Changes in statin therapy eligibility under the 2026 versus 2018 dyslipidemia guidelines.

BACKGROUND: The 2026 ACC/AHA Dyslipidemia Guideline replaced the 2018 guideline and adopted PREVENT-ASCVD as the preferred primary prevention risk framework. Although prior analyses applying PREVENT-ASCVD alone suggested that statin eligibility may decrease because of lower absolute risk estimates compared with the Pooled Cohort Equations, the population-level impact of the full 2026 guideline framework remains unclear. METHODS: We analyzed adults from the 2017-2020 National Health and Nutrition Examination Survey to assess statin eligibility under the 2018 and 2026 ACC/AHA guideline framewor

Rare LRRFIP1 variants contribute to pathophysiology of severe early-onset scoliosis and common scoliotic phenotypes.

Despite enormous advances in clinical genomics, idiopathic scoliosis remains an enigmatic condition with poorly understood genetic and pathophysiological underpinnings impeding molecular diagnosis and the development of targeted treatments. We performed linkage analysis, exome, genome and short- and long-read RNA sequencing in a multigenerational family affected by autosomal dominant early-onset scoliosis (EOS) with a unique pattern of spondylodysplastic elements and progressive endplate erosion and identified the LRR binding FLII interacting protein 1 gene (LRRFIP1) as the disease-causing gen

Recommendations of the German, Austrian and Swiss rheumatology and other medical societies for the management of polymyalgia rheumatica.

OBJECTIVES: Polymyalgia rheumatica (PMR) is the second most frequent inflammatory rheumatic disease in older adults, after rheumatoid arthritis. Recommendations for the management of PMR, developed by the German, Austrian and Swiss societies of rheumatology and other organizations were published in 2018. METHODS: These recommendations are now updated based on a systematic literature search that covered the evidence published between July 2016 and January 2024 on therapeutic interventions and prognostic factors for PMR. The search string followed that used in the original recommendations, with

Shifting Thresholds: Changes in Antihypertensive Eligibility Under the 2025 Versus 2017 Hypertension Guidelines.

BACKGROUND: Hypertension affects nearly half of U.S. adults. The 2025 American College of Cardiology/American Heart Association guideline adopts the Predicting Risk of Cardiovascular Disease Events (PREVENT) risk equations and updates treatment recommendations for stage 1 hypertension, potentially altering eligibility for antihypertensive therapy. OBJECTIVES: The primary objective was to quantify changes in antihypertensive treatment eligibility under the 2025 vs 2017 guidelines. Secondary objectives were to characterize adults newly meeting treatment thresholds, assess concordance and discor

Spine in Athletes.

Spinal lesions in athletes can be classified into overuse and acute injuries. Overuse injuries can be found after repetitive spinal load during sport activity; acute trauma injuries of the spine are common in contact sports. We present an overview of the wide spectrum of spinal injuries related to sport activity and review their imaging findings. Concerning overuse injuries, we focus on degenerative spine disease, spondylolysis, pedicle fractures, apophyseal ring injuries, Schmorl's nodes, and Scheuermann's disease. Acute spinal injuries are relatively frequently reported in athletes and may b

Intravenous 3-Dimensional Digital Subtraction Angiography: A Noninvasive, High-Quality Alternative to Cerebral Catheter Angiography.

BACKGROUND AND OBJECTIVES: Arterial angiography remains the gold standard for the assessment of cerebrovascular anatomy and pathology. However, when arterial access is higher risk (patients with extreme tortuosity, type 4 Ehlers Danlos Syndrome, or the young pediatric population), and for long-term follow-up or intraoperative scenarios, intravenous 3-dimensional digital subtraction angiography (IV 3D-DSA) is an acceptable substitute. We assessed our experience in consecutive IV 3D-DSA procedures. METHODS: Five hundred twenty-eight adult and 21 pediatric IV 3D-DSAs were performed at our instit

Associations between CT radiomics analyses and kidney biopsy in patients with kidney disease.

BACKGROUND: Kidney disease is characterized by microstructural alterations that currently require invasive biopsy for definitive assessment. However, it remains unclear to what extent radiomics features extracted from contrast-enhanced CT can non-invasively reflect kidney function and histopathological changes. METHODS: Between October 2020 and May 2025 all patients undergoing kidney biopsies and having CT scans prior to biopsy were retrospectively analyzed. A total of 49 patients (59% female, median age 60 years) were included. Of the included patients, 35 biopsies were performed in native k

Static Muscular Stretching Improves Microvascular Function in the Calf Muscles of Patients with Peripheral Artery Disease.

PURPOSE: Supervised walking exercise is a critical component of rehabilitation for patients with peripheral artery disease (PAD); however, various barriers could limit participation in such programs, highlighting the need for accessible exercise strategies that may facilitate a gradual transition to more intensive physical activity while improving vascular function and walking performance in this population. We examined the hypothesis that static muscular stretching would improve microvascular function and walking ability in patients with PAD. METHODS: Thirty-six PAD patients, classified as F

Consensus Lung Cell Reference: The Collaborative Cross-Consortium and Country Lung Cell Nomenclature Project (C3LCN). An Official American Thoracic Society Workshop Report.

Rapid advances in single-cell technologies now allow measurement of thousands of transcripts and other molecular features of individual cells offering unprecedented insight into lung biology in homeostasis and in disease. The accelerated generation of multimodal data has, however, been accompanied by the reporting of putatively "novel" cell types described without consensus regarding their ontogeny, identity, function, or defining markers. To fully realize the value of the technological advances and to enable rigorous comparison across studies, respiratory research will benefit from standardiz

Barriers and facilitators of brace compliance in adolescents with spinal deformities: a qualitative study.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) and Scheuermann's kyphosis (SK) are common spinal deformities affecting different aspects of teenagers' lives. Conservative treatments such as bracing and exercise are key for managing these spinal deviations before skeletal maturity. This qualitative descriptive study explored factors influencing adolescents' adherence to brace treatment for spinal deformities, with the goal of identifying barriers and facilitators to compliance. METHODS: Seventy-four participants, including 32 adolescents, their parents (27 mothers and 5 fathers), and health

Predictive value of upper instrumented level for proximal junctional Kyphosis in Scheuermann's Kyphosis.

Retrospective study. This study aimed to determine whether selecting Th3 and more distal vertebrae as upper instrumented vertebrae (UIV) affects the incidence of proximal junctional kyphosis (PJK) compared with more proximal levels or among themselves. Patients who underwent surgery for Scheuermann’s kyphosis (SK) between 2020 and 2023 were retrospectively reviewed. Individuals without a previous history of spine surgery and with a minimum follow-up of 2 year were included. A total of 34 patients were included, with 29 (85%) being male. The mean age was 17.67 ± 3.68 year. Patients with higher

Surgical outcomes in adult scheuermann's kyphosis: a matched-cohort comparison with adult idiopathic scoliosis as a clinical benchmark.

PURPOSE: Surgical treatment for Scheuermann's Kyphosis (SK) in adolescents is well-established, but its role in adults remains controversial. This study challenges the traditional non-operative stance by evaluating the clinical outcomes of surgery in adult SK patients, using a matched cohort of adult idiopathic scoliosis (Adult IS) patients as a clinical benchmark. METHODS: A retrospective, 1:3 matched-cohort study from a multicenter database with a minimum 2-year follow-up. Thirty-three SK patients were matched with 99 Adult IS patients by age and number of instrumented levels. Demographics,

Platelet Casein Kinase 2α is a pivotal player in arterial thrombotic occlusion and post-ischemic myocardial remodeling.

AIMS: Platelets play a major role in thrombo-inflammatory cardiovascular diseases such as myocardial infarction. Although platelet function is crucially determined by kinases, the impact of Casein Kinase 2α (CK2α) on platelet activation during arterial thrombosis and myocardial remodeling following ischemia and reperfusion (I/R) injury is not known. METHODS AND RESULTS: Using platelet-specific deletion of Csnk2a1 in mice, the evaluation of the CK2α-dependent platelet phosphoproteome revealed a diminished phosphorylation of the IP3 receptor type-1 in Csnk2a1-deficient mice. This finding was ac

Is vitamin D a risk factor in the development of Scheuermann's kyphosis?

Evaluation of vitamin D (D vit) levels in Scheuermann's kyphosis is important for a better understanding of environmental and metabolic factors that may contribute to the etiopathogenesis of the disease and for revising screening approaches. We aimed to evaluate the relationship between serum D vit, calcium, phosphorus, and alkaline phosphatase (ALP) levels between individuals with Scheuermann's kyphosis and an age- and sex-matched healthy control group. A total of 400 individuals (200 Scheuermann's kyphosis and 200 control), aged between 10 and 18 years, were included. Participants were consi
