Segmental spinal instrumentation with sublaminar fixation is widely used but a variety of materials and techniques are being employed for both scoliosis and other spinal surgery. This study addresses the question as to which is best. Both the materials and the methods of securing them were studied. The materials tested were stainless steel 316L, Ortron 90 high nitrogen steel, titanium alloys, nylon straps, and Mersilene tapes. Thirty-four nonambulatory patients with progressive neuromuscular spinal deformity were surgically managed using a 1/4" U-shaped double rod construct with segmental instrumentation from T2 to the pelvis accompanied by posterior spinal fusion. The authors report 8 cases of leg length discrepancy in young adults, treated surgically by shortening or equalization techniques. The ages ranged from 16 to 35 years and the discrepancy from 2.5 to 12 cm. Three techniques were used; subtrochanteric femoral shortening (4 cases), equalization by shortening one femur and lengthening the other at the same operation (2 cases), diaphyseal "Z" osteotomy with shortening (2 cases - one femur, one tibia). Histochemical studies of the thoracic part of the erector spinae muscles in scoliosis have shown a consistently higher proportion of Type 1 fibers on the convex side. The fiber distribution of the multifidus and semispinalis muscles was studied in adolescent idiopathic scoliosis (AIS). The results of management of 14 patients with Stage IV-S neuroblastoma are reported. The treatment policy, although not consistent over this time span, in general used a combination of radiotherapy and chemotherapy or infrequently one modality alone. Fractures of the transverse processes of the lumbar vertebrae may be difficult to identify radiographically because they are often obscured by gas and feces. With unilateral fractures, there is often scoliosis convex to the side of the fractures. We propose that this may be due to the ineffective action of the ipsilateral quadratus lumborum muscle which inserts into the transverse processes. 111 patients with amputation of the thigh of the last world war were examined. Besides the complaints in their thigh they suffered mostly from low back pain and osteoarthritis in the knee of the other leg. For a medical report it is important to decide whether the low back pain comes from lumbar disc disease or from secondary scoliosis of the lumbar spine which is caused by the amputation. Secondary changes in the spine and in the other leg can also be expected when a lower limb is heavily injured.
