Comparative Analysis of Inflammatory Response in Surgical Wound Drainage Fluid in Scoliosis Surgery: A Study of Neuromuscular vs. Idiopathic Patients.

This study examines immune and inflammatory responses in draining wound fluid over the course of the early stages of wound healing in patients recovering from spinal fusion surgery. The inflammatory phase of wound healing is essential for setting the stage for successful tissue repair and preventing chronic or poorly healing wounds. Scoliosis can be idiopathic or occur secondary to neuromuscular disorders, which are known to be associated with poor wound healing outcomes. We hypothesised that neuromuscular scoliosis patients would exhibit differences in inflammatory wound healing markers compa

Comparison of free-hand technique and patient-specific guiding template for pedicle screws positioning in adolescent idiopathic scoliosis surgery using neurophysiological monitoring.

INTRODUCTION: Screws mispositioning is an identifiable cause of intraoperative neurophysiological monitoring changes. To prevent screw mispositioning, a patient-specific guiding template for pedicle screw positioning (PSGT) was introduced to avoid implant mispositioning. The study aims to compare the changes in neurophysiological monitoring and screw mispositioning between free-hand (FH) and PSGT in patients surgically treated for adolescent idiopathic scoliosis (AIS). MATERIALS AND METHODS: All patients treated between January 2016 and December 2021 for AIS surgery, aged 14 to 20, were inclu

Hip pain after spinopelvic fusion in pediatric neuromuscular scoliosis.

PURPOSE: Spinopelvic fusion with sacral-alar-iliac (S2AI) screws effectively corrects spinal and pelvic deformities in neuromuscular scoliosis (NMS). However, postoperative hip pain may compromise functional outcomes. This study investigates the incidence and risk factors for hip pain after spinopelvic fusion in pediatric NMS patients. METHODS: We retrospectively reviewed 73 nonambulatory pediatric NMS patients who underwent spinopelvic fusion with S2AI screws from 2013 to 2022. Patient records were analyzed for hip symptoms, demographics, and radiographic parameters using logistic regression

Does Pelvic Fixation in Neuromuscular Scoliosis Correction Affect Radiological and Functional Outcomes?

Pelvic fixation is commonly used in neuromuscular scoliosis patients with pelvic obliquity (PO) >15°, yet its benefit over shorter constructs remains unclear. This systematic review analyzed five studies comprising 230 patients (120 with pelvic fixation (WPF), 110 without pelvic fixation (WoPF)) to assess radiological, functional, and surgical outcomes. Radiographic correction was similar between groups: Cobb angle improved from 62.8° to 32.4° in WPF vs. 76.7° to 29.4° in WoPF (p = 0.54), and PO correction did not differ significantly (final PO 8.6° vs. 11.5°, p = 0.5). Functional outcomes wer

Bipolar spinal correction reduces transfusion rate and hospital stay compared with traditional posterior spinal fusion in children with scoliosis secondary to cerebral palsy.

PURPOSE: Patients with neuromuscular scoliosis due to Cerebral Palsy (CP) are complex due to their curve severity and associated medical comorbidities. Spinal fusion is associated with high risk of peri-operative complications and large blood loss requiring allogenic red blood cell (RBC) transfusion. In June 2017, our tertiary paediatric institution implemented a new minimally-invasive approach for the treatment of neuromuscular scoliosis, Bipolar Spinal Correction (Bipolar), a technique first published by Miladi et al. in 2018. This study represents an analysis of the initial experience of th

Severe Cerebral Palsy and Short Stature Predict Absent Baseline IONM Signals in Pediatric Neuromuscular Scoliosis Surgery.

BACKGROUND: Intraoperative neurophysiological monitoring (IONM) is essential for detecting potential neurological injury during scoliosis surgery, but obtaining recordable baseline signals can be challenging in neuromuscular scoliosis (NMS) patients. Absent baseline IONM signals, characterized by unattainable initial IONM responses despite technical and anesthetic optimization, present significant challenges to intraoperative neurological assessment and surgical risk stratification. This study aims to identify predictive factors for absent baseline IONM signals in pediatric NMS patients and es

Long-term outcomes of the spinopelvic "reverse T-construct" combined with magnetically controlled growing rods for the treatment of neuromuscular scoliosis.

OBJECTIVE: Neuromuscular scoliosis (NMS) typically develops at an early age, leading to postural instability and cardiopulmonary complications due to spinal deformity and pelvic obliquity. Long-term comfort requires surgical intervention to restore spino-pelvic balance. This study aimed to report the long-term outcomes of a reverse T-construct pelvic fixation technique combined with magnetically controlled growing rods for the pediatric NMS treatment. METHODS: Between 2016 and 2023, 34 patients with NMS underwent surgery using the reverse T-construct using two sacral screws and two iliac scre

Comparison of Different Bipolar Construct Configurations for the Correction of Adult Spine Deformity: A Finite Element Analysis.

PURPOSE: A minimally invasive bipolar spinal fixation was recently developed to correct the deformity in pediatric neuromuscular scoliosis and has recently been adapted for adult scoliosis. Although the clinical results are promising, mechanical complications are still not negligible. In this work, alternative configurations of bipolar constructs were compared through numerical simulation, in order to evaluate stress distribution along the implant according to each configuration. METHODS: The configurations included doubling the rods, adding lumbar screws to strengthen the distal anchorage, a

Paediatric neuromuscular scoliosis and post-operative blood pressure targets: a retrospective analysis.

PURPOSE: This study investigates blood pressure variations and clinical outcomes in paediatric neuromuscular scoliosis patients after deformity correction surgery to mitigate the risk of ischaemic spinal cord injury (SCI). Hypotension is proposed as an aetiological mechanism for delayed SCI, but there is limited evidence regarding the frequency, severity, duration, and clinical effect of hypotension exposure in the immediate post-operative period. METHODS: This is a retrospective review of 94 patients with cerebral palsy or CP-like conditions, who underwent posterior spinal instrumentation at

Use and efficacy of antifibrinolytic agents in patients undergoing growth-friendly surgery for neuromuscular scoliosis.

INTRODUCTION: There is a paucity of data on the use, efficacy, and safety of antifibrinolytic agents (AF) in patients with neuromuscular scoliosis undergoing growth-friendly instrumentation. Previous studies have shown mixed results of AF agents in young patients with neuromuscular conditions, and other authors have expressed concerns regarding adverse effects in this medically fragile population. The purpose of this study was to investigate the rate of use of AF agents for growth-friendly surgery in patients with neuromuscular scoliosis, and assess its impact on blood loss and transfusion req
