BACKGROUND: Long-acting injectable antiretroviral therapy (LAI-ART) offers an alternative to daily oral ART and may reduce pill fatigue, stigma, and daily treatment burden. However, its implementation in rural and non-metropolitan settings is challenged by travel burden, limited local healthcare infrastructure, and privacy concerns. This study evaluated interest in, and preferences for cabotegravir/rilpivirine (CAB + RPV LA) among people with HIV (PWH) living outside a metropolitan area and identified factors influencing uptake.
METHODS: From March 2024 through December 2024 we conducted a cr

BACKGROUND: Community-acquired pneumonia (CAP) is a leading cause of death in adults, but recent data in people with HIV (PWH) are lacking. We studied South African adults hospitalized with CAP to describe mortality and risk factors.
METHODS: We prospectively enrolled consenting adults aged ≥18 years hospitalized with CAP, as defined by symptoms, signs, and chest x-ray, from March 2019 to October 2021 in 3 South African hospitals. Blood, sputum, nasopharyngeal/oropharyngeal swabs, and urine were tested for HIV, tuberculosis, SARS-CoV-2, influenza, respiratory syncytial virus, and Streptococcu

INTRODUCTION: While voluntary medical male circumcision (VMMC) reduces the individual-level risk of HIV acquisition by approximately 60% in randomised-controlled trials, little is known about the 'real-world' long-term effect of medical and traditional male circumcision on the cumulative risk of HIV infection. We estimate the association between these for the first time using a quasi-experimental study design-a household fixed-effects analysis-for sub-Saharan Africa, the global region with the largest HIV burden.
METHODS: We pooled individual-level cross-sectional data from the nationally-rep

BACKGROUND: The diagnosis of talaromycosis, a life-threatening invasive fungal disease endemic in Southeast Asia, still relies on protracted culture methods. We report the development and clinical evaluation of a novel lateral flow assay (LFA) for the rapid diagnosis of talaromycosis.
METHODS: The LFA used 2 novel monoclonal antibodies targeting the Talaromyces marneffei-specific protein Mp1p. In a retrospective, case-cohort study, we evaluated the diagnostic performance of the Mp1p LFA against the reference standard of culture-proven talaromycosis from any clinical specimens, and compared to

Host restriction factors can block HIV at multiple stages, but they are often studied as individual genes or within a single molecular layer. I performed an evidence-structured computational reanalysis of three independent public datasets to determine how the human HIV restrictome varies across clinical states, immune-cell compartments, and protein responses. The analysis used bulk monocyte RNA sequencing (GSE157198), PBMC single-cell/CITE-seq data (GSE256089), and primary CD4 T-cell proteomics (PXD012263) within a prespecified restrictome-immunometabolism framework. In GSE157198, ART-naive mo

BACKGROUND: Despite advances in HIV prevention, achieving elimination in the United States (US) remains challenging, particularly among gay, bisexual, and other men who have sex with men (GBMSM). Psychosocial barriers, including stigma and mental health challenges, influence sexual behaviors and hinder engagement in HIV prevention and care, suggesting that integrated interventions may improve HIV-related service uptake and reduce HIV incidence.
METHODS: We adapted a stochastic network model simulating HIV transmission dynamics in a southeastern US metro to include individual-level sexual stig

BACKGROUND: Differentiating recent from chronic HIV infections is critical for interrupting transmission chains. However, conventional Next-Generation Sequencing (NGS) is confounded by technical errors that inflate genetic diversity, blurring this distinction and compromising downstream molecular epidemiological analyses.
METHODS: We applied Multiplexed Primer ID-Based Next-Generation Sequencing (MPID-NGS) to plasma from 153 antiretroviral therapy (ART)‑naïve newly diagnosed with HIV individuals in Zhuhai, China (2022‑2024) to generate error‑corrected template consensus sequences, from which

INTRODUCTION: Despite systemic inequities in pre-exposure prophylaxis (PrEP) uptake among Latines (a gender-inclusive term for people of Latin American descent, used to promote inclusivity and language justice), clinic-level implementation strategies addressing health equity-related PrEP barriers are underutilized. This multi-method study assessed the feasibility and acceptability of community-co-designed, equity-focused strategies to improve PrEP uptake among Latines in the Southern United States.
METHODS: Facilitated by a community-based organization (CBO)/academic partnership, a workgroup

INTRODUCTION: HIV index case testing is an evidence-based practice in which health care workers (HCWs) counsel people living with HIV (index clients) to offer HIV testing services to their sexual partners and biological children (contacts). For index case testing to be effective, it is important that index- and contact-client counselling protocols are implemented as intended (fidelity). We evaluated whether a digital training implementation strategy improved HCWs' fidelity to these counselling protocols.
METHODS: We conducted a cluster-randomized controlled trial at health facilities (cluster

OBJECTIVE: Emergency department (ED) human immunodeficiency virus (HIV) screening programs are designed primarily to identify persons with undiagnosed HIV and, therefore, systematically exclude persons with diagnosed HIV (PWHDx). PWHDx who are out of care (OOC) contribute disproportionately to HIV transmission and are a priority population within Ending the HIV Epidemic (EHE) initiatives. Because PWHDx OOC are not routinely identified through screening, complementary strategies are needed to support re-engagement in care. We evaluated an electronic health record algorithm (EHRa) designed to id

INTRODUCTION: Integrating HIV pre-exposure prophylaxis (PrEP) into family planning (FP) services offers a strategic opportunity to reach women at risk of HIV acquisition. We assessed fidelity to core components of oral PrEP delivery in real-world FP clinics in Kenya.
METHODS: Data are from the FP Plus Project, a stepped-wedge cluster-randomized trial integrating oral PrEP into 12 public FP clinics in Kisumu, Kenya, where trained FP healthcare providers delivered PrEP services within routine services. Standardized unannounced client actor visits conducted between December 2021 and April 2022,

INTRODUCTION: Black Americans and individuals in the Southern United States bear a disproportionate burden of HIV. To address stigma, a key contributor to these inequities, we developed Cutting Out Stigma (COS), a barbershop-based education and multimedia intervention.
METHODS: We recruited and trained Tennessee (U.S.) barbers as COS Men's Health Ambassadors and invited them to participate in a 6-month COS pilot implementation trial from September 2024 to March 2025. COS barbers provided sexual health education, referrals to HIV (and other ancillary) services for barbershop clients and shared

INTRODUCTION: Long-acting injectable antiretroviral therapy (LAI ART) represents a transformative development in HIV care because it may mitigate longstanding barriers that contribute to gaps in health outcomes. Providing LAI ART to people not consistently on HIV treatment or lacking treatment options requires intentional programme planning to overcome implementation barriers, without which only well-resourced patients benefit, worsening health outcome gaps. The ALAI UP Project delivered clinic-level implementation strategies to support eight clinics across Ending the HIV Epidemic jurisdiction

BACKGROUND: Understanding the preferences of core HIV medical case management (MCM) program staff for long-acting injectable antiretroviral therapy (LAI ART)-related support services could help integrate those services into MCM programs, thus facilitating uptake of LAI ART and increasing opportunities for viral suppression among clients who have struggled with daily oral ART adherence.
SETTING: New York City (NYC)-area Ryan White Part A MCM programs.
METHODS: We conducted a survey among MCM program administrators, clinicians, and non-clinical direct-service providers (hereafter, "providers")

BACKGROUND: Despite advances in HIV treatment, many people living with HIV fail to achieve or maintain viral suppression. Individuals who experience interruptions in HIV care, adherence challenges, and viral nonsuppression are often underrepresented in clinic-based cohort studies. Virtual cohort methodologies offer opportunities to recruit, engage, and retain these populations while integrating longitudinal behavioral, biomarker, and digital engagement data needed to characterize dynamic changes in HIV care engagement and viral suppression.
OBJECTIVE: The Exploring, Predicting, and Intervenin

BACKGROUND: The widespread use of antiretroviral therapy (ART) has shifted a change in hospitalization patterns among people with HIV. However, long term trends and causes of hospitalization among people with HIV (PWH) in Korea remain underexplored. This study aimed to investigate 20-year trends in hospitalization rates and causes among PWH.
METHODS: We retrospectively analyzed hospitalization data from 803 PWH aged ≥ 18 years who received care at the university-affiliated hospital from 2001 to 2020. Hospitalization data were examined across five 4-year intervals. Hospitalization rates per 10

INTRODUCTION: Metabolic dysfunction-associated steatohepatitis (MASH) is more common in people with HIV and associated with systemic inflammation and extrahepatic complications, including cognitive impairment. While cross-sectional studies suggest an association between MASH and worse cognitive outcomes, its impact on longitudinal cognitive trajectories in people with HIV remains unclear.
METHODS: This prospective study included people with HIV aged≥50 years from HIV-NAT, Bangkok, Thailand, between 2015 and 2024. MASLD and MASH were identified at baseline using the FibroScan-AST (FAST) score.

INTRODUCTION: It is increasingly acknowledged that transformative change is needed to effectively address the complex and persistent health challenges facing society today. Realizing transformative change requires multistakeholder processes that co-define and implement research and innovation (R&I) priorities and actions that better respond to the complexity of these challenges in a contextualized manner. However, there is limited insight into the operationalization of participatory systems approaches for identifying the systemic elements of these complex challenges and for defining and implem

BACKGROUND: South Africa has mounted the largest HIV prevention and treatment programme in history, delivering substantial gains in survival and prevention of new HIV infections; however, service disruptions and recent funding losses might jeopardise these successes. There is a scarcity of high-quality, population-based empirical evidence on key epidemic metrics needed to accurately gauge recent progress towards epidemic control in South Africa. We aimed to evaluate recent progress towards HIV epidemic control in South Africa by quantifying long-term trends in key epidemic control metrics in a
