Asymptomatic L5 Spondylolysis Does Not Necessarily Preclude Elite Gymnastics Participation: A Case Report.

CASE: An 8-year-old girl world-level rhythmic gymnast was diagnosed with bilateral L5 pars interarticularis defects and was asymptomatic. Imaging showed bilateral spondylolysis without spondylolisthesis or degeneration. Despite recommendations for sport restriction and external scrutiny, she continued elite training under specialist surveillance. Serial imaging over 18 years demonstrated persistent defects without progression. She competed internationally for >10 years without pain or limitation. CONCLUSION: Bilateral pediatric spondylolysis may remain clinically and radiographically stable d

Activity-Related Low Back Pain in the Athlete.

Low back pain (LBP) in the athlete is a common condition that warrants specialized care. Knowledge of the various biomechanical and sport-specific factors remains critical in identifying the underlying etiology of LBP. There are both intrinsic spine causes of LBP, including entities such as degenerative disk disease, spondylolysis, and lumbar disk herniation and extrinsic causes of LBP, most commonly musculotendinous pathology. Accurate diagnosis and timely treatment are critical to a full return to sport. Further, preventative measures must be used to preserve the longevity of the athlete and

Patient-Specific 3D-Printed Reference Rod for "Smiley Face Rod" Repair of Lumbar Spondylolysis in a Patient with High Body Mass Index.

INTRODUCTION: The "Smiley Face Rod" (SFR) method is an effective direct repair technique for lumbar spondylolysis. However, three-dimensional rod contouring is technically demanding, particularly in patients with a high body mass index (BMI), where deep soft tissue obscures anatomical landmarks. TECHNICAL NOTE: We report a technique using a patient-specific three-dimensional (3D)-printed model to create a sterilized "Reference Rod" template. A 27-year-old male with bilateral L5 spondylolysis and severe obesity (BMI 39.8 kg/m2) underwent SFR repair. A 3D model was printed from preoperative com

Mapping the genetically predicted associations between lower-extremity muscle traits and spinal diseases.

PURPOSE: Humans are the only bipedal vertebrates, and the links between the lower-extremity muscles and spinal disorders have been highlighted. However, the exact relationships between them are not fully understood. This study aimed to elucidate the genetically predicted associations between lower-extremity muscle traits and spinal diseases using Mendelian randomization (MR) analysis. METHODS: We used summary statistics from genome-wide association studies for lower-extremity muscle traits from UK Biobank, including thigh muscle volume, thigh muscle fat infiltration, usual walking pace, etc.;

[Not Available].

The surgical management of isthmic spondylolysis and spondylolisthesis in athletes should be individualized, taking into account functional failure, spinopelvic parameters, and the goal of return to play (RTP). Direct pars repair preserves motion and often allows for a faster RTP, whereas in situ L5-S1 fusion remains the standard of care for unbalanced cases, high-grade slips, or neurological involvement, with reported success rates of approximately 80 to 90 %. Minimally invasive and robot-assisted techniques appear promising for reducing morbidity and accelerating recovery, but long-term seri

One in five cases of spondylolysis are missed on MRI: a retrospective analysis of pediatric patients with back pain.

INTRODUCTION: Lumbar spondylolysis is a common cause of pediatric low back pain. Although CT is highly sensitive, MRI is often used first to avoid radiation. Pars defects may be missed on MRI, potentially delaying care. The frequency and clinical impact of these missed diagnoses remain poorly defined. The purpose of this study is to assess how often spondylolysis is missed on MRI reports. METHODS: A single-institution retrospective review was performed of pediatric patients with lumbar spondylolysis treated between 2021 and 2024. All patients underwent MRI for evaluation of low back pain. MRI

Stress Reactions in Bone and Stress Fractures.

The term "stress fracture" encompasses a spectrum of bone injuries resulting from repetitive submaximal loading that exceeds the bone's capacity for repair. This review explores the pathophysiology and clinical classifications of stress-related skeletal injuries, distinguishing between fatigue fractures-occurring when abnormal stress is applied to healthy bone-and insufficiency fractures, which arise from normal physiological stress on weakened bone. I discuss specific manifestations, including spondylolysis, distal clavicular osteolysis, atypical femur fractures, and pregnancy-associated vert

Case report-Stumped: back pain in a cricketer.

A 19-year-old collegiate fast bowler presented with chronic low back pain that acutely worsened during a tournament. Examination suggested a mechanical origin, but plain radiographs showed subtle lucency at L5 and bilateral sacroiliac sclerosis. MRI was interpreted as bilateral sacroiliitis with effusion, raising concern for axial spondyloarthropathy. Laboratory tests, including HLA-B27, were unremarkable, and rheumatological evaluation did not support a seronegative spondyloarthropathy. Computed tomography ultimately demonstrated stress fractures with developing spondylolysis at L4 and L5. Th

Anatomical Variants of the Cervical Spine and Sports-Related Injury in Athletes: A Narrative Review.

Congenital and developmental cervical spine variants, as well as acquired or sport-adapted cervical morphologies, may alter canal reserve, segmental stability, foraminal dimensions, and the cervicothoracic outlet in athletes. This narrative review aimed to integrate the anatomically and methodologically heterogeneous literature linking these findings with sports-related injury and return-to-play decisions. PubMed/MEDLINE was searched from database inception through 6 July 2026 using controlled vocabulary and free-text terms for cervical anatomy, specific variants, athletic exposure, neurologic

Training activities associated with lumbar pain exacerbation in competitive swimmers with prior lumbar spine injury: a cross-sectional survey.

Competitive swimmers often accumulate high training volumes that place repetitive stress on the lumbar spine, potentially worsening injury and pain. This study examined training activities associated with self-reported lumbar spine pain exacerbation in competitive swimmers with prior lumbar spine injury. We conducted a retrospective cross-sectional survey of 285 swimmers (mean age 18.7 ± 3.1 years) assessing demographics, training exposures, and injury history. 29 swimmers (10.2%) reported a prior lumbar spine injury, including disc pathology (20.7%), spondylolysis/spondylolisthesis or facet p
