model of spinal cord injury was used in which the spinal cord was partially (3/4) transected, with the left lateral columns left intact. Partial transection of the spinal cord in the rat caused scoliosis that was maximal four to five vertebrae distal to the lesion site. As in previous

Iliosacral screw fixation for pelvic obliquity in neuromuscular scoliosis. A long-term follow-up study.

STUDY DESIGN: This was a retrospective review of a consecutive series of patients with neuromuscular spinal deformity who underwent posterior fusion and pelvic fixation using a long construct and an iliosacral screw. OBJECTIVES: To evaluate the risks and benefits of iliosacral screw fixation. SUMMARY OF BACKGROUND DATA: Neuromuscular scoliosis with pelvic obliquity poses one of the most challenging instrumentation problems, mainly because of the poor bone quality frequently found within the sacrum. Complications include failure of instrumentation, loss of sacral fixation, loss of lumbar lord

A study of desmopressin and blood loss during spinal fusion for neuromuscular scoliosis: a randomized, controlled, double-blinded study.

BACKGROUND: Studies examining the use of desmopressin acetate (DDAVP) have shown variable results in DDAVP's efficacy for reducing blood loss. Studies of adults having cardiac surgery and of children having spinal fusion have suggested that patients with complicated medical histories and complex surgical procedures may benefit from use of DDAVP. Therefore, this study was designed to examine the homeostatic effects of DDAVP in children with severe cerebral palsy undergoing spinal fusion. METHODS: A randomized, double-blinded, and placebo-controlled trial of DDAVP was designed to enroll 40 pati

Neuromuscular scoliosis: diagnostic and therapeutic considerations.

Neuromuscular scoliosis in a child presents as a diagnostic and therapeutic dilemma to the pediatric neurologist, the pediatric neurological surgeon, and the pediatric orthopaedist. Because of its progressive nature, it tends to be refractory to conservative management and requires active intervention. Neuromuscular scoliosis is a symptom, and the diseases leading to the deformity must be addressed together with the spinal abnormality. The framework of the diagnosis and management is addressed in this article.

Modification of the original Luque technique in the treatment of Duchenne's neuromuscular scoliosis.

Luque segmental instrumentation with Galveston technique for pelvic fixation is generally used in the correction of Duchenne's neuromuscular spinal deformities with pelvic obliquity. Particularly difficult is the control of the lumbopelvic junction. Instrumentation failures and only mediocre correction of pelvic obliquity are reported. To obtain better sacropelvic anchorage and to improve pelvic correction, this technique was modified with the introduction of sacral screws in each S-1 pedicle and a device for transverse traction between the caudal right-angle bends of the L-rods. From 1988 and

[The effect of halo-gravity traction in the preoperative treatment of neuromuscular scoliosis].

The purpose of this study is to evaluate the influence of halo-gravity-traction on paralytic scoliosis in various neurologic diseases. Between 1980 and 1993 preoperative halo-gravity-traction was applied in 32 patients with paralytic scoliosis (23 patients with myelomeningocele, 6 patients with poliomyelitis, 3 patients with cerebral palsy). In the myelomeningocele group the average curvature before treatment was 97.8 degrees, after surgery 45.1 degrees; which is an improvement of 53.9%. Halo-gravity-traction accounted for 12.8% improvement. In the poliomyelitis group the average curvature bef

Development of an instrument for clinical evaluation after surgery for neuromuscular scoliosis.

Surgical treatment for neuromuscular scoliosis is effective for most patients. Although those afflicted constitute a heterogeneous group, the aim of surgical treatment is approximately the same for all patients: a spine balanced in the coronal and sagittal planes over a level pelvis. Surgery results in a more stable and straighter spine, which should in turn improve performance in different activities. Previous evaluations of surgery for neuromuscular scoliosis reported in the literature have focused primarily on Cobb angles; there are very few studies dealing with the ability to perform vario

Same-day versus staged anterior-posterior spinal surgery in a neuromuscular scoliosis population: the evaluation of medical complications.

The medical complications occurring in 29 patients with neuromuscular spinal deformity undergoing two-stage anterior-posterior spinal fusion ("staged") were compared with 16 neuromuscular patients undergoing single-stage anterior-posterior spinal fusion ("same day"). Thirty-six (124%) major and minor medical complications occurred postoperatively in the staged patients, whereas 14 (88%) major or minor complication were present in same-day surgery patients. Thirty-five percent of staged patients had no complications, whereas 63% of same-day patients were without complications. Associated findin

Posterior instrumentation and fusion of the thoracolumbar spine for treatment of neuromuscular scoliosis.

We reviewed the clinical and technical outcomes of 25 patients with neuromuscular scoliosis, who were treated by Luque instrumentation and posterior spinal fusion from the upper thoracic spine to L5 between 1981 and 1988. A mean curve correction of 46% was obtained operatively with a mean 8 degrees loss of correction during the follow-up period that ranged from 1.9 to 9.4 years (mean, 5.5). Pelvic obliquity was improved 50% from a mean of 16.1 degrees to a mean of 8.1 degrees in 24 patients for whom data were available. At final follow-up, the mean pelvic obliquity increased to 11.4 degrees wi

Impact of orthoses on the rate of scoliosis progression in children with cerebral palsy.

This study was undertaken to determine the impact of spinal bracing on curve pattern and the rate of progression of neuromuscular scoliosis in children with cerebral palsy. Twenty-one patients were treated with a Wilmington custom-molded orthosis with 23 h a day brace wear for a mean bracing period of 67 months (range, 22-173). Twenty-two patients had a similar follow-up to spinal fusion but had no bracewear. Patients who were braced had a scoliosis that reached a magnitude of 50 degrees at a mean 12.5 years, compared with a mean age of 14 years for those who were not braced (p > 0.05). Spinal
