Utilization of Magnetic Resonance Imaging-based Synthetic Computed Tomography for Pedicle Screw Navigation in Children: A Feasibility Study.

BACKGROUND: Pedicle screw instrumentation using a fluoroscopy-assisted freehand technique (FAFT) or using navigation based on a preoperative traditional computed tomography (tCT) scan is a common practice. However, concerns remain regarding the long-term effects of repeated ionizing radiation exposure. In 2025, the first pediatric patients in the USA underwent navigated posterior spinal fusions (PSFs) using magnetic resonance imaging (MRI) converted to CT scan-like quality, known as synthetic CT (sCT). The purpose of this study is to report on the clinical, radiographic, and complication outco

The Lower Lordotic Arch More Precisely Predicts Pelvic Positioning for Adult Spinal Deformity Patients with an Aligned T4-L1-Hip Axis.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To assess the combined effect of lower lordotic arch (LLA) and thoracolumbar (T4-L1-hip axis) alignment correction on the ability to predict sacral slope (SS) (i.e. pelvic retroversion) in adult spinal deformity (ASD). SUMMARY OF BACKGROUND DATA: The Roussouly framework characterizes the spine through a series of lordotic and kyphotic arches. The LLA should be equivalent to SS, meaning that pelvic positioning should be predicted by lumbar lordosis modifications between the lordotic apex and S1. However, positive T4-L1-hip axis mismatches will dr

Influence of surgical approach on sexual dysfunction following single-level lumbar fusion in men: a propensity-matched analysis.

BACKGROUND: Sexual dysfunction represents a meaningful yet understudied complication of spine surgery, with substantial implications for quality of life. Differences in surgical exposure between anterior and posterior approaches may influence these outcomes through variation in anatomic disruption, neural risk, and perioperative course. While existing studies have reported select postoperative sexual complications in broad patient populations, comparative data specifically assessing dysfunction by operative approach among men undergoing single-level lumbar fusion remain sparse. This study comp

Facet tropism is not associated with lumbar spondylolysis: a computed tomography-based three-dimensional morphometric case-control study.

OBJECTIVE: Facet tropism has been proposed as a morphologic modifier of posterior element loading, but prior evidence in lumbar spondylolysis is inconsistent and often based on 2D measurements. This study aimed to evaluate whether facet tropism differs between patients with lumbar spondylolysis and controls using a CT-based three-dimensional (3D) morphometric workflow with vertebra-anchored reference planes. METHODS: This retrospective case-control study included 39 individuals: 19 with CT-confirmed lumbar spondylolysis and 20 controls (age and sex comparable). Vertebrae L1-L5 were segmented,

Effect of Sagittal Imbalance on Functional Disability in Patients with Degenerative Spondylolisthesis.

BACKGROUND: Sagittal imbalance often causes pain and disability in patients with degenerative spondylolisthesis (DS). This occurs due to the slipping of one vertebra over another as a result of degenerative changes in the disc and facet joints. In this study, we aimed to assess the impact of sagittal imbalance on functional disability in these patients. METHODS: This was a single-center cross-sectional study conducted among 50 patients with DS, aged over 45 years, over a period of 1 year. The participants underwent standing lateral radiographs of the spine, with their knees maximally extended

Comparison of Acupuncture and Lumbar Traction for Lumbar Disc Herniation and Subsequent Risk of Lumbar Fusion or Discectomy: A Retrospective Cohort Study.

BACKGROUND: Lumbar disc herniation (LDH) is commonly treated with conservative therapies, but comparative evidence on acupuncture versus lumbar traction and subsequent surgery is limited. We examined whether acupuncture was associated with lower 5-year risks of discectomy or spinal fusion than traction among adults with LDH. METHODS: We conducted a retrospective cohort study using deidentified electronic health records from the TriNetX US Collaborative Network from January 1, 2000, to December 31, 2023. Adults aged 20 years or older with LDH who received acupuncture or traction after diagnosi

Morphological Study of the Foramen Transversarium of the Typical Cervical Vertebrae with Clinical Correlation.

BACKGROUND: The foramen transversarium (FT) is a distinctive anatomical structure in the cervical vertebrae for the passage of the vertebral artery, vein, and sympathetic plexus. The FT is a feature in all cervical vertebrae and is important for normal vertebral physiology. In this article, the anatomy of the FT is presented and discussed with respect to the variability of this foramen, its structural dimensions, and its clinical applications. The relevance for the contents of the FT are important because they contain nerves and blood vessels, and given its intimate relation with the neurovasc

Three-dimensional printed artificial vertebral body versus titanium mesh cage and nanohydroxyapatite/polyamide-66 cage in anterior cervical corpectomy and fusion: a retrospective cohort study with a minimum 5-year follow-up.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) is a commonly performed procedure for cervical spondylotic myelopathy (CSM) and ossification of the posterior longitudinal ligament (OPLL), but the choice of reconstructive device after corpectomy remains unsettled. Titanium mesh cages (TMCs) and nanohydroxyapatite/polyamide-66 (n-HA/PA66) cages are both widely used in clinical practice, although subsidence and postoperative loss of cervical alignment remain important concerns. Three-dimensional printed artificial vertebral bodies (3DP-AVBs) have been introduced as an alternative recon

How Do I: How and When Do I Inject Suboccipital Muscles in Cervical Dystonia.

Cervical dystonia (CD) is characterized by involuntary, sustained or intermittent contractions of neck muscles, leading to abnormal postures, often associated with pain or tremor. Utilizing the COL-CAP framework, the Caput subtype describes the head motion resulting from the action of muscles on the occipito-atlantal (C0-C1) and atlantoaxial (C1-C2) joints; while the Collis subtype describe the neck motion induced by muscle action on the cervical spine (C2-C7). Co-existence of various Caput and Collis subtype combinations in CD is well known. The suboccipital muscles are significant drivers of

Advanced Camera-Based Scoliosis Screening via Deep Learning Detection and Fusion of Trunk, Limb, and Skeleton Features.

Scoliosis significantly impacts quality of life, highlighting the need for effective early scoliosis screening (SS) and intervention. However, current SS methods often involve physical contact, undressing, or radiation exposure. This study introduces an innovative, non-invasive SS approach utilizing a monocular RGB camera that eliminates the need for undressing, sensor attachment, and radiation exposure. We introduce a novel approach that employs Parameterized Human 3D Reconstruction (PH3DR) to reconstruct 3D human models, thereby effectively eliminating clothing obstructions, seamlessly integ

When imitation meets exploration: quantifying social efficiency deficits (SED) in vaccination behaviour.

Vaccination behaviour plays a critical role in shaping epidemic dynamics, yet the societal consequences of decentralized vaccination decisions remain difficult to quantify. We develop a behavioural-epidemiological framework that combines adaptive vaccination dynamics, behavioural exploration and socially desirable vaccination strategies. To evaluate the societal impact of vaccination behaviour, we introduce the social efficiency deficit (SED), a novel metric that quantifies the welfare loss arising from decentralized vaccination decisions and measures the gap between individual incentives and
