Personalized Lumbar Vertebrae Modeling for Dynamic Assessment of Idiopathic Scoliosis.

Clinical assessment of idiopathic scoliosis (IS) patients primarily relies on static imaging techniques. Dynamic digital human (DDH) can provide comprehensive spatio-temporal information for dynamic assessment of the scoliotic spine in IS patients comparing with static imaging techniques, such as X-ray for general assessment and computed tomography (CT) for surgical planning. The lumbar vertebrae exhibit greater morphological variability than the thoracic region when subjected to different postures and mechanical loads, making them particularly important for dynamic assessment. Therefore, a pe

The Effect of a Schroth Method-Based Exercise Program, on Enhancing the In-Brace Correction on Individuals with Idiopathic Scoliosis: A Retrospective Cohort Study.

Conservative treatment with a brace has shown significant improvement in patients with idiopathic scoliosis. Initial in-brace correction can serve as a predictive factor for the treatment outcome. Additionally, significant data highlight the effectiveness of Schroth exercises. There is also evidence that the combination of Schroth and Bracing can provide a better treatment outcome. The study aimed to investigate the effect of a Schroth exercise program on the initial in-brace correction and in a 6-month follow-up in-brace radiological evaluation. Fifty children aged 10-17 years, diagnosed with

Evolving Concepts in Adult Spinal Deformity Surgery.

Adult spinal deformity is a multifaceted condition increasingly encountered in aging populations, often complicated by coexisting hip and knee osteoarthritis. It is important to present an integrated review of biomechanical, surgical, and technologic advances shaping modern adult spinal deformity management. First, the hip-spine-lower extremity relationship and how lower limb pathology impairs compensatory mechanisms, alters spinopelvic alignment, and influences surgical outcomes should be examined. Next, the evolution of classification systems and alignment philosophies-ranging from the Scoli

The Multifaceted Role of p53 in Musculoskeletal Diseases: A Comprehensive Review.

BACKGROUND: While extensively studied for its central role in tumor suppression (regulating cell cycle, DNA repair, and apoptosis), the p53 gene is now recognized for its significant involvement in musculoskeletal diseases. Aberrant p53 expression and function are key factors in the pathogenesis and progression, although the specific mechanisms and clinical change potential remain incompletely defined. OBJECTIVE: This review systematically summarizes the multifaceted roles of p53 in major musculoskeletal diseases, analyzes the molecular mechanisms driving disease progression, and evaluates it

The Impact of an Anterior Lumbar Interbody Fusion on Coronal Alignment in Adult Spinal Deformity Surgery.

BACKGROUND AND OBJECTIVES: This study examined the impact of anterior lumbar interbody fusion (ALIF) on coronal alignment, before and after the placement of posterior instrumentation, and the changes in coronal alignment based on patient positions in adult spinal deformity surgery. METHODS: A total of 65 consecutive adult spinal deformity patients with scoliosis presenting with a main curve cobb angle ≥20° undergoing thoraco-lumbar fusion with 1-2 levels of ALIF performed at L5-S1 or L4-S1 were retrospectively reviewed. L4 tilt, L5 tilt, and fractional curve (FC) cobb angle were measured to a

Semi-automated end-to-end quality control system for annotation data based on automated Cobb angle measurement.

PURPOSE: The field of automated scoliosis diagnosis necessitates large-scale data annotation; however, existing annotation practices are hampered by the lack of a systematic, end-to-end quality control framework. To address this gap, we aim to develop a Semi-Automated End-to-End QC (SAE-QC) System designed to ensure annotation accuracy while substantially accelerating the data collection pipeline and reducing operational costs. This system is expected to provide critical support for the construction of high-quality datasets and the development of reliable AI models in the domain of automated C

Circulating microRNA signatures for diagnosis and prediction of curve progression in pediatric patients with idiopathic scoliosis.

BACKGROUND: Idiopathic scoliosis (IS) is the most common pediatric spinal deformity, yet no biomarker currently enables early diagnosis or reliable prediction of progression to guide individualized treatment. Circulating microRNAs (miRNAs) are promising non‑invasive biomarkers reflecting multifactorial disease mechanisms. METHODS: In our prospective monocentric study, a Czech cohort comprising 114 pediatric IS patients at the time of diagnosis and 89 age‑matched healthy controls was studied. Risk groups were defined based on the final Cobb angle at the end of follow-up at skeletal maturity. P

Single-Stage Press-Fit Femoral Osseointegrated Limb Replacement: A Prospective Cohort Study.

BACKGROUND: Press-fit femoral osseointegrated limb replacement (FOLR) allows for a direct transcutaneous skeletal connection between an artificial leg and the residual femur in a single-stage procedure that can be performed open or percutaneously. A skeletally anchored prosthesis can offer enhanced mobility, balance, and proprioception to amputees, as well as eliminate problems associated with socket mounted prostheses, such as skin problems, ulcers, and pain. The purpose of this research is to describe the safety, functional, and patient-reported outcomes for this technique. METHODS: We retr

Arthrogryposis as a neuromuscular phenotype: lessons from PIEZO2 loss-of-function.

Arthrogryposis multiplex congenita is a heterogeneous group of disorders characterised by multiple joint contractures resulting from impaired fetal movement. A 43-year-old woman presented with a long-standing congenital clubfoot, progressive scoliosis since childhood and distal joint contractures. She had pure sensory findings, with global areflexia, severe sensory ataxia and a positive Romberg sign, significantly affecting her balance and gait. Genetic analysis identified two likely pathogenic variants in the PIEZO2 gene, consistent with an autosomal recessive inheritance pattern. We discuss

Achieving Breast Symmetry in a Patient With Neurofibromatosis-1: A Case Report and Literature Review.

There is limited literature describing the management of breast deformities secondary to neurofibromatosis-1. We report the management of breast asymmetry and distortion secondary to neurofibromatosis of the chest wall and spine. A 24-year-old woman presented with breast asymmetry as a result of scoliosis, chest wall asymmetry, and soft-tissue distortion secondary to neurofibromatosis-1. The right breast displayed loss of projection, effacement of the inframammary fold, and caudal displacement. Ptotic tissue on the right hemithorax immediately caudal to the breast was folded superiorly in the

A fully autonomous AI system for accurate and reproducible Cobb angle measurement in adolescent idiopathic scoliosis: a multicenter study.

BACKGROUND CONTEXT: The gold standard diagnostic test for Adolescent idiopathic scoliosis (AIS) involves manually measuring the spinal column deformity by determining the Cobb angle on a full-spine X-ray image. This measurement involves a subjective interpretation of vertebrae position and angle calculation, and inter-observer variability is widely accepted as one of the main causes of diagnosis uncertainty. PURPOSE: Our objective was to develop an automated and reproducible system based on artificial intelligence (AI) to assist in Cobb angle estimation on full-spine radiographs without human

Hemimyelomeningocele: a systematic review and meta-analysis of the literature.

Hemimyelomeningocele (HMM) is a rare split cord malformation where only one hemicord forms a myelomeningocele-like sac, and the opposite hemicord undergoes normal neurulation. We aimed to compile all published HMM cases to concisely summarize embryology, presentation, imaging, associated anomalies, management, and outcomes of the disorder. Following PRISMA 2020, we searched PubMed/MEDLINE, Scopus, Web of Science, and Google Scholar (1968-Feb 2025) for studies with confirmed human HMM, extracting clinical, radiological, surgical, and follow-up data. Of 688 records screened, 25 articles met incl

Sagittal alignment goals in adult spinal deformity surgery: a narrative review focusing on proximal junctional complications and clinical outcomes.

Adult spinal deformity (ASD) is a complex condition associated with significant disability and reduced health-related quality of life (HRQOL). Surgical correction has increasingly emphasized restoration of sagittal alignment; however, the optimal radiographic targets and their relationships to clinical outcomes and mechanical complications remain subjects of debate. This narrative review summarizes five major alignment strategies in ASD surgery and examines their relevance to HRQOL and the prevention of proximal junctional kyphosis/ failure (PJK/PJF). The Scoliosis Research Society-Schwab clas
