Causal relationship between circadian rhythm-related genes expression and spinal disorders: systematic druggable genome-wide Mendelian randomization analysis and virtual screening.

To systematically investigate the causal relationships between circadian rhythm-related genes and the pathogenesis of six spinal disorders (scoliosis, spinal osteochondrosis, instability, stenosis, spondylosis, and spondyloarthritis) using an integrated bioinformatics framework. Mendelian randomization and Bayesian colocalization were applied to cis-eQTL data and GWAS summary statistics. Functional enrichment, protein-protein interaction network analysis, drug prediction, and structure-based virtual screening were subsequently performed. Our research identified 16 potential circadian genes, wi

Barriers and facilitators of brace compliance in adolescents with spinal deformities: a qualitative study.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) and Scheuermann's kyphosis (SK) are common spinal deformities affecting different aspects of teenagers' lives. Conservative treatments such as bracing and exercise are key for managing these spinal deviations before skeletal maturity. This qualitative descriptive study explored factors influencing adolescents' adherence to brace treatment for spinal deformities, with the goal of identifying barriers and facilitators to compliance. METHODS: Seventy-four participants, including 32 adolescents, their parents (27 mothers and 5 fathers), and health

Four-level pedicle subtraction osteotomy for severe rigid thoracic hyperkyphosis: a technical case report.

STUDY DESIGN: Technical case report. INTRODUCTION: Severe thoracic hyperkyphosis caused by multiple osteoporotic fractures can lead to significant disability. When deformities are rigid, surgical correction may be required. This report describes a rare case of four-level pedicle subtraction osteotomy (PSO) to address a fixed deformity, with focus on technique, outcomes, and complications. CASE PRESENTATION: A 65-year-old female with Scheuermann's disease developed progressive thoracic hyperkyphosis due to osteoporotic vertebral fractures. Radiographs showed a rigid kyphosis of 130.3°. The pa

Is Osgood-Schlatter's associated with an increased risk of pars fracture? A retrospective exploratory study of male academy football players.

This retrospective exploratory cohort study investigated the association between Osgood-Schlatter disease and pars interarticularis fractures in elite male academy footballers. Among 341 players, those with a history of Osgood-Schlatter disease were over 19 times more likely to sustain a pars fracture (relative risk: 19.27). Findings suggest Osgood-Schlatter disease may act as a clinically observable risk marker for future spinal pathology. Early identification and management of Osgood-Schlatter disease could help identify those at risk of future pars injuries and may therefore help optimise p

Predictive value of upper instrumented level for proximal junctional Kyphosis in Scheuermann's Kyphosis.

Retrospective study. This study aimed to determine whether selecting Th3 and more distal vertebrae as upper instrumented vertebrae (UIV) affects the incidence of proximal junctional kyphosis (PJK) compared with more proximal levels or among themselves. Patients who underwent surgery for Scheuermann’s kyphosis (SK) between 2020 and 2023 were retrospectively reviewed. Individuals without a previous history of spine surgery and with a minimum follow-up of 2 year were included. A total of 34 patients were included, with 29 (85%) being male. The mean age was 17.67 ± 3.68 year. Patients with higher

Surgical outcomes in adult scheuermann's kyphosis: a matched-cohort comparison with adult idiopathic scoliosis as a clinical benchmark.

PURPOSE: Surgical treatment for Scheuermann's Kyphosis (SK) in adolescents is well-established, but its role in adults remains controversial. This study challenges the traditional non-operative stance by evaluating the clinical outcomes of surgery in adult SK patients, using a matched cohort of adult idiopathic scoliosis (Adult IS) patients as a clinical benchmark. METHODS: A retrospective, 1:3 matched-cohort study from a multicenter database with a minimum 2-year follow-up. Thirty-three SK patients were matched with 99 Adult IS patients by age and number of instrumented levels. Demographics,

3D U-net for volumetric segmentation of spinal cord MRI in degenerative disorders.

The correct identification of spinal cord structures in magnetic resonance imaging (MRI) plays a vital role in identifying degenerative spondyloarthrosis, osteophytes, osteochondrosis, haemangioma, osteophytes and physiological lordosis malformations. Delineation, which is done physically, is time-consuming and can be varied. We will present a 3D U-Net-based deep learning model of volumetric segmentation of spinal cord MRI data in Digital Imaging and Communications in Medicine (.dcm) format in this research. The model takes advantage of three-dimensional convolutions to learn spatial dependenc

Is vitamin D a risk factor in the development of Scheuermann's kyphosis?

Evaluation of vitamin D (D vit) levels in Scheuermann's kyphosis is important for a better understanding of environmental and metabolic factors that may contribute to the etiopathogenesis of the disease and for revising screening approaches. We aimed to evaluate the relationship between serum D vit, calcium, phosphorus, and alkaline phosphatase (ALP) levels between individuals with Scheuermann's kyphosis and an age- and sex-matched healthy control group. A total of 400 individuals (200 Scheuermann's kyphosis and 200 control), aged between 10 and 18 years, were included. Participants were consi

[Diagnostic imaging of the equine cervical spine - Part one: Radiography - A review of the literature on findings and their clinical significance].

A number of indications exist that necessitate radiologic examination of the equine cervical spine. Different neurologic and pain-associated clinical signs within the scope of lameness or poor performance may be associated to pathologies of the cervical spine. Moreover, the number of requests for radiographic examinations of the cervical spine, for example as part of the pre-purchase examination, has increased significantly in recent years. However, the interpretation of radiographs represents a challenge, particularly in the case of clinically sound horses, as it often necessitates the resolu

Computed Tomography Prevalence of Cervicothoracic Endplate Junction Alterations in Dogs.

Spinal degenerative changes can predispose back pain and neurological deficits in dogs. In humans and veterinary patients, the endplate junction has been proposed as an alternative site of failure in addition to damage of the annulus fibrosus, leading to intervertebral disc herniation. Some alterations lead to contour abnormalities with or without regional mineralizations and can be classified as Endplate Junction Failure (EPJF), osteochondrosis (with or without fragmentation), or spondylarthritis. The objective of this retrospective study was to describe the prevalence of endplate junction al

Comparison Between stable sagittal vertebra (SSV) and first lordotic vertebra (FLV) instrumentation for prevention of distal junctional kyphosis in Scheuermann disease: an updated systematic review.

BACKGROUND: Scheuermann kyphosis (SK) is a spinal deformity characterized by exaggerated kyphosis, frequently requiring surgical correction. Selection of the lowest instrumented vertebra (LIV) is critical in optimizing outcomes, with options including the stable sagittal vertebra (SSV) and first lordotic vertebra (FLV). This systematic review evaluates the clinico-radiological outcomes and complications associated with SSV and FLV as LIVs. METHODS: A systematic review were conducted following PRISMA guidelines. Seven studies comprising 311 patients (142 in the SSV group and 169 in the FLV gro

Insights into the Pathophysiology of Scheuermann's Kyphosis: From Structural Deformities to Genetic Predisposition and Underlying Signalling Pathways.

Scheuermann's kyphosis (SK) is a rigid dorsal kyphosis of unclear pathophysiological origin. The aim of this review is to summarise current theories and both clinical and experimental findings regarding the underlying mechanisms of SK. Emerging evidence highlights the significant role of excessive mechanical loading as a major contributor to defective growth of the cartilaginous vertebral endplate. This is associated with the formation of Schmorl's nodes, disruption of the ring apophysis, and compromised intervertebral disc integrity-ultimately resulting in vertebral body wedging and thickenin

Assessing the biomechanics of scheuermann's kyphosis affected thoracolumbar spine in forward flexion at the tissue-level using a finite element model.

Thoracolumbar kyphosis (TLK) secondary to Scheuermann's disease often leads to low back pain, which may be related to altered biomechanical properties of the spine. However, There is a lack of biomechanical studies in the literature that comprehensively evaluate tissue-level stresses and strains in the thoracolumbar spine affected by Scheuermann's kyphosis, particularly during functional motions such as forward flexion. This study analyzed biomechanical changes during forward flexion in TLK patients using musculoskeletal dynamics and finite element modeling. Twenty TLK patients and twenty heal
